Abstract

  Background & objectives: The hand is one of the sensivitive organs of the body. The hand injuries usually occur in workers and are one of the most common causes of disabilies. The aim of this study was to investigate the causes of the hand injuries leading to defect among industrial workers and farmers’ reffered to emergency department of Fatemi hospital in Ardabil.

  Methods: In a cross-sectional study, from January 2009 to Agust 2010, 100 patients with hand injuries were included. The data were collected by the chek list including: category of severity, time and type of incident, and location and cause of the injuri and then were analyzed statistically by Chi-square test.

  Results : Most of the industrial workers (58%) ranging as 20–29 years old and had 38 years as an average. Sixty two percent of them were working more than 8 hours per day and 64.7% experienced severe injuries in their hands. Amputation showed to be the most common injury in this group. Most of the farmers (30%) were in age group 20-29 years and from them 70% were working less than 8 hours per day. In this group, 54% of injuries occurred in left hand and the most common type of injuries was amputation (54%).

  Conclusion: The highest incidence of hand injuries has been shown to occur among industrial workers with 28 years as mean and working for more than 8 hours per day without any formal training before starting to work. So, it seems to be need we use the expert workers with the high level background and don’t use adolescents under 15 years.

Abstract

  Background & objectives: The most common causes of mortality in patients with head trauma are epidural and subural hematomas. In the last three decades because of improvements in saving and monitoring patients in intensive care unite (ICU), the therapeutic outcomes have been improved dramatically. The aim of this study is to clarify the factors affecting treatment outcomes of patients with epidural and subdural hematomas in Fatemi hospital in Ardabil.

  Methods: The subjects of the study were selected from 109 recorded files in Ardabil Fatemi hospital in years 2001-2006. Data were analyzed by descriptive statistical methods, chi-square and Student's t-test using SPSS software.

  Results: Sixty nine (63%) and 40 (37%) of patients had epidural and subdural hematomas, respectively. The mean age of patients was 33. GCS at the time of admission was 10 and mean hospitalization days in neurosurgery section and ICU were 6 and 8 days respectively. Ninty two (84%) of patients were men and 17 (16%) were female. According to Glosco system, treatment autcome of 76 (70%) patients was desired and that in 33(30%) patients was poor. Nine (13%) of patients with epidorel hematomas and 23 (53%) patients with subdural hematomas were died.

  Conclusion : This study showed thattype of hematoma (epidural and subdural), age, length of ICU stay and GCS at the time of admission affect the treatment outcomes.

Abstract

  Background & Objectives: Consumption of ω3 fatty acids supplementation inhibits oxidative stress injury, increases activity of antioxidant enzymes and decreases lipid peroxidation in gastric cancer patients. In this study, we examined effects of ω-3 fatty acid intakes on oxidative stress in gastric cancer patients undergoing chemotherapy.

  Methods: This double blind clinical trial study was conducted on 30 adult patients (15 cases and 15 controls) with gastric cancer during chemotherapy in Ardabil city in 2010. Case and control groups were selected by randomized allocation. Three grams ω -3 fatty acid supplementation (1.8 g EPA & 1.2 g DHA in 10 g fish oil) and placebo were given case and control groups for 45 days, respectively. Anthropometric indices (weight, height & BMI) were measured. Blood samples were taken and then biochemical factors including triglycerides, total cholesterol, HDL and LDL cholesterol, MDA, and total antioxidants were evaluated at the beginning, middle and end of the study . The data were analyzed by using Paired sample t-test, Independent sample t-test and repeated measures test.

  Results: MDA, Weight and BMI of omega group after intervention were significantly more than control group at the end of the study (p<0.05). Weight and BMI were decreased but serum MDA was significantly increased in control group during the study (p<0.05). Weight, BMI , and total antioxidants were significantly increased in omega group during day 30-45, (p<0.05). There were no significant differences in other biochemical factors at the end of study.

  Conclusion: The present investigation shows administration of ω3 fatty acid supplements to gastric cancer patients during chemotherapy increases the total antioxidants capacity and prevents the enhancement of oxidative stress.

Abstract

  Background & Objectives: Using of drugs arbitrarily leads to failure of treatment and causes side effects in prolonged time. The aim of this study was to survey prevalence of arbitrary drug use and the factors affecting it.

  Methods: Three hundred students participated in this cross-sectional and descriptive study in Ardabil (Islamic Azad, Mohhaghegh Ardabili, Payame-Noor and Medical Sciences Universities) during 2010. The samples were selected randomly from four universities and data were collected by questionnaire and then, analyzed by descriptive and analytical statistics such as chi-square test.

