Abstract
Background & objectives: Trichomonas vaginalis is a protozoan parasite lives and also causes symptoms in urogenital system of human. Trichomoniasis is the widest spread infection which is transmitted through sexual contact and more than 170 million people are a ffected with Trichomonas vaginalis in the world. Trichomonas vaginalis in women causes pelvic inflammatory disease, increase in the risk of fallopian tube dependent infertility, ectopic pregnancy, preterm labor, the birth of low weight infants and the increase in the possibility of HIV transmission. Considering the importance of Trichomonas vaginalis infection in pregnant women and lack of data from Ardabil city, this study was performed to identify this infection in pregnant women referred to health and medical centers of Ardabil city.
Methods: Vaginal discharge from 500 pregnant women was collected with sterile swap and disposable speculum and examined for Trichomonas vaginalis by direct microscopic examination and cultured in Diamond specific medium. A testimonial and questionnaire were completed for each case and the results analyzed using chi-square test by SPSS statistical software version 19.
Results: In this study, the culture of samples displayed 12 positive cases (2.4%). Furthermore, Trichomonas vaginalis trophozoites were observed in five cases (1%) with direct microscopic examination. Among 12 positive cases, five respondents (41.7%) were in age range of 16-25 years and seven (58.3%) in 26-35 years old. Among different clinical manifestations there was a significant relation between discharge and the infection.
Conclusion: The present survey confirmed the presence of Trichomonas vaginalis infection in pregnant women of Ardabil city. Therefore, an effective healthcare program by health authorities for prevention of infection in this group seems to be needed.

Abstract
Background & objectives: Parasitic infections are among important health problems all over the world especially in developing countries. Considering the epidemiological importance of parasitological diseases and necessity to evaluation the parasites prevalence in different areas and populations, current study aimed to determine the prevalence of intestinal parasites in patients admitted to the laboratories of Baqiyatallah Hospital in Tehran.
Methods: This is a retrospective cross sectional study in which stool examination reports of all referred patients (70978) to central and emergency laboratories of Baqiyatallah hospital were evaluated during 2010-2014. In this course, at least one stool sample of patients was assessed by direct smear and formalin-ethyl acetate methods. The data were analyzed using descriptive statistics in SPSS 16. Chi-square test was used for comparison of the prevalence.
Results: From 70978 patients, 42421(59.77%) and 28557(40.23%) were male and female, respectively. From 2617 infected individuals, 1841(70.3%) and 776 (29.7%) were males and females, respectively. The prevalence of pathogen and non-pathogen intestinal parasites was 2283(87.23%) and 334(12.76%) respectively. Also the prevalence of intestinal helminthes and protozoa in infected population was 0.42% and 99.57%, respectively. Blastocystis hominis and then Giardia lamblia were the prevalent parasites.
Conclusion: In current study the prevalence of parasites is lower than those reported in other investigations. Differences in geographical location, sample size, duration and type of study (prospective or retrospective) and study population may be effective agents. The higher prevalence of "Blastocystis hominis" in comparison with other parasites can attract more attention to improve the quality of laboratory and clinical diagnosis of this protozoa

Abstract
Background & objective: One of the objectives of health reform plan in Iran is to reduce the rate of cesarean section delivery. This needs research and use of results to improve the maternal and neonatal outcomes. The aim of this study was to compare the maternal and neonatal outcomes after normal vaginal and cesarean section delivery in hospitalized low-risk mothers in Arash Hospital in Tehran, Iran.
Methods: In this prospective cohort study, 1900 low-risk pregnant women, with 37-40 weeks of gestational age, referring to the emergency department and being hospitalized for delivery were included. The data were collected from two groups of patients (normal vaginal delivery and cesarean section delivery). Data on wound infection, abnormal postpartum hemorrhage, decrease of hemoglobin, transfusion requirement, injuries to the genitourinary system, fever until 10 days after delivery, neonatal respiratory complications, NICU admission, jaundice and obstetric injuries were collected by a questionnaire.
Results: From 1900 pregnant women, 62.1% had cesarean delivery and 37.9% had normal vaginal delivery. The reasons for cesarean delivery included repeated cesarean (68%), failure to progress (18.1%), and fetal distress (13/9%). Wound infection (p=0. 004), abnormal postpartum hemorrhage (p=0.042) and low hemoglobin (p<0.001) were more frequent in cesarean delivery group than in normal vaginal delivery group. But obstetric injuries were higher in normal vaginal delivery group (p<0. 0001). There was no difference between the two groups in terms of fever ten days after delivery and blood transfusion requirement. For neonatal outcomes, transient neonatal tachypnea (p=0.032), neonatal respiratory distress syndrome (p=0.002), and NICU admission (p<0. 0001) were more frequent in cesarean delivery group than in normal vaginal delivery group. There was no difference between the two groups in neonatal jaundice rate and neonatal injuries.
Conclusion: Due to the high rate of cesarean section delivery in our country and higher rate of maternal and neonatal complications after cesarean section delivery, appropriate strategies are required to be applied to decrease unnecessary cesarean section delivery and increase normal delivery with minimum maternal and neonatal complications.

