Abstract
Background & Objective: Most of the acute poisonings are mild with no complication but in some rare cases they can be life threatening. About 70% of acute poisoning happens in children. More than 90% of these toxic exposures are incidental and easily preventable. The factors leading to acute poisoning are variable based on cultural and socioeconomic differences among the population. The increasing production and distribution of new drugs and chemicals change the etiologic factors. This study was conducted to investigate common acute poisonings and their victims in Tabriz. Methods: In this retrospective study 306 cases of acute poisoning were reviewed from April 1999 to March 2002, in Tabriz Pediatric Medical Center. The factors that were considered included: age, gender, manner (incidental versus intentional), agents route of intoxication, residential area (rural versus urban), duration of admission, clinical presentation, date of first admission and mortality. Results: 306 patients (53.3% male and 46.7% female) were studied in this research. Most of the cases (31%) were 1-2 years old, and the fewest (1.3%) were under 6 months of age. About 95% of the cases were incidental, and 5% intentional suicide attempts among children above 10 years of age. 63% of the subjects were living in Tabriz, 29.5% of them were from rural areas and the rest (7.5%) were dwelling small towns. Poisons in the order of prevalence included: drugs (45.5%), hydrocarbons (26%), organo phosphates (7%), atropinic material and datura intoxication (6%), Rodenticides (3.6%), Opiates (2.9%), Carbon monoxide (2.2%) and alcohol, mushrooms, animal bites, acids & alkalines (6.8%). Average duration of hospitalization was 2 days and the total mortality rate was 3 cases. Conclusions: The most common etiologic factor was drug poisoning followed by hydrocarbons as the second most prevalent factor in this regard. It seems that mass media especially TV programs can play a key role to increase the public knowledge about this issue.

Abstract
Background & Objective: The nationwide amblyopia prevention program is performed by prevention deputy of welfare organization every year. In this analytic report practical results of screening program in 1998-2001 were presented and compared with similar findings in other countries. The results were also compared with the findings of previous similar researches in Iran. Methods : The amblyopia screening program was performed on all 3-65 year-old children in day-care centers as well as those referring to the visual acuity measuring centers. The first examination was accomplished by trainers. In the second step doubtful cases were referred to optometrist by trainers and amblyopia was diagnosed by an optometrist and confirmed by an eye specialist after excluding organic causes. Results: 608 out of 8427(7.21%) children examined by trainers in 2001 were referred to an optometrist because of suspected visual disorders. The prevalence of amblyopia in different cities of Ardabil province was 2%-15%. The figures in 2000-2001 were 10% (Rangs=1%-13%), respectively. In 1998 it was 11% (Range =5%-13%). In the second phase of amblyopia plan the prevalence of visual problems was estimated about 21%, 45%, 47%, and 74% in 1998-2001, respectively. In the third phase of the plan the amblyopia prevalence for children examined by eye-specialist in 1999-2001 were 1.6%, 1.41% and 1.25% respectively. Conclusions: In this research the prevalence of amblyopia was found to be 1.42% which has a conformity with the findings of other countries. Moreover, the higher variation in the estimation of the prevalence of visual disorders in this province requires further comparative researches.

Abstract
Background & Objectives:Understanding children’s behavior and its affecting factors is the foundation for practicing dentistry for children effectively and, at the same time instilling a positive dental attitude into them. A number of studies have been done in this regard during the last three decades. This study is another attempt to find out proper strategies in order to control and optimize the diagnostic and trearment measures as well as to investigate the effect of mothers ’ personality characteristics on children ’ s behavior. Methods: The sample consisted of 104 randomly selected children (53 boys and 51 girls) at the age of 3 to 6 together with their mothers. The children had no previous dental experiences and no mental or physical disabilities entailing special care. Children’s behavior was assessed during three different dental visits according to Frankle ’ s Behavioral Rating Scale, in which four distinctive scales, namely absolutely negative, negative, positive and absolutely positive were precisely defined and scaled. The first visit was dental examination, the second was fluoride therapy and the last one was restorative dental treatment. Meanwhile, their mothers ’ personalities were determined through an interview conducted by a psychiatrist and based on DSMIV criteria and using General Health Questionnaire (GHQ) these personality characteristics were categorized in four groups: Normal, Depressed, Anxious and Obsessional. Results:The findings revealed that 12.5% of children had no cooperation during 3 visits, most of whom were 3 to 4 years old. The difference was significant compared to 4-5 and 5-6 year old children (p<0.05). In addition, sex did not influence children ’ s behavior. This study showed that children of mothers with obsessional personality were the most cooperative ones during the 3 visits compared to mothers with other personality characteristics. This difference was statistically significant at the first visit, (p<0.05). Conclusion: Mother ’ s personality has the strongest effect on children’s behavior during the first dental visit.

