Abstract
Background & Objective: Limited information is available on adolescent’s substance abuse in Iran. The aim of this study is to estimate prevalence of substance abuse among the sample of sophomore male students in Tabriz city, and to evaluate the associated factors. Methods: Of all sophomore male students in Tabriz, 1785(13.7%) were randomly sampled, and a self-administered questionnaire with 48 questions was distributed to students in February and March 2005. The questions aimed to obtain information on substance abuse, smoking, self-confidence, tendency toward smoking and risk-taking behaviors as well as demographic information. The influence of different factors on substance abuse was evaluated with a logistic regression model. Results: The mean age of the subjects was 16.3±0.87 (min. 15, max. 19) years. Among 1785 students 226(12.7%) had the experience of taking alcohol and 36(2.0%) had used substances. The results indicate that, older age (OR= 1.55), having general risk taking behavior (OR= 1.70), higher smoking stage (OR= 3.70), having masochism (OR= 1.22), high socioeconomic class (OR= 1.62), and use of illicit substances (OR= 5.72) were factors associated with prevalence of the use of alcohol. Also higher smoking stage (OR= 3.26), high socioeconomic class (OR= 1.52), and the use of alcohol (OR= 7.81) were factors associated with prevalence of the use of substances. Conclusion: This study showed the low prevalence of substance abuse, and determined some of its risk factors among students. This study may be supportive of interventions that target multiple behaviors. The findings of this study is important for public health officials however, more studies are necessary to generalize the findings.

Abstract
Background & Objective: Herbal medicine has an old history with a broad application all over the world. Many researches have focused on the curative as well as antinociceptive effects of herbal extracts. In the previous study the analgesic effect of Chamomile extract containing 2 mg/kg essence in Rats which revealed a significant analgesic effect were studied. In this study it is planned to compare the analgesic effect of chamomile extract and Morphine (as a standard analgesic) in mice. Methods: This experimental study was carried out in Shahid Sadughi Medical School on 48 Syrian mice (25-30 grams) which were randomly divided into 8 groups. In this study the analgesic effect of intraperitoneal administration of Chamomile extract containing 2 mg/kg essence and different doses of Morphine (0.5, 1.0 and 2.0 mg/kg) were assessed by using Formalin Test (for chronic pain during 1 hr. post Formalin injection) and Tail Flick Test (for acute pain during 2 hr. post drug administration in 15 min. time intervals). Results: The results of this study showed that 2nd phase of Formalin Test had more analgesic effect than that of 0.5 mg/kg morphine. In the case of Tail Flick Test its analgesic effect was prominent 30-90 min. after drug administration which was identical to the analgesic effect of 1.0 and 0.5 mg/kg Morphine Sulfate (P > 0.05). Conclusion: Data from this study confirms the analgesic effect of chamomile essence which was indicated in our previous study and that this analgesic effect is comparable with 1 mg/kg of morphine sulfate in both the Formalin TEST & Tail Flick TEST. Chamomile as an analgesic should be studied more in different studies.

Abstract
Background & Objectives: Esophageal carcinoma has more prevalency in northwest of Iran. Relapsing dysphagea after esophagectomy is one of the common delayed complication. It emerges due to technique of surgery. Reflux and or leakage in anastomotic area. The aim of this study is to evaluate the prevalence and etiologies of dysphagea after esophagectomy and postoperative endoscopic dilation results. Methods: This is a retrospective discreptive study, during 2000-2005 at Imam Khomeini hospital, all patients who had esophagectomy because of esophageal carcinomas entered this study. Data collected from medical records, Primary tumoral diagnosis, operation endoscopy, dysphagea, histopathological findings, Postoperative complications, especially dysphagia and it’s etiology, and different methods of therapy, reoperation and or endoscopic dilatation were studied. Results: Among 324 patients 81 patients had esophagectomy for esophageal carcinomas. In eighteen patients (22.2%), dysphagea reappered after operation. Rate of dysphagea in patients with carcinoma of lower one third area was 5.3 times greater than middle third of the esophagus. Mean time between first hospitalization and second hospitalization due to relapsing dysphagea was 10.03 months. The main cause of relapsing dysphagia in 88.24%, was benign stricture in anastomotic area. 17 patients with endoscopic dilation and one patient with surgical stricturoplasty, were treated. Conclusion: Relapsing dysphagea after esophagectomy is common in our patients. This represents mostly benign strictures after esophagectomies for esophageal carcinoma. In benign anastomotic strictures, endoscopic dilation is an effective treatment and it is recommended.