Abstract
Background & objectives: Treatment of dysmenorrhea in women is aimed to bring them to their normal condition. In the present study, the effect of mefenamic acid, a non-steroidal anti-inflammatory drug, was compared with that of transdermal glyceryl trinitrate (GTN) as a tocolytic drug in the management of primary dysmenorrhea.
Methods: A total of 160 nulliparous women aged 18-30 years with primary dysmenorrhea were included in this single blind, clinical trial, which was carried out from 2014 to 2015. The patients were randomly divided into two equal groups. At the beginning of menstruation cycle, the patients in group A received 500 mg oral mefenamic acid, followed by 250 mg mefenamic acid every 6 hours. The patients in group B initially were administered 2.5 mg transdermal glyceryl trinitrate 0.2% every 12 hours to the abdominal skin under the umbilical cord. Treatment was continued for up to 48 hours and repeated for three cycles. Pain scores were assessed by Numerical Rating Scale (NRS) every 4 hours. Adverse effects such as headache and gastrointestinal disorders were recorded. The decrease of pain scale was the primary outcome and adverse effects were the secondary outcome.
Results: The mean pain severity score in in the first 24-hours in mefenamic acid group was lower than that of the glyceryl trinitrate group (p=0.01). On the second day, the mean pain severity scores were not significantly different between the two groups. The mean pain severity scores in the second day of second cycle (p<0.001) and in the first day of third cycle (p=0.001) were significantly lower in mefenamic acid group than in glyceryl trinitrate group. The side effects were also higher in the glyceryl trisitrate group than in the mefenamic acid group, but this difference was not statistically significant. The most common complication was headache in the group receiving glyceryl trinitrate (18.75%) and nausea in the group receiving mefenamic acid (26.25%). The satisfaction rate was 42.2% in the patients receiving transdermal glyceryl trinitrate, while it was 78.5% in patients receiving mefenamic acid; therefore, the patients in the mefenamic acid group were more satisfied (p=0.004).
Conclusion: The analgesic effects of oral mefenamic acid were better than transdermal glyceryl trinitrate in the management of primary dysmenorrhea. The adverse effects of these two drugs were not significantly different, but the type of complications was different in both groups.

Abstract
Background & objectives: Due to the increasing prevalence of bacterial resistance, there is a growing attention to application of auxiliary treatment of pharyngitis. The aim of this study was to investigate the effect of honey throat spray in the auxiliary treatment of patients with a diagnosis of pharyngitis.
Methods: At the start, different physicochemical quality parameters of the honey sample were evaluated. Then, the bottles of honey and the placebo throat spray were prepared. The numbers of 70 patients with the diagnosis of pharyngitis were included in a one-step and double-blinded study. In addition to standard treatment, each patient received either one of the honey or placebo throat spray. The clinical symptoms of the patients were compared between the two groups at baseline and then at 24 hours, 48 hours, 5 days and 10 days after the starting the treatment procedure.
Results: As per results, the quality of honey sample was met to the standard honey criteria. The treatment and placebo groups were statistically similar in terms of demographic specifications or clinical symptoms at the time to enter the study. Pharyngitis recovery was better in treatment group than in the placebo group; this positive effect was statistically significant for symptoms of morning phlegm and painful swallowing, 24 hours after the starting treatment. In treatment group, after 48 hours, symptoms of fever and painful swallowing were resolved, while this indicator was 23% in placebo group. Also, throat pain of 85.71% of patients in treatment group and 17.14% of patients in placebo group was relieved. Overall, the mean recovery time in treatment group was shorter than placebo group.
Conclusion: The findings of this study showed that the use of honey throat spray as auxiliary treatment of pharyngitis can improve the clinical symptoms of pharyngitis and reduce recovery time.