Abstract
Background & Objectives: Cryptosporidium is a coccidial protozoa parasite belonging to subphylum of Apicomplexa. Although it is globally widespread and its species exist all over the world, this parasite was unknown until several years ago. Nowadays it is considered as an important infective agent in Immunocompromised hosts especially patients with AIDS. Due to high mortality rate of this infection in immunocompromised and cancerous patients and because of the popularity of Urmia as one of the animal husbandry regions in Iran and since this disease is likely to transfer from animals to human beings this study was conducted to determine the rate of contamination with this parasite. Methods: This case-control study was performed on 72 children (case) with cancer who referred to oncology clinic of Urmia Imam Hospital during 2001. Another 30 children with normal immune system were selected as controls. To determine the amount of contamination with parasite, two fecal specimens were collected from each patient. After being concentrated with Formalin–Ether, these specimens were examined using modified acid fast method. Results: Three cases ofcryptosporidiosis was seen in the patients (4.16%). Of these, there were 2 patients from the rural area and 1 from urban region. There was no significant relationship between the presence of cryptosporidiosis and living in rural or urban areas. Conclusion: Cryptosporidium was detected in three cases of the patients under study who were undertaking chemotherapy. However since the population under study was small, no statistically significant relationship was found between being immunocompromised and contamination with parasite.

Abstract
Background and Objectives: Children are one of the most vulnerable groups of society. Any qualitative and quantitative problem in offering health services to them is a main health and social problem. Cooperative health centers for the first time in Iran were established in East Azerbaijan as a proper strategy for solving health problems and limitations of expansion of government health centers. This study was conducted to evaluate their quality of services to children under one year old in comparison with governmental health centers. Methods: This descriptive and cross-sectional study was done on 7 cooperative health centers and 14 neighboring government health centers selected by convenience method. Data were collected by filling checklists obtained from observation of services offered to these children by healthcare personnel as well as studying the children's health records. Descriptive (absolute and percent frequency distribution, mean and standard deviation) and inferential (independent samples t-test) statistics were used for analyzing the data. Results: The findings showed that the quality of health care for children under one was good (85.71%) in cooperative centers and moderate (78.57%) in government health centers and their statistical difference was significant (P=0.002). In terms of services about growth monitoring of child, nutrition of infant and mothers' education, cooperative health centers had better practice compared to government health centers (P= 0.001, 0.007and 0.023respectively). Regarding the equipment both centers had good facilities (85.71%) and there was no statistically significant difference between them (P= 0.912). Conclusion: The results showed that, in most cases, the quality of health care given to children under one was good in cooperative health centers and moderate in government centers. This optimum performance of cooperative centers can encourage health care authorities to plan for the expansion of such centers to compensate for the deficiencies of health services especially in children under one year old.