Abstract
Background and Objectives: Three major semen parameters that involves on male infertility are sperm count, motility and morphology. In 50% of couples referring to physicians, because of infertility, male factor, solely or accompanying with female factor is responsible of infertility. Some of drugs are used to improve the semen parameters. The goal of this study was to study the effect of pentoxifylline on semen parameters (count, motility, morphology, volume of semen). Methods: Sixty one patients with impairment in semen parameters were selected and semen samples requested from each participant for analysis before treatment. Then patients were treated with pentoxifyllin, 400 mg three times a day, for three months. After that we took semen specimens again and analyzed them. Results: Mean total sperm count in case group before treatment with PF was (17 millions) and after treatment with PF reached to (21 millions). In 68.9% of patient, Sperm count increased (p<0.001) Mean motile sperm percent in case group before treatment with PF was 19.42% and After treatment with PF reached to 28.78% and 88.52% of patient this parameter increased (p<0.001). Parameter motility showed the most important after treatment. Mean normal morphology of sperm percent in case group before treatment with PF was 24%and after treatment with PF reached to 26.39% and in 32.8% of patient this parameter increased (p<0.001).Mean semen volume in this group before treament with PF was 1.95ml and after treament with PF reached to 1.93 ml. There was no significant variation in semen volume after treament with PF. (p=0.321) Conclusion: Results show that Pentoxifylline has significant efficacy for increasing sperm count, motility and morphology correction (specially for sperm motility). Considering that oral PF is safe and cheap, with easy application, we can use it for improving semen parameter’s quality before performance ART.

Abstract
Background & objectives: The hand is one of the sensivitive organs of the body. The hand injuries usually occur in workers and are one of the most common causes of disabilies. The aim of this study was to investigate the causes of the hand injuries leading to defect among industrial workers and farmers’ reffered to emergency department of Fatemi hospital in Ardabil. Methods: In a cross-sectional study, from January 2009 to Agust 2010, 100 patients with hand injuries were included. The data were collected by the chek list including: category of severity, time and type of incident, and location and cause of the injuri and then were analyzed statistically by Chi-square test. Results : Most of the industrial workers (58%) ranging as 20–29 years old and had 38 years as an average. Sixty two percent of them were working more than 8 hours per day and 64.7% experienced severe injuries in their hands. Amputation showed to be the most common injury in this group. Most of the farmers (30%) were in age group 20-29 years and from them 70% were working less than 8 hours per day. In this group, 54% of injuries occurred in left hand and the most common type of injuries was amputation (54%). Conclusion: The highest incidence of hand injuries has been shown to occur among industrial workers with 28 years as mean and working for more than 8 hours per day without any formal training before starting to work. So, it seems to be need we use the expert workers with the high level background and don’t use adolescents under 15 years.

Abstract
Background & objectives: Origanum Vulgare is an herbal plant which is widely distributed in the north and northwest parts of Iran and posses therapeutic properties. The aim of this study was to evaluate the effect of intracerebroventricular (ICV) injection of aqueous extract of Origanum Vulgare L. ssp. viride on pain threshold in male rats. Methods: In this study,28 Male Wistar rats (200-250 g) were divided randomly into 4 groups (n=7) . Rats were anaesthetized with a mixture of ketamine and xylazine and placed in stereotaxic apparatus. A guide cannula was inserted into ventricular area, according to the atlas of Paxinos and Watson. One week after surgery , three groups of rats received microinjection of Origanum extract (1, 3, 6µg/rat). Controls received the microinjection of the same volume of saline. Tail flick latency (TFL) was used to assess the nociceptive response each 15 min for 120 min, using tail flick test. The results were analyzed by repeated measurement test. One-way analysis of variance (ANOVA) was used to compare the mean values of quantitative variable among the groups. The data are expressed as mean ± SEM. p < 0.05 was considered significant. Results: Intracerebroventricular administration of the Origanum extract resulted in significant and dose-dependent increase in TFL compared to controls (p<0.05). Origanum extract at dose of 3µg/rat showed the highest analgesic response. The maximum analgesic response was observed at 60 min and 90 min post extract injection (p<0.05). Conclusion: These results suggest that the ICV injection of aqueous extract of Origanum Vulgare possesses dose- dependent antinociceptive activities in the tail flick test in rats.

