Abstract

  Background and Objectives: pericardial effusion is characterized by the accumulation of excessive fluid in the pericardial space, which can lead to cardiac dysfunction or death. Most of the previous studies have been performed in developed countries and its epidemiologic aspects in developing countries were not well-recognized. The aim of this research was determination of etiology, clinical and paraclinical findings in patients with pericardial effusion.

  Methods: This was a cross-sectional, descriptive and retrospective study. From all of the patients admitted to Buali hospital between 2001 to 2003 ,49 patients that showed moderate to massive pericardial effusion in echocardiography were selected and studied in terms of history, physical examination, CXR, ECG and pericardial fluid analysis (if performed).

  Results: From 49 patients 25 (51%) were female and 24 were male. Age ranges of patients were 11 to 85 years. The age of 42% of the patients was above 60 years. The most common clinical complaints were dyspnea (44.9%) and chest pain (24.5%). ECG in most of them (53.1%) were normal sinus rhythm. Cardiac electrical axis in 83.7 % was normal, but ECG in 83.7% of cases showed low voltage QRS complex. The most common findings in CXR was cardiomegaly (85.7%) and 14.3% of patients had normal CXR. Common etiologies of pericardial effusion were chronic renal failure (16.3%), tuberculosis (8.2%), heart failure (8.2%) and malignancies (6.1%). Etiology of disease in 44.8% of patients was unknown.

  Conclusion: It seems that most of pericardial effusions occur in higher ages. It can be due to high prevalence of different diseases such as heart failure, chronic renal failure or malignancies in senile persons.

Abstract

 Background & Objective: Visceral leishmaniasis is a reticulo endothelia system and it is known as 'black fever' by Indians. According to WHO every year there are 500 thousand new cases of it in the world. The province of Ardabil, with 25-40% from all the country's leishmaniasis has a major part in the country and is considered as one of the main focuses in Iran. Due to its importance, this study was done to identify clinical signs and demographic characteristics of the patients.

 Methods: This was a descriptive cross-sectional study which was done retrospectively 110 children who were hospitalized in three different active hospitals between 1375-1382 entered in this study. Disease definition include parasite detection in bone marrow aspivation or special clinical signs (fever, spleenomegal, pansytopenia and weight loss) in addition direct agglutination test was positive. The samples were investigated in terms of demographic information. Place of living, clinical signs and the length of time between the symptoms of the disease and its diagnosis. To get to the results, simple descriptive statistics was used. 37.8% of the patients were diagnosed from the beginning of symptoms with in a month.

 Results: There were 56.4% boys and 43.6% girls of all the 110 children. DAT test was done in 97 patients which was positive in 91 cases (minimum positive titer 1/400). In 42 patients biopsy and bone marrow aspiration was done which was positive in 26 cases (lishman body detection). The age of the patients was 4 months -14 years. The most common findings were fever 97.3% and anorexia 97.1%. There was anemia in 90% thrombocytopenia in 60.8% and leukopenia in 24.8%. There was death in 6 patients (5.5%) and unresponsiveness to the treatment in 3 cases (2.7%).

 Conclusion: The results of the present study nearly corresponds to those of other studies. The main vulnerable community of visceral lieshmaniasis are mainly children. In order to decrease the mortality and morbidity its early diagnosis is essential. It is necessary that the physicians working in endemic regions be familiar enough with the disease.

Abstract

 Background & Objectives: This study aimed to compare the clinical efficacy of two-drug regimen of glucosamine and chloroquine with one-drug regimen of glucosamine in treatment of knee osteoarthritis.

  Methods: In this randomized clinical trial, 30 patients received glucosamine and chloroquine (trial group) and 25 cases underwent treatment by only glucosamine (control group) for 12 weeks. To evaluate pain and disability, WOMAC ( Western Ontario and McMaster Universities) was used.

  Results: Mean value of WOMAC index in the trial group was 3.51 at the beginning, 1.96 in week 6, and 1.84 in week 12. In the control group, this value was 3.15 at the beginning, 2.36 in week 6, and 2.35 in week 12. In week 12, mean value of WOMAC index was significantly lower in the trial group compared to the control group (P = 0.02). In females aged less than 65 years and with body mass index of equal or less than 30, a significant difference regarding mean value of WOMAC index was seen in week 12 between two groups (P < 0.05).

