Abstract

  Background & objectives : Different types of practises are known for improving motor skills in intellectually disabled boys. The purpose of this study was to compar e the impact of spark motor program and basketball on improving of gross motor skills in this people.

  Methods: In this semi-experimental study , from 98 educable intellectually disabled students who studied in special school in Urmia, 30 children ( age range of 9 to 13 years and IQ mean 64.4 ) were selected objectively and divided in three groups (2 experimental and 1 control) based on pre - test. BOTMP was used as a measurement of motor ability. Selected motor program (Spark motor program) including strengthening training, games, sports and basketball techniques was performed for 24 sessions. T-tests (dependent) and co-variance were used to comparison of results.

  Results: In Spark group after 24 sessions, there were significant effects on balance (p= 0.000), bilateral coordination (p=0.000) and strength (p=0.001). There was no significant effect in agility and speed (p= 0.343) in basketball techniques group after 24 sessions, there were significant effects in agility and speed (p= 0.001), balance (p= 0.000), bilateral coordination (p= 0.013) and strength (p= 0.007).

  Conclusion: Based on the results of this study, it can be claimed that the Spark program and basketball techniques improve gross motor skills in educable intellectually disabled students. We also found a significant difference between the Spark program and basketball techniques efficacy on the improved skills. Furthermore, the efficacy of Spark program was significantly higher than basketball techniques (p<0.05).

Abstract

Background & Objectives: Obesity is associated with metabolic abnormalities such as insulin resistance. Weight loss is always recommended for meaningful reduction in these abnormalities. Therefore, we examined the effects of balanced low calorie diet on weight loss, fasting insulin and insulin resistance in obese women attending the nutrition clinic.

Methods: In this clinical trial, forty five volunteer apparently healthy obese women (body mass index: 33.28 ± 2.90 kg/m2 and age: 28.40 ± 7.98 years) were recruited from the nutrition clinic in their first visit. The participants were received a balanced low calorie diet over 6 months after anthropometry measurement and blood sampling for determination of fasting insulin and insulin resistance level. Balanced low calorie diet was an individualized diet with an energy deficit of 500 calories of daily energy intake that was calculated from 3-day food records for every person. At 6 months after intervention, continuous and dichotomous variables were assessed using paired t-test and McNemar test, respectively.

Results: The mean recommended energy intake was 2241.78 ± 219.02(kcal). After 6 months, there were significant reductions in body weight (p=0.003), body mass index (p=0.005), waist and hip circumferences (p<0.001 and p<0.001, respectively). There was also significant improvement in fasting insulin level (13.47 ± 6.55 vs. 11.95 ± 4.17 µU/ml, p=0.01). Whereas, insulin resistance had not significant reduction (p=0.1).

Conclusion: In obese women attending the nutrition clinic, the balanced low calorie diet resulted in weight loss and improvement in fasting insulin. These positive changes can help to decrease the risk factor profile in obese individuals.

Abstract

Background & objectives: Epithelial ovarian carcinoma seems to be one of the most lethal cancer types among all gynecological malignancies. The conventional course of therapy includes chemotherapy. Actually most cancers respond to chemotherapy but in the long run drug resistance and side effects cause treatment failure. In addition, milk thistle (silibinin), a plant that has been used from ancient time because of its good effects on different organs, determined to have powerful antioxidant activity.  The aim of this study was to examine the effect of silibinin on SKOV-3 cancer cell line after 48 hours of treatment and P21 gene expression which involves in cell cycle progression.

Methods: The human epithelial ovarian cancer cell line SKOV-3 was cultured as monolayer in 25 cm2 flask in RPMI-1640 medium supplemented with 10% fetal bovine serum (FBS). Then the numbers of live cells were calculated using hemocytometer method and the cells were seeded in 96-well flat-bottomed culture plates and treated with different concentration of Silibinin. MTT assay was carried out to determine cell viability. To study P21 gene expression, RNA extraction and cDNA synthesis were carried out and real-time PCR was done.