Abstract
Background & Objective: Visceral leishmaniasis is a reticulo endothelia system and it is known as 'black fever' by Indians. According to WHO every year there are 500 thousand new cases of it in the world. The province of Ardabil, with 25-40% from all the country's leishmaniasis has a major part in the country and is considered as one of the main focuses in Iran. Due to its importance, this study was done to identify clinical signs and demographic characteristics of the patients. Methods: This was a descriptive cross-sectional study which was done retrospectively 110 children who were hospitalized in three different active hospitals between 1375-1382 entered in this study. Disease definition include parasite detection in bone marrow aspivation or special clinical signs (fever, spleenomegal, pansytopenia and weight loss) in addition direct agglutination test was positive. The samples were investigated in terms of demographic information. Place of living, clinical signs and the length of time between the symptoms of the disease and its diagnosis. To get to the results, simple descriptive statistics was used. 37.8% of the patients were diagnosed from the beginning of symptoms with in a month. Results: There were 56.4% boys and 43.6% girls of all the 110 children. DAT test was done in 97 patients which was positive in 91 cases (minimum positive titer 1/400). In 42 patients biopsy and bone marrow aspiration was done which was positive in 26 cases (lishman body detection). The age of the patients was 4 months -14 years. The most common findings were fever 97.3% and anorexia 97.1%. There was anemia in 90% thrombocytopenia in 60.8% and leukopenia in 24.8%. There was death in 6 patients (5.5%) and unresponsiveness to the treatment in 3 cases (2.7%). Conclusion: The results of the present study nearly corresponds to those of other studies. The main vulnerable community of visceral lieshmaniasis are mainly children. In order to decrease the mortality and morbidity its early diagnosis is essential. It is necessary that the physicians working in endemic regions be familiar enough with the disease.

Abstract
Background & Objective: Yersinia spp is from Enterobacteriacea family. The members of this group are all gram negative bacilli or cocobacili and non-spore and negative heme-oxidase which casuses a vast spectrum of diseases in humans the commonest one of which is gastroenteritis. Other diseases and clinical syndromes are septicemia, mesenteric lymphadenitis, Appendicitis, pharyngitis and rarely Reiter's syndrome. Due to frequent reports of febrile gastro-enteritis from Health centers of Ardebil, the probability of gastroenteritis resulted from Yersinia in this cold province is predictable. Thus this study was done to determine the role of Yersinia spp in the diarrhea of children under 5 in the cold seasons of the year in Ardebil. Methods: Stool samples from 490 children under 5 with diarrhea in a 7-month period were collected, and transferred in carry-Blair to the laboratory. Stool samples were cultured on Mac-Conkey agar and Salmonella-Shigella agar and Yersinia selective agar, TCBS agar and selenate agar. After adequate incubation, the growing colonies were identified according to the standard biochemical methods. Results: of 490 samples, only 405 were suitable to be cultured of which 9.38% pathogenic bacteria were isolated. The frequency distribution of the separated bacteria were as follows Y.enterocolitica, 13 species vibrio, 11 species Enteropathogenic E.coli, 6 species Salmonella, 5 species Shigella, 2 species and plesiomonas, one species. Conclusion: The findings of this study are the indicator of the role of Yersinia spp in diarrhea in the cold seasons of the year in the cold and mountainous province of Ardebil. On the other hand, the frequency of isolated vibria was due to simultaneous cholera epidemy in this area.

Abstract
Background and objectives: Common cold is the most common disease in children. Young children have an average of 6 to 7 colds each year. Some complications of common cold include: secondary bacterial infections, school absence and excessive cost for treatment. The management of the common cold consists primarily of symptomatic treatment. An effective and safe drug is needed to reduce the duration of common cold ۥ s symptoms in children and reduce subsequent complications. In some studies zinc sulfate has showed an antiviral effect and immune enhancing activity. Therefore this study designed to evaluate the efficacy of zinc sulfate in duration of cold ۥ s symptoms in children. Methods: This was a clinical trial study. The subjects were children between 1 - 7 years old with common cold that attended to pediatrics clinic of Arak Amir Kabir Hospital. The study included 112 patients randomized in two groups. One group received zinc sulfate within 10 days and other group didn’t receive this drug. Using a researcher made questionnaire duration of symptoms: rhinorhea and nasal obstruction, cough, sneezing, fever and duration of illness, in both groups were evaluated. Results: Consumption of zinc sulfate significantly reduced the mean duration of rhinorhea and nasal obstruction in children with common cold (p < 0.05). Also the mean duration of cough in zinc group was lower than the control group (p < 0.05). Compared to the group that didn’t receive zinc sulfate, the zinc group had shorter mean duration of sneezing (p < 0.05). Also the mean duration of fever in zinc group was shorter than the other group (p < 0.05). The mean duration of illness in patients who had received the zinc sulfate was significantly less than the other group (p <0.05). No side effect was observed in subjects received zinc sulfate. Conclusion: According to the results zinc sulfate can be administered with other supportive treatments of common cold, for reduction of symptoms duration and complications in children.

