Abstract

 Background & Objective: Hypertension and diabetes mellitus coexist more commonly than predicted by chance and the synergistic effect of these two diseases is the cause of early disability and high rate of mortality in these patients. Controlling the blood pressure has a critical role in decreasing cardiovascular mortality in diabetic hypertensive cases. The goal of this study is to determine the rate of recognition and treatment of hypertension in type II diabetic subjects referring to diabetes clinic of Bou-ali hospital, Ardabil.

 Methods: In this cross-sectional study 300 type II diabetic patients were examined in terms of blood pressure control.

 Results: The patients who participated in this study ranged between 28 to 80 years of age (mean: 55.5 ± 11.4). About 61.6% of diabetic patients had hypertension (BP>140/ 90 mmHg) 76.7% of these patients were aware of their hypertension. Only 8.8% of these patients with hypertension had a well-controlled blood pressure and the rest of them (91.2%) had a poorly- controlled blood pressure (BP>130/80 mmHg). About 21.8% of hypertensive diabetics although aware of their hypertension were not on any anti- hypertensive agents.

 Conclusions: This study indicates poor control of hypertension in diabetic patients. It seems that reconsidering CME programs for the physicians, emphasizing the control of risk factors especially in high-risk groups and educating the diabetic patients will improve the control of hypertension in these patients.

Abstract

  Background & Objectives: Hypertension is one of the major factors of cardiovascular diseases among hemodialysis patients which shoes its effect along with vascular changes. 75-80% of the patients with ESRD are hypertensive. Despite many available druge to decrease the blood pressure, many of these patients remain hypertensive. The major cause Of Hypertension is the increase in the blood volume which is due to the water and salt retention in the body. It doesn't seem that the uptake of body's fluid for a short period can have a remarkable effect on the pressure.

  Methods: This descriptive analytic study was done on 80 hemodialysis patients who were under trearment at Buali hospital. First patients were investigated according to age, sex, frequency of dialysis in the week and dialysis duration. These for each patient the blood pressure before and during dialysis and weight changes before and after dialysis with two consectiue sessions were measured. Calculated and MAP was determined. Finally data was analyzed through SPSS.

  Results: 48 out of 74 patients (64.9%) had hypertension. Blood pressure was controlled in 41 patients (55.4%). Age, sex, dialysis period and sessions of dialysis did not have a significant relationship with hypertension control. The study showed a high prevalence of high blood pressure in hemodialysis patients (64.9%). The blood pressure was not controlled in the majority of patients (44.6%).

  Conclusion: Patients' weight changes have no significant effect on hypertension. This shows that the causes for blood pressure can be due to other causes different from increase in the blood volume among hemodialysis patients and perhaps the chronic blood volume increase is not related to hypertension.

Abstract

  Background & Objective: Many risk factors are important in genesis of coronary artery disease. Their effect and prevalence are different in hypertensive patients. The aim of this study was to evaluate the predictive value of cardiac risk factors in hypertensive patients and compare those in normotensive patients.

  Methods: With descriptive cross sectional study and random sampling, 976 of 3000 patients’ units who underwent coronary angiography (CAG) during year of 2004 were studied. Demographic and angiographic data were collected. Data entry and analysis were done by SPSS 11.5 and EPI 6 statistical software.

  Results: Prevalence of patients who underwent CAG was 60.5%. Mean age of patients were 58.6 and 53.9 years and prevalence of CAD were 84 and 84.5 percent in hypertensive and non hypertensive patients respectively. Prevalence of two and three vessel disease was higher in hypertensive patients. Male gender, diabetes, hyperlipidemia and age were main predictors of CAD in hypertensive patients but in normotensive patients male gender, smoking, familial history and age were major predictors.

  Conclusion: Prevalence of HTN, RFs and CADare high in patients who underwent CAG and pattern of CAD in our country follows western country patterns. Male gender, diabetes, hyperlipidemia and age were main predictors of CAD in hypertensive patients, respectively regarding to high prevalence of cardiac risk factors and their severe effect on genesis of CAD in hypertensive patients rapid health measures are needed for control of HTN in general population.

Abstract

Background & Objectives:Stroke is the third most common cause of death after heart disease &cancer and the most common disabling neurological disorder. The incidence increases with age, and is somewhat higher in male than in female. The main risk factor of stroke is hypertension. In last years incidence of stroke has decreased due to improvement of hypertension treatment but due to occurrence of cerebrovascular accident with normal blood pressure, in many patients with stroke. This study aimed to evaluate the frequency of mortality and morbidity in stroke patients with hypertension & normal blood pressure.

Methods:This descriptive retrospective study, evaluated 8121 patients with stroke who were admitted at hospital (1997-2006). The inclusion and exclusion criteria were defined. Data was gathered through check list and analyzed by SPSS soft ware 10 edition and the results were compared with each other.

Results: The total mortality and morbidity rate of stroke was 13.59% who 19.3% of patients with high mean blood pressure and 10.2% of patients with normal mean blood pressure were expired due to stroke. The mortality based on age in two groups was similar and more frequent in 65 to 74 year olds. The rate of mortality and morbidity in patients with high mean blood pressure in male and female were similar but in normal mean blood pressure cases were more frequent in male than in female. Hemorrhagic stroke was the most common type of stroke in hypertensive patients (59.55%) and in normotensive patients, ischemic stroke was more frequent (37.7%). The duration from admission to death was similar in two groups and the death was highest within 72 hrs after admission.

Conclusion:Since mortality and morbidity according to age had no significant difference in both groups. It seems other risk factors are effective in evaluating cerebrovascular accidents (prognosis).

Abstract

  Background & Objectives: Hypertension is one of the most important and common health problems in societies. Hypertension is often asymptomatic and can be simply treated. Many of drugs are available for treatment of hypertension i ncluding d iuretics, beta blockers, blockers of calcium channel, angiotensin-converting enzyme inhibitors, angiotensin receptor antagonists, alpha blockers and arterial dilators. This research was performed to evaluate the efficacy of some of these drugs on the hypertension. This may help to choose an appropriate medication for the treatment of stage 2 hypertension.

  Methods: A total of 150 patients with hypertension who were not under antihypertensive treatment and didn’t have contraindications for using hydrochlorothiazide , enalapril and atenolol were randomly allocated into three groups. These groups received atenolol 50 mg daily, hydrochlorothiazide 50 mg daily and enalapril 5 mg twice daily, respectively . After three weeks blood pressure of patients was measured and results were analyzed using SPSS.

  Results: Atenolol reduced systolic blood pressure (26.7±6.7 mm Hg 16.1%), diastolic blood pressure (10.3±1.2 mm Hg 10.4%) and mean arterial blood pressure (16.1±6.4 mm Hg 13.1%). Enalapril reduced systolic blood pressure (30.6±8.8 mm Hg 17.4%), diastolic blood pressure (11.5±4.4 mm Hg 11.4%) and mean arterial blood pressure (17.9±7.0 mm Hg 14.2%). Hydrochlorothiazide reduced systolic blood pressure (25.1±5.8 mm Hg 14.6%), diastolic blood pressure (9.2±2.3 mm Hg 9.3%) and mean arterial blood pressure (14.5±6.4 mm Hg 11.8%).

  Conclusion: Despite recommendations the use of hydrochlorothiazidein the treatment of hypertension, seems lower effect only use of this drug groups in comparison with other groups, and maybe add this category to other drugs and combined treatment is better than monotherapy with these groups .