Results: Cell growth was inhibited considerably by Silibinin treated groups compared with control after 48 hours. P21 gene expression was increased as well.

Conclusions: According to the results, Silibinin can be used as an effective drug in cancer treatment. More studies on animal models are also suggested.

Abstract

Background & objectives: Enterococcus spp. are predominant in the faecal microflora which enter the environment directly or through wastewater. These bacteria play an important role in the development of nosocomial infections due to their ability to acquire resistance genes and their transmission to other bacteria, particularly Staphylococcus aureus. The aim of this study was to identify the prevalence of vancomycin-resistant enterococci (VRE) and to detect van A, van B and van C1/C2 genes in VRE strain isolated from environmental samples of the in southern Fars province.
Methods: This cross-sectional study was carried out on 155 Enterococcus spp isolates collected from environmental samples (hospital wastewaters and surface waters) in different areas of Larestan and Jahrom cities. Isolates were identified and confirmed as Enterococcus spp. using the membrane filtration method, selective growth on Kenner Fecal Streptococcus Agar (KF) medium and biochemical tests. The disk diffusion test and Macro Broth dilution method based on CLSI guidelines were used to determine antimicrobial susceptibility against conventional antibiotics and vancomycin and to measure the minimum inhibitory concentration (MIC), respectively. Finally, the presence of van A, van B and van C1/C2 genes in VRE strains was determined by multiplex PCR technique.
Results: Out of all of Enterococcus spp. isolates, 41 cases (26.45%) were belonged to E.faecalis, 6 cases (3.87%) to E.faecium and 108 cases (69.68%) to non-faecalis and non-faecium. In total, 46 isolates (29.67%) were resistant to vancomycin and 4 isolates showed MIC ≥128 μg/ml. Resistant to all types of antibiotics was observed in 4 isolates (8.70%). Further, 2 isolates (50%) had vanA gene and 2 isolates (50%) had vanB gene, but vanC1/C2 genes were detected in none of them.
Conclusion: The results indicated that the VRE strains are widespread in the studied area, therefore there is an urgent need for prudent use of vancomycin and implementation of control measures to prevent the environmental spread of VRE strains.

Abstract

 
Background & objectives: The aim of the present study was to evaluate 8 weeks individualized balanced low calorie diet on anthropometric measurements in apparently healthy obese women.
Methods: Forty apparently healthy obese women (age: 18-40 years and 27.5≤BMI≤40 kg/m2) were recruited from the nutrition clinic in Ardabil city. The participants received a balanced low calorie diet after anthropometric measurements. Paired T-test was used to compare the changes during 8 - week intervention.
Results: Individualized balanced low calorie diet resulted in a significant decrease in body weight (-3.99±0.34 kg, p<0.001), body mass index (-1.61±0.14 kg/m2, p<0.001), waist circumference (-2.91±0.32 cm, p<0.001), hip circumference (-1.89±0.37 cm, p<0.001), waist to hip ratio (-0.01±0.004, p=0.001), waist to stature ratio (-0.02±0.002, p<0.001), body adiposity index (-0.97±0.15%, p<0.001), abdominal volume index (-1.07±0.11, p<0.001) and conicity index (-0.007±0.002, p=0.001) during 8 weeks.
Conclusion: Based on the results, an individualized balanced low calorie diet induced weight loss and decreased other anthropometric measurements during 8 weeks. Balanced low calorie diet-induced moderate weight loss alone can be a good way to reduce metabolic risk factors in obese women. .