Abstract
Background & Objectives: Bipolar disorder in children and adolescents is a childhood critical disorder with negative course and outcome consequences. The aim of this study was determination of six-month outcome predictor factors (recurrence rate, the rate of hospitalization, severity of illness and recovery rates) in manic and mixed bipolar disorders of children and adolescents admitted in Tehran Rozbeh hospital. Methods: In this prospective Cohort study, 80 patients with bipolar disorder (10-18 year's old) admitted in Tehran Roozbeh Hospital from January 2009 to July 2010 were selected. The available sampling method was used for selection. Participants at admission, discharge, and follow-up at 3 and 6 months, were evaluated by using researcher made questionnaires, K-SADS (to confirm the diagnosis), CDI ( Children Depression Inventory) or BDI ( Beck Depression Inventory) , YMRS ( Young Mania Rating Scale) , CGI- S ( Clinical Global Impression Scale) , CGAS ( Children's Global Assessment Scale), CGI-S ( Clinical Global Impression Scale) and PAS ( Premorbid Adjustment Scale) . The data were analyzed by Pearson correlation coefficient and Multivariate regressions . Results: The results showed that disease outcome was not associated with age. Gender (male) had correlation with mania severity in 6-month follow-up. Disease duration predicted recurrence rate and severity of disease. Manic type disorder was related with mania severity, and mixed mania predicted mania severity negatively in 6-month follow-up. Therapeutic compliance was correlated with mania severity (negative correlation) and improvement rate (positive correlation). Presence of psychosis was correlated with recurrence rate positively in 6-month follow-up. Co-morbidity with ADHD ( Attention Deficient /Hyperactivity Disorder) predicted clinical global improvement (CGI-G) negatively and mania severity positively in 6-month follow-up. Pre-morbid coping showed negative relationship with mania severity and positive relation with global improvement rate in 6-month follow-up. Conclusion: Our results showed that gender of patients, duration and manic type of disease, presence of psychosis at admission have a direct relation with inappropriate outcome of manic and mixed bipolar disorders of children and adolescents. These findings emphasize necessity of special curing during treatment.

Abstract
Background & Objectives: Epidemiological and experimental evidences suggest that oxidative stress is elevated in children with Down syndrome (DS). Some studies show that consumption of diets with a high content of fruits and vegetables results in a significant reduction in markers of oxidative stress. We investigated the frequency of fruits and vegetables intake in children with DS and the influence of variations in the level of this consumption on biomarkers of oxidative stress . Methods: Frequency of consumption of fruit and vegetables by children with DS were recorded by interview with the children’s parents. Serum malondialdehyde (MDA) and urinary 8-hydroxy-2'-deoxyguanosine (8-OHdG) were also measured as biomarkers of oxidative stress. Data analysed using ANOVA and Student’s t-tests . Results : Mean consumption of fruits and vegetables was 4.6 and 0.74 servings per week, respectively. No significant relationships were found between consumption of fruits and vegetables with markers of oxidative stress. Based on our findings, the 65.9 % of the children did not eat any vegetables and the rest of the children consumed less than one serving per day. There was a significant correlation between the frequency of fruits and vegetable consumption (r=0.212 p <0.04). Conclusion : There wasn’t any significant relationship between fruits and vegetables consumption and indicators of oxidative stress in children with DS. This lack of correlation could be due to low consumption of fruits and vegetables in children with DS.