  Results: The average of students’ age was 20.64±2 years. 23.3% of them were married from whom 92.2% were non-medical students and 7.1% were medical students. 76.7% of all students were single. 83% of all students were using drugs arbitrarily. The highest mean score for student attitude was reflected to safety of used drugs. Lack of time to visit a physician, believing the safety of many drugs and selling of drugs without prescription were the most common causes in arbitrary drug use among students.

  Conclusion: Regarding to the high arbitrary drug use among students, it is necessary to make a new policy for education and correct informing of students in order to reduce self-treatment.

Abstract

  Background & Objectives: Several evidences show that oxidative stress is high among patients with hemodialysis . This study was aimed to investigate the oxidative stress and serum level of zinc and copper in patients with non-diabetic hemodialysis .

  Methods: In a case-control study, 60 patients with non-diabetic hemdialysis referred to hemodialysis section, Ardabil Bou-Ali hospital, and 60 healthy individuals, matched by age and sex, were selected. In each group, lipid profile, blood glucose, creatinine, urea, total protein, albumin, zinc, copper, malondialdehyde, total antioxidant capacity, activity of glutathione peroxidase and superoxide dismutase were measured. The results were analyzed statistically by using Pearson correlation coefficient and Student-t test.

  Results: The amount of zinc, blood sugar, cholesterol, triglyceride, total protein, albumin, high density lipoprotein-C and low density lipoprotein-C were significantly lower in patients with hemodialysis in comparison with those in normal individuals. T he activity of superoxide dismutase, glutathione peroxidase in red blood cells and serum total antioxidant capacity were significantly high in patients in comparison with those in control group. However, serum concentrations of copper and malondialdehyde in two groups were not statistically different.

  Conclusion : The results of this study show that increase of antioxidant enzymes activity reduces the oxidative stress status in patients with non-diabetic hemodialysis . Zinc deficiency was also observed in these patients.

Abstract

  Background & Objectives: Coronary artery disease is the main cause of mortality in developing and industrial countries. Recently the involvement of infectious agents as a risk factor for Acute Coronary syndrome is drafted. So this study was designed to investigate the probable association between Acute Coronary syndrome and Helicobacter pylori infection.

  Methods: This case-control study was carried out on 300 hospitalized patients with the diagnosis of Acute Coronary syndrome (UA and MI) and 300 hospitalized patients without the history of coronary heart disease. Anti Helicobacter pylori Antibody level was determined by as an indicator of infection history. Using chi-square and t- test the results were analyzed in SPSS software.

  Results: Results showed that 79 patients (26.3%) in control group and 122 patients (40.6%) in case group were seropositive and the difference was significant. Relationship between cronory diseases risk factors and levels of IgG was not significant. Also the results showed that the rate of hypertension in seropositive patients in case group was significantly upper than control group.

  Conclusion: Regarding the findings of this study we can conclude that Helicobacter pylori infection probably is a risk factor for Acute Coronary Syndrome. Thus, further studies are needed to elucidate the association between Helicobacter pylori infection and Acute Coronary Syndrome.

Abstract

  Background & Objectives: Urinary stone disease is one of the common urinary tract complications that can lead to renal failure. It occurs usually in middle-aged and older men The aim of this study was to determine the association between types of urinary stones and risk factors for urinary stone formation in patients referred to Ardabil therapeutic centers.

  Methods : In this descriptive cross-sectional study, 150 patients with urinary stone were followed from March 2008 to July 2009 in therapeutic centers of Ardabil.

  Data were collected by a questionnaire include demographic factors such as age, gender, body mass index (BMI), Job and Habitation place. Data were analyzed by chi-square test and ANOVA in SPSS 16 software. The level of significance was p<0.05.

  Results: 116 of patients (77.3%) had calcium oxalate stone, 15 of patients (10%) had phosphate calcium stone, 10 of patients (6.7%) had acid uric and 9 of patients (6%) had phosphate ammonium stone. The mean age of patients was 41.90 + 14.41 years (48% of patients were 30-50 years old) and the mean body mass index (BMI) was 25.81+3.66. 91 of patients (60.66%) were male and 59 (39.4%) of them were female. There was no significant difference between types of urinary stones and factors including BMI the job of patient, (p=0.252), habitation place (p=0.173), history of previous illness (p=0.207) and history of drug use (p=0.247).

  Conclusion: The most common type of urinary stones was calcium oxalate (77.3%) in both men and women however ammonium phosphate (6%) is much less common.