Abstract

 
Background & objectives: Impaired immune system provides favorable conditions for colonization by Acanthamoeba in the human body. In this case control study, we compared the molecular and culture methods in identifying Acanthamoeba in the nasal and oral secretions of HIV+/HIV­ human.
Methods: In a current case control study, nasal and oral discharge of 53, HIV+ patients and 53, HIV­ people were evaluated. The nasal and oral secretions of each patient were prepared by sterile swabs and transferred to the laboratory. All samples were cultured but only the positive samples used for molecular analysis.
Results: By cultivation method, of the 53, HIV+ patients, a total of 11 samples, including 5 nasal and 6 oral samples, were contaminated with Acanthamoeba. Of the 53, HIV­­ people, 3 samples of nasal discharge were contaminated with this parasite. The molecular method approved the contamination of 10 samples, including 5 oral and 5 nasal samples from HIV+ patients with this parasite. Statistical analysis showed the rate of infection in HIV+ patients was significantly different compared to HIV­ people
Conclusion: The results of the current study showed that the rate of Acanthamoeba infection in HIV+ patients was higher than that of HIV- individuals. Also, considering that in the control group (HIV- individuals) only the nasal discharge were infected with the parasite, it seems that in the case group (HIV + patients) the infection of the oral discharge with the parasites is due to the  entry of its cysts into the nose and transmission to the mouth

Abstract

 
Background & objectives: Considering the high prevalence of body image dissatisfaction and its consequences, it is necessary to consider effective interventions to reduce it. The aim of this study was to investigate the effect of a balanced low calorie diet (BLCD) and slight weight loss on the body image score and its subscales.
Methods: In this clinical trial, 74 apparently healthy, non-pregnant, non-lactating and non-menopausal women (age range: 18-49 years with body mass index range: 27.5-40 kg/m2) were selected using random sampling method from a nutrition clinic in their first visit in Ardabil city between November 2019 and February 2020. Data collection was done by Anthropometric measurements and multidimensional body self-relations questionnaire. The paired T-test was used to compare the changes during 4 - week balanced low calorie diet intervention. BLCD was an individualized diet with an energy deficit of 500 calories of daily energy intake that was calculated from 3-day food records for every person.
Results: After 4 weeks, BLCD resulted in a significant decrease in body weight (p=0.001), body mass index (p=0.001), waist circumference (p=0.002), waist to height ratio (p=0.012), abdominal volume index (p=0.009) and conicity index (p=0.045) and a significant increase in the subscale scores of fitness evaluation (p=0.01), health orientation (p=0.03), body area satisfaction (p=0.01) and overweight preoccupation (p=0.001).
Conclusion: According to the results, it seems that even slight weight loss with a balanced low-calorie diet can be effective in improving anthropometric indices and some subscales of body image in 4 weeks. These results support the effectiveness of short-term interventions.

Abstract

 
Background & objectives: Vitamin D deficiency and poor sleep quality are important factors in health disorders and are common among women. The aim of this study was to investigate the effect of physical activity on vitamin D levels and improving sleep quality in women.
Methods: This descriptive-analytical study was conducted on active and inactive women (85 individuals in each group), aged 30-48 years that were selected using convenience randomized sampling. The Pittsburgh Sleep Quality Questionnaire was used to assess sleep quality and a score higher than 5 was determined as poor sleep quality. For analysis of data, Kruskal Wallis and Mann Whitney U tests and Spearman coefficient were used at a significance level of 0.05.
Results: The results showed that 54.12% and 28.82% of women had a deficiency and insufficient levels of vitamin D, respectively, and 61.8% had poor sleep quality. Active women had higher levels of vitamin D and better sleep quality compared to inactive women (p<0.05). There was also a direct relationship between vitamin D levels and sleep quality in active and inactive women. However, this association was significant in active women with vitamin D deficiency and inactive women with different vitamin D status.
Conclusion: It seems that physical activity can increase vitamin D levels and improve sleep quality in women. But vitamin D status, especially its deficiency, may be one of the most important determinants of sleep quality in active and inactive women.