Abstract
Background & objectives: Long QT syndrome (LQTS), congenital or acquired disorder, is characterized by a prolonged QT interval associated with syncope attacks and sudden death. Jervell and Lange-Nielsen first described LQTS in conjunction with sensory-neural congenital deafness. In long QT syndrome, the modified QT interval is greater than 440 ms and 460 ms in men and in women respectively. Acquired forms of prolonged QT are the consequence of metabolic disorders and various medications.
Since the number of people with congenital deafness in the society is not low and studies in Iran about this syndrome, especially in children are limited, therefore identification of these patients by performing an ECG and simple measures, may be useful in reducing sudden death among these patients. The aim of this study was to determine the prevalence of prolonged QT syndrome in children with congenital sensory-neural deafness which undergoing cochlear implant surgery.
Methods: In this study, a single-center cross-sectional observational study, all children with congenital deafness which undergoing cochlear plantation at northwest of Iran were enrolled. Patients' heart rate and QT interval were calculated using Bazett's formula. The association of long QTc with syncope and sudden death, evaluated by using Schwartz's criteria in children with long QT syndrome.
Results: Of the 357 studied patients, 204 (57.1%) were male and 153 (42.9%) were female. The mean±SD of the current age of the patients was 7.15±2.93 years with a mean of 7 years. The mean±SD of QT interval in the studied patients was 291.01±26.89 ms with a mean of 280 ms. the mean±SD of the QTc interval in the studied patients was 400.52±25.74 ms with a median of 404 ms. Frequency of Long QTc in the studied patients was 17 cases (4.8%) and long QT syndrome was 5 cases (1.4 %).
Conclusion: The prevalence of prolonged QT syndrome in patients with congenital deafness in northwest of Iran in 2019 was 4.8%, and 2 cases had syncope and 3 cases had sudden death.

Background & objectives: Covid-19 disease with widespread prevalence in 2019 worldwide has been identified as the biggest health threat, according to the definition of the World Health Organization (WHO), especially for children. In addition, sleep disorders are one of the consequences of this emerging disease; the aim of this study was to investigate the nutrition and sleep disorders of active and inactive children during the corona pandemic.
Methods: Random multistage cluster sampling was used in four geographical regions of Iran (north, south, west and east) in this research. This study was conducted in 2021 which the questionnaires were gathered from the cities and the sample size (194 people) was determined based on G-Power 3.1 software with 95% confidence and 80% statistical power (evaluation through virtual networks and web questionnaire). The Eating Attitudes Test (EAT-26) Questionnaire, the Children's Sleep Habits Questionnaire (CSHQ), and the International Physical Activity Questionnaire (IPAQ) were used. Mann-Whitney U and T-Student tests were used to compare the two groups of active and inactive children. The significance level was considered at p≤0.05.
Results: The results showed that in the Covid-19 pandemic, the group of active children had a better level of physical activity (energy consumption) and status compared to the inactive children (p≤0.001). Also, it was shown that girls had more nutrition disorders than boys (p≤0.05), but there was no significant difference between sleep disorders between boys and girls, and there was no significant difference between nutrition disorders in active and inactive children.
Conclusion: In summary, during the Covid-19 pandemic, there was no difference in eating and sleeping disorders in active and inactive children, while gender is associated with nutrition disorders in children during Corona disease and was higher in girls than boys. It is suggested that parents pay more attention to physical activity and nutrition disorders in girls during home quarantine

Abstract
Background & objectives: The purpose of this study was to model the causal behaviors related to bullying based on the experience of child abuse mediating the role of emotional security.
Methods: Method of this research was correlational. The statistical population of this study included male high school students in Ardabil city in the academic year of 97-98 in which 385 people were selected as a statistical sample by cluster random sampling. Bernstein's Child abuse (CTQ) and Emotional Security Questionnaires were used to collect data. Data were analyzed using structural equation modeling using AMOS22 and SPSS22 software.
Results: The results of the study were in line with the hypotheses that showed child abuse (p<0.05) had a positive effect and emotional security (p<0.05) had a negative effect on bullying-related behaviors. Also, the fit of the proposed model has a good fit and the variable power of emotional security in mediating the impact of child abuse on bullying-related behaviors was confirmed.
Conclusion: According to the results, parents should be aware of the devastating consequences of child abuse on their children's mental and physical health and the importance of emotional security and safety in their children's healthy development, to prevent and reduce bullying-related behaviors. Teachers and school officials should also be aware of bullying and related behaviors to reduce and prevent harmful effects on students.