Abstract

  Background & Objectives: Insomnia is one of the most common sleep disorders in the world. It causes disruption in daily activities and increases the risk of major depression. Hence, clinically the appropriate and persistent treatment of insomnia is very important. Using of hypnotic drugs such as benzodiazepines is the common treatment for insomnia but they show several side effects and it seems that new medications should be used for treatment of sleep disorders. The aim of this study was comparison between the effects of electromagnetic therapy and conventional drug usage in the treatment of insomnia.

  Methods: In a blind randomized clinical trial study, 60 people referred to the private office of the psychiatrist and experienced more than 3 months extended primary insomnia were selected. They were diagnosed by DSM-IV criteria and had no other underlying problems. The subjects were divided in two groups: 30 people in each and treated electromagnetically or with Alprazolam for 3 weeks. Before treatment, immediately and one month after treatment, quality of sleep and severity of the insomnia were evaluated by using the standard questionnaires and finally, the results were analyzed statistically.

  Results : In this study, 60 individuals participated from whom 28 were male (46.7%) and 32 patients were female (53.3%).The mean age was 37.3 years old in a range of 17- 65. The mean point of each questionnaire, before and immediately after treatment significantly didn't show any difference but one month after treatment, there was a significant difference in both groups.

  Conclusion : To treat insomnia, electromagnetic therapy appears to be used as a replacement for sedative medicines. It also has more stability in comparison with other sedative medicines and no side effects have been reported yet.

Abstract

 Background & Objectives: Matrix Metalloproteinases (MMPs), a member of photolytic enzyme family, degrade the extra cellular matrix. MMPs have very important roles in physiological and pathological processes. It has been reported that MMPs concentration increase in malignancies such as stomach, breast, colon, lung, head and neck cancers . Infectionwith Helicobacter pylori is risk factor for gastric cancer and may increase the serum level of MMP-9. The aim of this study was to measure the concentration of MMP-9 in sera of patients infected with H. pylori.

 Methods: In a descriptive-experimental study, apparent healthy individuals who were refereed for stool and blood tests were randomly selected and their stools and sera samples were collected. A questionnaire containing age, sex, smoking and special diseases in family and type of their diseases was filled for every volunteer. The sera collected immediately after blood sampling and stored in -70oC until used. The concentration of MMP-9 was assessed using ELISA. Stool samples were used for detection of H. pylori antigen.

 Results: H. pylori positive, negative and equivocal volunteers were 52.38%, 29.76% and 17.85%, respectively. Differences between MMP-9 concentration and H. pylori negative and positive groups were not significant (p=0.25). A significant increase in MMP-9 concentration was found in sera from drivers in compare with housewives.

 Conclusion: Although there was an increase in serum concentration of MMP-9 in H. pylori infected person, differences between H. pylori negative and positive groups were not significant. The raised concentration of MMP-9 in drivers may arise from difference in their health condition in compare with the housewives. This study suggests that MMP-9 level in serum may increase before initiation of gastric cancer in H. pylori infected individuals.

Abstract

  Background & Objectives : Intense training increases the production of free radicals and causes inflammatory response in athletes. Strengthen and improving athlete’s immune system may reduce the harmful effects of intense physical activity. The aim of this study was to investigate supplementary consumption of coenzyme Q10 on serum concentration of tumor necrosis factor-alpha (TNF-α) during the maximal activity.

  Methods : Twelve healthy active males (age 21.75 ± 0.64 yr, BMI 23.7±0.94 kg/m2) performed 30-min exercise at 80% to 85% HRmax. Subjects 120 minutes pre-exercise received either of the following regimens: Coenzyme Q10 (2 mg per kg body weight) or placebo (food color). Blood samples were obtained prior to supplement consumption and immediately after exercise, then groups were reversed after 4 days. The data were analyzed using paired and independent t-test. The statistical significance was set at p<0.05.

  Results: TNF-α serum level increased in both supplementation and placebo group (4.2% and 5.12% respectively) and the difference between two groups was insignificant.

  Conclusion : The results of this study indicated that after maximal activity increasing of TNF-α serum level was slower in the supplement group in comparison with placebo group but Q10 consumption did not caused a significant difference between two group (p=0.8).

Abstract

  Background: Multiple sclerosis (MS) is an inflammatory disease of the central nervous system with multifocal areas of demyelination. Genetic and environmental factors could be involved in etiology of disease. The aim of this study was to evaluate important risk factors for MS progression in patients admitted in Alavi hospital, Ardabil.