Abstract

Background & objectives: Long QT syndrome (LQTS), congenital or acquired disorder, is characterized by a prolonged QT interval associated with syncope attacks and sudden death. Jervell and Lange-Nielsen first described LQTS in conjunction with sensory-neural congenital deafness. In long QT syndrome, the modified QT interval is greater than 440 ms and 460 ms in men and in women respectively. Acquired forms of prolonged QT are the consequence of metabolic disorders and various medications.
Since the number of people with congenital deafness in the society is not low and studies in Iran about this syndrome, especially in children are limited, therefore identification of these patients by performing an ECG and simple measures, may be useful in reducing sudden death among these patients. The aim of this study was to determine the prevalence of prolonged QT syndrome in children with congenital sensory-neural deafness which undergoing cochlear implant surgery.
Methods: In this study, a single-center cross-sectional observational study, all children with congenital deafness which undergoing cochlear plantation at northwest of Iran were enrolled. Patients' heart rate and QT interval were calculated using Bazett's formula. The association of long QTc with syncope and sudden death, evaluated by using Schwartz's criteria in children with long QT syndrome.
Results: Of the 357 studied patients, 204 (57.1%) were male and 153 (42.9%) were female. The mean±SD of the current age of the patients was 7.15±2.93 years with a mean of 7 years. The mean±SD of QT interval in the studied patients was 291.01±26.89 ms with a mean of 280 ms. the mean±SD of the  QTc interval in the studied patients was 400.52±25.74 ms with a median of 404 ms. Frequency of Long QTc in the studied patients was 17 cases (4.8%) and long QT syndrome was 5 cases (1.4 %).  
Conclusion: The prevalence of prolonged QT syndrome in patients with congenital deafness in northwest of Iran in 2019 was 4.8%, and 2 cases had syncope and 3 cases had sudden death.

Abstract

Background & objectives: Covid-19 disease with widespread prevalence in 2019 worldwide has been identified as the biggest health threat, according to the definition of the World Health Organization (WHO), especially for children. In addition, sleep disorders are one of the consequences of this emerging disease; the aim of this study was to investigate the nutrition and sleep disorders of active and inactive children during the corona pandemic.
Methods: Random multistage cluster sampling was used in four geographical regions of Iran (north, south, west and east) in this research. This study was conducted in 2021 which the questionnaires were gathered from the cities and the sample size (194 people) was determined based on G-Power 3.1 software with 95% confidence and 80% statistical power (evaluation through virtual networks and web questionnaire). The Eating Attitudes Test (EAT-26) Questionnaire, the Children's Sleep Habits Questionnaire (CSHQ), and the International Physical Activity Questionnaire (IPAQ) were used. Mann-Whitney U and T-Student tests were used to compare the two groups of active and inactive children. The significance level was considered at p≤0.05.
Results: The results showed that in the Covid-19 pandemic, the group of active children had a better level of physical activity (energy consumption) and status compared to the inactive children (p≤0.001). Also, it was shown that girls had more nutrition disorders than boys (p≤0.05), but there was no significant difference between sleep disorders between boys and girls, and there was no significant difference between nutrition disorders in active and inactive children.
Conclusion: In summary, during the Covid-19 pandemic, there was no difference in eating and sleeping disorders in active and inactive children, while gender is associated with nutrition disorders in children during Corona disease and was higher in girls than boys. It is suggested that parents pay more attention to physical activity and nutrition disorders in girls during home quarantine