Abstract
Background & objectives: Over the last couple of decades, research has focused on attempting to understand the genetic influence on sports performance. Whole exome sequencing (WES) has great potential to explore all possible genetic variants associated with physical performance. However, this new technique has not been used in sports genetics research. Therefore the aim of this case study was to report new variants in genes involved in skeletal muscles strength (ACE, NOS3, IGF1R, IL-6) in an elite weightlifter using WES.
Methods: After obtaining a written testimony from a 32-year-old man, who is a member of the Iranian national weightlifting team, 5 cc of blood was taken and then DNA was extracted. DNA samples were analyzed using the whole exome sequencing method.
Results: Whole exome sequencing analysis revealed a missense variant (99454613G>A) in IGF1R, 22771156C>T polymorphism in IL-6, 150695726T>C and 150704250C>G polymorphisms in NOS3 and 61573761T>C and 61564052A>G in ACE genes.
Conclusion: Results of whole exome sequencing analysis revealed a missense variant and polymorphism in muscle strength and hypertrophy related genes in this elite weightlifter. However, other studies with larger sample sizes are needed to investigate the relationship between these variants and power athlete status.

Abstract
Background & objectives: Caffeine can affect many physiological functions of the body. The aim of this study was to investigate the response of aerobic function, blood pressure and some immune system factors of trained men to acute caffeine consumption in dehydrated condition and warm and humid environment.
Methods: Thirty male athletes with a mean age of 26.6±3.9 years in dehydrated conditions voluntarily participated in this study, which was performed in a double-blind manner. Subjects were divided into three groups: caffeine consumption group (n=10), placebo group (n=10) and control group (n=10). The Caffeine group consumed 6 mg/kg body weight of caffeine. Sixty minutes later, the subjects performed an increasingly exhausting exercise. Blood samples collecting and blood pressure measuring took place before, immediately after and 24 hours after exercise. Analysis of variance with repeated measures (group * time) and one-way analysis of variance were used for statistical calculations.
Results: The duration of the exhaustion test in the caffeine consumption group was significantly longer than the control (p≤0.01) and placebo (p≤0.05) groups. In the blood sampling immediately after exercise, the level of leukocytes, neutrophils and lymphocytes in the exercise groups were significantly higher than the control group (p≤0.001). Also, the caffeine group experienced a lower increase in leukocytes and neutrophils compared to the placebo group (p≤0.001). Blood pressure values in the exercise groups were significantly higher than the control group (p≤0.001). Also, the caffeine group experienced a greater increase in systolic blood pressure at baseline and immediately after exercise compared to the placebo group (p≤0.001).
Conclusion: In general, exhausting exercise in a warm environment and dehydrated condition increases the number of immune system cells and blood pressure. In addition to improving aerobic function, caffeine seems to prevent further increases in the number of immune system cells, while increasing blood pressure in the normal range in warm and humid environment.

Abstract
Background & objectives: Coronary heart disease (CHD) is an important medical disorder and one of the most common heart diseases worldwide, which causes disability and economic burden. The medical and research community is increasingly interested in computer-aided coronary heart disease diagnosis through the use of machine learning methods. This study aimed to diagnose coronary heart disease using a discriminant analysis algorithm in active elderly men.
Methods: This analytical study was conducted on 351 patients of Ayatollah Kashani Hospital in Tehran. This work used discriminant analysis algorithm to diagnose coronary artery disease. Python software was used for data analysis.
Results: The results showed that by using 14 characteristics as risk factors related to the subjects' laboratory, personal and lifestyle information. The discriminant analysis algorithm could distinguish healthy and sick people with 94.4% accuracy and 88.9% precision.
Conclusion: The results of the present study showed that this system can probably be used as an effective and intelligent method along with other diagnostic methods by cardiologists to predict coronary artery disease. Also, new data mining methods can be effective in reducing invasive risks.