  Methods : The present research is an analytical, case-control study in groups with or without MS with same age and sex (80 subjects in each group). A self-made questionary including demographic characteristics and risk factors used as a data compilation instrument which validity reliability were confirmed by content and α-Coronbach tests respectively. Data were analyzed by SPSS software using descriptive and inferential statistics.

  Results: Factors such bachelorship (p=0.037), accidental head and back traumas (p=0.003), smoking (p=0.035), smoke exposures (p= 0.0001), meat regimen (p=0.0001), contact with birds (p=0.032), inadequate sunlight exposure (p=0.034), occupational pollution (p=0.002), menarche age (p=0.016), family history (p=0.029), being third born in family (p=0.034), were found as significant risk factors for multiple sclerosis.

  Conclusion : The present study demonstrates that factors such as smoke, trauma, environmental pollution, touching birds and family history of MS are important risk factors for onset of multiple sclerosis.

Abstract

  Background & Objectives: Endothelium nitric oxide synthase (eNOS) is a type of enzyme which produces an endogenous factor called nitric oxide (NO). NO plays important role in progress of euplastic diseases. In chronic gastritis, the increased level of NO causes damages to DNA. The aim of present study is to evaluate eNOS concentration in sera of healthy people and those infected by Helicobacter pylori .

  Methods: The sera and stool specimens from 84 voluntaries (Female: 58.3%, Males 41.6%) were collected. Helicobacter pylori antigen in the stool specimens and eNOS levels in sera were determined using ELISA . Obtained data were analyzed using Excell software.

  Results : The age range was from 1 to 78 years old (Mean: 30 years old). In terms of special diseases, 70.2% did not have any special diseases, but 29.76% showed at least one special disease, mainly thyroid disease and hypertension. The results for H. pylori stool antigen detection showed that 16.6%, 29.76% and 53.57% of collected specimens were equivocal, Helicobacter pylori negative and positive respectively. Comparison of sera concentrations of eNOS showed that there is no significant change among these three groups.

  Conclusion : As mentioned in results, the eNOS sera concentrations showed no significant change in Helicobacter pylori positive and negative groups. Albeit the other studies showed the significant increase in serum concentration of Helicobacter pylori positive patient, this controversy may arise from race and variations in Helicobacter pylori pathogenic islands such as those containing VacA and CagA. We propose to conduct a similar study in Ardabil to focus on the pathogenic islands of H. pylori strains in this province.

Abstract

  Background & Objective: Currently, migration of rural population to cities and developing slums around them are main problems in population settlement that lead to imbalance in health standards such as difference in age groups death. The aim of this study was to examine factors associated with infant mortality in slums of the Ardabil city.

  Methods: This was a case- control study. All infants’ deaths from 2008 to 2010, which occurred in slums as a case, were included. For each case, tow controls, were chosen. Controls the ones who survived and were living in non marginalized area and years of birth and sex were matched in two groups. Information collected from health centers through questionnaires, interviews with parents and child death files.

  Results: The majority of mortalities (74.6%) were in neonatal period and 25.4% was in post- neonatal period. The most important causes of deaths in neonatal were prematurity (57%), and in post neonatal period were congenital abnormality (35.4%). Analysis of effective factors showed that birth weight, parents age and education, family income and occupation and smoking of father were highly associated with mortalities.

  Conclusion: Infant mortality of the family that their father is a smoker and they have low income, and low parental education level is significantly higher and should be taken into serious consideration to reduce the mortality.

Abstract

  Background & Objective s: Poisoning is one of the most common causes that referred to the Emergence and one of the most important Medicine immediacy. Correct and immediate diagnosis and accommodative treatment can be life saving. Knowing the overall pattern of poisoning in any geographical area would help to better manage and treat the victims.

  Methods : In a cross sectional descriptive study, the required information was collected from the records of patients showing drugs and chemical poisoning who referred to Imam Khomeini hospital Collected data was statistically analyzed using SPSS software.

  Results : Out of 2852 case of poisoning, 106 people passed away due to severe complication. Of these 56.8% were men and 43.2% were women. The majority of cases (76.8%) were from urban areas. Suicide was the main cause of poisoning (66.53%). Accidental poisoning was recorded for 5.01% of cases. In 28.45% of cases, the cause of poisoning was not identified. The age group 21-30 years made the highest number of cases (52.3%). Tramadol, Benzodiazepines and Aceteminophen were the most poisons used respectively. Benzodiazeine was the most taken drug is in suicide (19.97%).