Abstract

Background & objectives: Considering the importance of professional commitment in medical services, it is necessary to provide an appropriate questionnaire to measure professional commitment and behavior. Therefore, the present study was conducted with the aim of investigating the psychometric properties of the questionnaire "Patients' views regarding the observance of professional behavior by specialist doctors".
Methods: The present study is a psychometric research that was conducted in four phases. The first phase of this study was based on a professional behavior tool guide, review of texts and collection of experts' opinions. in  the second phase , determining the validity of items in the form of the face and content validity, in the third phase , the pilot study, and in the fourth phase,instrument reliability was conducted in two ways: internal consistency and instrument stability.
Results: A pool of 50 items was formed to design the questionnaire. After interviewing the study population and removing overlapping items, the number of items was reduced to 31 items. The results of CVR calculation showed that out of the total of 31 items, 13 items had a cutoff point less than 0.42, which were removed, and 18 items with a CVR score > 0.62 remained according to the opinion of the research team. In the face validity stage, the Impact factor < 1.5 was used as the basis for exclusion, and all the items had a high impact factor, and finally, the draft of the questionnairewas approved with the same 18 items. Cronbach's alpha coefficient was 0.88 and the internal correlation coefficient of the questionnairewas 0.93, which indicated the appropriate reliability of the questionnaire.
Conclusion: Based on the results of the present study, the questionnaire on "Patients' views regarding the observance of professional behavior by a specialist physician" in Iranian society has the necessary content and face validity and adequate reliability. By using this questionnaire, hospital officials can check the status of professional ethics of their specialist physician from the patients' point of view and take effective measures to eliminate the shortcomings.

Abstract

Background & objectives: Obesity and vitamin D (VD) deficiency is associated with poor sleep quality (SQ), and lifestyle changes can effectively improve SQ. The aim of this study was to investigate the effect of high- intensity interval running training (HIIRT) with VD intake on the brain-derived neurotrophic factor (BDNF) and SQ in young women with VD deficiency.
Methods: This clinical trial study was conducted on 52 women with an average age of 25.98±1.95 years, body mass index of 27.54±1.13 kg/m2 and low VD status. After being selected by the available sampling method, the subjects were randomly divided into control, running training, VD and combined groups. HIIT performed in 12x1-min running bouts at 80-90% HRmax interspersed with one minute of active recovery. VD was consumed weekly with a dose of 50,000 units. SQ was measured using the Pittsburgh Sleep Questionnaire. Data were analyzed using one-way ANOVA with repeated measures, one-way ANOVA, Kruskal-Wallis and paired t- test (p<0.05).
Results: After 8 weeks, the levels of BDNF in the experimental groups increased and the SQ score decreased significantly (p<0.05). But the combined intervention had more effect on the change of SQ scales (p<0.05) and BDNF (p<0.001) compared to the other two interventions (p<0.05). In addition, no significant difference was observed between the changes in the SQ score of the running training and VD groups (p<0.05).
Conclusion: Lifestyle changes such as HIIT, VD intake, and combined intervention can lead to improvement of SQ through the up-regulation of BDNF in women with overweight and VD deficiency. Also combined intervention may have more potent effects on improving these variables.

Abstract

Background & Objective: Hepatitis B virus [HBV] infection is one of the most important public health concerns. The present study was conducted to provide a descriptive analysis of detected HBV infection in Ardabil province.
Methods: In this retrospective longitudinal study, the hepatitis B surveillance database of Ardabil province was examined from 2014 to 2018. The frequency of hepatitis B virus infections was presented based on age, gender, marital status, place of residence, primary symptoms, and reporting sources. Also, the trend of changes in the number of detected cases over time was investigated using time series analysis.
Results: A total of 817 positive cases with a mean age of 43.1 years were reported. Most of them were male [n=503, 61.5]. The Frequency of identified positive cases decreased over time. Most of the infections were asymptomatic [87.5%]. Almost 90.0% of patients with HBV were married and the majority of the identified patients were resided in urban settings [n=598, 73.2%]. Most diagnostic tests were done voluntarily [n=219, 26.8%] or during prenatal care [n=109, 13.3%]. Most cases were identified and reported by the health network [n=292, 35.7%] and blood transfusion organization [n=190, 23.2%].
Conclusion: Decision-makers of the health sector of the province can use this information as a source for evidence-based decision-making in the effective management of the disease.