  Conclusion: Two-drug regimen of glucosamine and chloroquine in a 12-week period had a better clinical efficacy than one-drug regimen of glucosamine in lowering the pain and improving joint function of patients with knee osteoarthritis, especially in females with body mass index less than 30 and younger than 65 years of age.

Abstract

  Background & Objectives: Adherence to a diabetic diet is one of the most important aspects of diabetes management. Compliance with a diabetic diet has usually been described in terms of total intake. We examined indexes of the use of specific foods that would normally be either recommended or prohibited by Food Frequency Questionnaire.

  Methods: In order to study the dietary habits of the patients with type II diabetes 228 outpatients with type II diabetes referring to Ardabil Diabetic Clinic were randomly selected.

  Results : A common diabetic diet was reported by 87.8% of women (N = 115) and 77.1 % of men (N = 74). The dietary habits of the patients showed their awareness of limiting use of simple sugars. Use of foods with high simple sugars (e.g.: honey, jam, soft drink) were low among the patients. Potato was consumed by 58.8% of the female and 74.2% of the male, and of 43.8% of the subjects used mulberry with tea.

  Conclusions: Our result showed that there should be a greater focus on counseling, education and improving dietary of patients with diabetes.

Abstract

  Background & objectives: Today, the concept of q uality of services is particularly important in health care and customer satisfaction can be defined by comparing the expectations of the services with perception of provided services. The aim of this study was to evaluate the quality of services provided for outpatients in clinic of Ardebil city based on the SERVQUAL model.

  Methods: This descriptive study was conducted on 650 patients referred to outpatient clinic since July to September 201 3 using a standardized SERVQUAL questionnaire (1988 ) with confirmed reliability and validity. The paired t-test and Friedman test were used for analysis of data by SPSS software.

  Results: 56.1 % of respondents were male and 43.9 % of them were female . The mean age of patients was 33 ± 11.91 , 68.9 % of patients were in Ardabil and 27.3 % of them had bachelor's or higher. The results showed that there is a significant difference between perceptions and expectations of the patients about five dimensions of the service quality (tangibility, reliability, assurance, responsiveness, and empathy) in the studied clinic (P< 0.001). The highest mean gap and minimum gap were related to empathy and assurance, respectively.

  Conclusion: Regarding to observed differences in quality , the managers and also planners have to evaluate their performance more accurately in order to have better planning for future actions. In fact, any efforts to reduce the gap between expectation and perception of patients result in greater satisfaction, loyalty and further visits to organizations.

Abstract

Background & objectives: Human immunodeficiency virus (HIV) is a globally widespread infection that spreads mainly through sexual contact, mother to child and intravenous drug use. The disease can vary from an asymptomatic state to advanced immune deficiency and AIDS-related opportunistic infections and complications. Patients may be diagnosed at different stages of the disease. In this study, we aimed to analyze the clinical and epidemiological characteristics of patients with HIV and its consequences during the past 10 years (2005-2014) in Ardabil city.
Methods: In this descriptive and case series study, patients with HIV (including live and dead patients) referring to the Ardabil behavioral counseling center were included during 2005-2014 years. The checklist containing demographic data, modes of transmission, time of diagnosis, clinical and therapeutic problems during this period, etc. was prepared for each patient. In dead patients, the causes of death as well as the interval between diagnosis and death were investigated.
Results: In this study, 60 patients with HIV were evaluated, the mean age was 42.96 years, 76.7% of patients were male and 48.2% were intravenous drug users. In 39.7% of cases, patients have probably been infected through intravenous injection and 33.3% by sexual contact. The mean CD4 + lymphocyte cell count of patients was 252 cells per ml. The most common early symptoms of the disease were fever (63.33%), aphthous stomatitis (35%) and diarrhea (28.33%). In 7 cases (50%), tuberculosis was the most common opportunistic infection. 6 patients (10%) had coinfection with hepatitis B or C. 28 patients (46.6%) died within the time of study. The median time from diagnosis to death was 44.84±39.65 months and cardiorespiratory failure was the most common cause of death among patients.
Conclusion: This study showed that most patients were middle-aged and intravenous drug use was the most common risk factors for HIV. The most common way of transmission was intravenous injection and hepatitis C and tuberculosis were the most important comorbidities, respectively. Raising community awareness about the disease, early diagnosis and appropriate care during illness are basic measures to deal with this global problem.