  Conclusion : Results showed that poisoning with Tramadol and Benzodiazepines is high in Ardabil Province. Opioids, Aluminum Phosphide and Organophosphores are the main causative of death in poisoned victims in the province of Ardabil.

Abstract

  Background & objectives: More than 95% of bile ducts diseases are pertain to either gallbladder stone or cholecystitis. Normally, there is not amylase in gallbladder and its presence indicates reflux from the pancreas. The aim of this study was to investigate the association between presence of gallstones and amylase level in gallbladder.

  Methods: In this cross-sectional study 100 patients with cholecystitis were selected and after removal of the gallbladder under sterilized conditions, about 1-5 ml of fluid has been aspirated from bladder. The amylase level of samples was measured in the laboratory and collected data analyzed using statistical tests such as t-test and ANOVA by SPSS 18 software.

  Results : A total of 100 patients with cholecystitis, 42% had measurable amylase in gallbladder (25-65 IU/L). The patients were in the range of 22-92 years old and 47% of them was in the age group of 40-60 years old. The mean age of patients was 48.2 years old (SD=12). There was a significant linear correlation between the gallbladder amylase level and the age of patients (r=0.76, p=0.001).

  Conclusion: Results of our study showed a significant relationship between the formation of gallestones and pancreatic amylase reflux to gallbladder. So, in patients with gallestones, the possible existence of pancreatitis and specially the probability of the pancreas and bile duct obstruction or narrowing should be checked .

Abstract

  Background & Objectives: It is estimated that one third of the world’s population is infected by M. tuberculosis. Because of differences in immune system activity against the invasive microorganisms, the disease is developed only among 10% of them. Vitamin D metabolism and its receptor activity are important factors in human native immune system against tuberculosis. In the present study we investigated ApaI polymorphism of vitamin D receptor (VDR) gene and association with susceptibility to tuberculosis.

  Method: This study was performed on 84 cases with tuberclosis (male =50, female =34) and 90 controls (male =49, female = 41). DNA was extracted from cases and controls leucocytes and elected sequences amplified in polymerase chain reaction (PCR) procedure by using specific primers. ApaI polymorphism of VDR gene evaluated by RFLP technique on PCR products. Finally statistical analysis performed using Chi- square to compare genotype frequencies between cases and controls.

  Results: In case and control groups, AA genotype frequency were 34.5% and 33.3% respectively (OR=0.905, 95% CI 0.469-1.747, p = 0.766) and a genotype frequency in patients and control group were 15.47% and 13.3% respectively (OR=0.808, 95% CI 0.333 –1.961, p=0.637).

  Conclusion: In the present study we could not find any significant relationship between genotype frequency of ApaI (A/a) polymorphism in VDR gene and susceptibility to tuberculosis.

Abstract

  Background & Objectives : Poisoning is a common cause of hospital emergency visits in many countries. Incidence of poisoning is different based on cultural and economic characteristics. The aim of this study was to investigate the epidemiology of poisoning in pediatrics.

  Methods: This cross sectional study was performed on 336 patients under 13 years of age referring to Bu-Ali hospital during five years (2007-2011). Variables such as age, sex, residency place, hospitalized duration, cause of the poisoning, clinical signs, treatment and outcome were obtained by a checklist. Collected data analyzed by statistical methods in SPSS v16.

  Results : Among the subjects, 197 patients (58.6%) were male. The average age of the children was 4.1 years and the most common age range was 1 to 4 years with (210 patients, 62.5%). The mean duration of hospitalization of children was 1.7 day (SD= 0.2), 77.7 % of the children were living in the city and the summer was accounted as a common season in which the patients referred to the hospital (32.7 %). The most used materials were drugs with 60.71% and in 86.6% of patients the cause of poisoning was accidental.

  Conclusion : Results showed that the most common cause of poisoning among children are drugs and methadone was the most commonly abused substance. It requires that parents should be trained about keeping their children away from drugs somewhere.

Abstract

  Background & Objectives: Delivery is one of the most important events in a woman's life, and pain is an inevitable part of childbirth. The non-controlled pain leads to prolonged labor and multiple injuries to mother and fetus. This research was done to determine the effect of acupressure at the SP6 and LI4 points on the duration and pain of the labor in nulliparous women.