Abstract


Background & objectives: Maintaining the capacity of mitochondrial biogenesis during aging is considered a key factor to prevent the development of age-related diseases. this study aimed  to investigate the effect of 8-weeks of high-intensity interval training (HIIT) and hawthorn extract on the peroxisome proliferator- activated receptor-γ gamma coactivator-1α (PGC-1α) and mitochondrial transcription factor A (TFAM) proteins levels in the soleus muscle of aged rats.
Methods: Thirty-six aged rats were randomly and equally assigned into control (CON), HIIT, extract or combined (COM, HIIT+ extract) groups. The HIIT protocol consisted of 6 to 9 sets×5-min of running on a treadmill, which was performed 4 minutes at 16 m/min and one minuteat a speed velocity of 25 m/min, 5 times a week for 8 weeks. Over the intervention period, rats in the extract and COM groups received 100 mg/kg per day hawthorn. After the interventions, TFAM and PGC-1α protein expression levels were detected in the soleus muscle using the western blotting method.
Results: There was a significant increase in PGC-1α for all interventions when compared to the CON, although this increase in the COM was significantly higher than the other groups. After 8 weeks , there was a significant increase in TFAM level in  extract group compared to CON,also, the training groups had more effects on this marker in comparison to the former intervention (p>0.05).   
Conclusion: It seems that, 8-weeks of the HIIT+ intake hawthorn is a more efficient approach in improving mitochondrial oxidative capacity compared to HIIT and extract alone.

Abstract

Background & Objective: Futile or fruitless life-saving interventions are in conflict with the four principles of medical ethics, which are not only ineffective for the patients, but also have a high burden on society. Therefore, this study aimed to determine the causes of unsuccessful life-saving treatment and interventions from the point of view of care providers in the neurology and neurosurgery departments of Tehran University of Medical Sciences hospitals.
Methods: The present study is a descriptive cross-sectional study that was conducted on 64 doctors and 72 nurses working in hospitals affiliated to the Tehran University of Medical Sciences in 2018. A 25-item researcher-made questionnaire was used to collect data. Spss-24 software and descriptive statistics tests were used to analyze data.
Results: The average age of doctors and nurses was 34.25±5.1 and 35.12±8.2 years, respectively. From the point of view of physicians and nurses, the most important reason for the ineffective provision of life-saving services was the lack of legal, moral and Islamic permission and guidelines to cut off life support devices and the lack of resuscitation in patients facing death (end stages of life).
Conclusion: The results of this study showed that, from the point of view of nurses, the most common reasons for providing futile care and cure are the moral and humane obligation of the treatment and care team to provide ineffective life-saving interventions, and from the point of view of the physician, the lack of legal, ethical and religious permission and instructions to cut off life support devices in patients has resulted in death. Some of the causes of unnecessary care were presented in this study, and some of these causes cannot be solved without solving theoretical problems and practical expert studies. Developing guidelines to define futile treatment, especially in end-of-life care, and providing solutions to control and manage treatment and unsuccessful life-saving interventions seems necessary.

Abstract

Leishmaniasis is a parasitic infectious disease that accounts for approximately one million new cases annually. The treatment of this disease is complex and costly, particularly in developing countries. Numerous studies have been conducted on various vaccines utilizing live attenuated parasites, killed parasites, subunit antigens, recombinant vaccines, and DNA technology. However,  an effective and widely applicable preventive vaccine for leishmaniasis has not yet been developed. Present study aimed  to examine the progress in the development of leishmaniasis vaccines.
Articles for this study were selected from the PubMed, Web of Science and Scopus databases using relevant keywords, focusing on subject matter, scientific quality, and publication date, with an emphasis on more recent publications.
Research on the development of leishmaniasis vaccines indicates that several candidates, such as Leishmune, CaniLeish, and Leish-Tec, which are at various stages of clinical trials, may serve as suitable options for controlling and preventing leishmaniasis in dogs. The LeishChim vaccine, designed using immunoinformatics and molecular docking techniques, has shown promising efficacy results in mouse studies. Additionally, the mutant gene-centered LmCen-/- vaccine has completed Phase 1 clinical trials in humans.
Given the importance of developing a leishmaniasis vaccine, research in this area continues. Utilizing immunoinformatics and biological modeling studies can aid in the faster identification of effective vaccine candidates.