  Methods: This randomized controlled clinical trial was conducted on 84 nulliparous women who were attended hospital in Ardabil. The participants were divided into two intervention and control groups using randomized blocking. Pressure has been done on SP6- LI4 points in intervention group. Pain intensity was measured before and after the intervention using visual analog scales and length of active phase and second stage were recorded through vaginal exam in the checklist. Determination of the effect of pressure on the intensity of labor pain and delivery length performed using paired t-test, Student’s t-test and chi-square by SPSS v.16.

  Results: After the intervention the mean level of pain intensity in the intervention group was decreased. Also, there was a significant decrease in mean intensity of pain after each intervention in the intervention group with different dilations (4, 6, 8, and 10 cm) (p<0.001).The mean duration of active phase and second stage in intervention group was significantly less than the control group . (p<0.001, p=0.008).

  Conclusion: Since acupressure at the SP6- LI4 point in different dilatations leads to the reduction in labor pain intensity and delivery duration, therefore, this method can be used to reduce the pain and duration of labor.

Abstract

  Background & objectives: Heart failure is considered as a major cause of hospitalization. Many studies have shown association between sleep-related breathing disorders and heart failure. It has been shown that the relationship between nocturnal hypoxia and left ventricular dysfunction can cause significant morbidity and mortality in patients with congestive heart failure (CHF ). Accordingly, treatment of sleep related breathing disorders (SRBD) can give rise to improvement in CHF treatment too. This study surveys the prevalence of sleep disorder in stable heart failure patients regardless of ejection fraction.

  Methods : This study was a descriptive-analytical study. One hundred and eight patients with heart failure disease were studied. A questionnaire consisting of two parts (part I consistent of demographic information and part II consistent of sleep disorders) and clinical examination (pulse oximetery and echocardiography) were used for collection of data. The data were analyzed with SPSS statistical software using descriptive and analytical tests including the chi-square, Pearson correlation and ANOVA.

  Results: Fifty six persons (51.9%) of patients were female and 52 persons (48.1%) were male with mean age of 65.42 ± 11 years. In total sleep duration, 95 patients (88%) had nocturnal hypoxemia . There was correlation between arterial oxygen desaturation at night and ejection fraction .

  Conclusion : This study confirmed strong associations between nocturnal hypoxia and left ventricular dysfunction and SRBD should be considered in clinical treatment of systolic heart failure.

Abstract

  Background & objectives: Procalcitonin (PCT) is a precursor of calcitonin hormone, comprising 116 amino acids, which is produced in thyroid neuroendocrine cells. However, in conditions such as septic shock, metastatic cancers, bacterial and fungal infections PCT is secreted by lung neuroendocrine cells, liver cells, and leukocytes. PCT is involved in acute phase of inflammatory process as a reactant protein. Hence, in this study serum and pleural fluid PCT was evaluated in common types of pleural effusion.

  Methods: This is a cross-sectional case-series study carried out on 60 patients with pleural effusion. In this study, effusion cases due to tuberculosis, malignancy, effusion followed by pneumonia, and also transudate were investigated. Other cases of pleural effusion were excluded from this study. After collecting the samples, PCT levels in pleural fluid and serum of patients were measured and compared statistically with respect to the primary etiology of effusion.

  Results: Sixty patients (10 patients with TB, 10 patients with malignancy, 10 patients with para-pneumonic, and 30 patients with transudative pleural effusion) participated in this study in which 61.7% were men and 38.3% women (p=0.204). The mean age of participants was 62.68 ±19.38 years. Results showed that mean of PCT in pleural fluid of patients with exudative effusion was 0.74 ng/ml (0.63 ng/ml in malignancy, 0.42 ng/ml in TB and 1.51 ng/ml in para-pneumonic) and in patients with transudative was 0.42 ng/ml. In addition, mean of PCT in the serum of patients with exudative pleural effusion was 0.74 ng/ml (0.65 ng/ml in malignancy, 0.40 ng/ml in TB and 1.20 ng/ml in para-pneumonic) and in patients with transudative was 0.43 ng/ml. The analyzed data showed that there was a significant difference in the pleural fluid (p=0.016) and serum PCT (p=0.009) of patients with pleural effusion transudate and exudates. After excluding patients with transudative, a significant difference was also observed among patients with exudative in such a way that in the para-pneumonic group the amount of PCT level in the serum and pleural fluid was more than tuberculosis and malignancy groups.

  Conclusion: Serum and pleural fluid PCT is increased as a marker in response to inflammation. Although its increase in patients with bacterial and pneumonia infection is significant, it can be increased in patients with secondary inflammation of tuberculosis or malignancy and consequently can be used as an inflammatory indicator in the evaluation of patients with pleural effusion.