Abstract
Background and Objectives: The importance of considering to job stress among women is not related to loss of economics alone, but it makes an unsafe condition in health of body and soul of person, family and society, too. This study was aimed to determine the rate of job stress among employed women in Ardebil during 2010. Methods: In a descriptive cross-sectional study, 1250 governmental-employed women were selected in different offices of Ardabil city. Data were collected by job-contained Karasek questionnaire and results were analyzed by chi-square test. Results: The results showed that 62.8% of women experienced medium level of stress and 36.5% revealed to have severe stress. Our findings significantly showed that there are relationship between stress rate of individuals with number of children, level of education, place of living, job experience, rate of work hours per day and night work shift, employment status, physical and mental diseases (p <0.05 ( . Conclusion: With respect to severe stress found in employed women in study setting, it seems to need to increase mental health indices in individuals, families and community, and the authorities should develop a comprehensive plan to enhance productivity and satisfaction among employees.

Abstract
Background and Objectives: Opiodergic system has important role in pain control. Origanum vulgare is a folk medicine with analgesic properties which is widely distributed in the north and northwest parts of Iran. The mechanism of therapeutic effects of Origanum vulgare is not understood. The aim of this study was to evaluate the intervention effect of opioid agonist (morphine) and antagonist (naloxone) on analgesic effects of Origanum vulgare. Methods: In this study 28 Male Wistar rats (200-250 g) were used (n=7). The rats were anaesthetized by ketamine (80mg /kg) and xylazine (10mg /kg) and a cannula was inserted into the left ventricle according to atlas of Paxinos characteristics using stereotaxic apparatus. The animals were allowed to recover for 5-7 days .In pilot examination, the effective dose of ORG extract determined 3µg/rat i.c.v. Rats were divided into 4 groups:Control group given saline 0.5ml.i.p/ saline 5µl.i.c.v or ORG 3µg/rat.i.c.v. other groups are morphine (2mg/kg ,i.p) and ORG3µg/rat,i.c.v , Naloxone (1mg/kg,i.p) and ORG 3µg/rat,i.c.v. The latency response of rats to thermal stimulation was recorded (30, 45, 60, 75, 90 &120 min after treatment) by Tail flick test. Repeated Measurement test and ANOVA were used to determine significant differences. Results: There was significant decrease in the pain threshold following the co-administration of ORG extract with naloxone in the Tail flick test. There also was significant decrease in the latency response or pain threshold 90 and 120 min after intervention in naloxone group compared with that in control group (p< 0.05). Conclusion: The results of this study showed that analgesic effect of aqueous extract of Origanum vulgare may be mediated, at least in part, by opioidergic system.

Abstract
Background & Objectives: I ntra-hippocampal adminestration of origanum (ORG) improves spatial learning of rats. T he aim of the present study was to investigate the possible mechanism for origanum extract on spatial learning and memory in the hippocampus. Methods : In this study 42 adult male Wistar rats were used . Animals were cannulated bilaterally in the posterior laterl of hippocampus. After the recovery period, the spatial learning and memory were assessed using Morris Water Maze (MWM). Saline, ORG (0.03μg/site) glutamate receptor antagonist MK801 (0.08, 0.2 and 0.4 μmol/site) and co-injected of MK + ORG was injected into the posterior lateral of hippocampus 20 minutes before the training and retrival sesions (for 5 consecutive days) (n=7). Results: The results showed that the intra-hippocampal injection of MK 801 significantly blocked the decreased distance and time of reaching (due to ORG injection) to find hidden platform of MWM (p<0.05). On the retrival tests, the average of time spent in the target area is reduced in the co-injected of MK801 + ORG group. Conclusion: The intra-hippocampal injection of aqueous extract oforiganum may improve working memory in rats through glutamate-dependent NMDA receptors.

Abstract
Background & Objectives : Nowadays, coronary artery disease is the most common cause of death in developed countries and in the whole world. According to the WHO reports, it will be the main cause of death in 2020. Myocardial infarction is the most common diagnosis among hospitalized patients in industrialized countries. This disease causes more mortality and morbidity than others. Coronary artery bypass grafting surgery (CABGS) is one of the common treatments for ischemic heart disease but it may have some complications. In this study we wanted to evaluate the incidence of CABGS complications in Imam Khomeini Hospital of Ardabil in the first month after CABG. Methods: This study was a cross sectional analytic descriptive type and performed on all of the patients underwent CABGS in Imam Khomeini Hospital during 2011-2012. All of the data from patients (like demographic, past medical history, physical exam findings, paraclinical findings and CABGS complications) were inserted in special forms after gathering, and analyzed by SPSS v.16. Results: In this study, 211 patients were studied. About 145 (68.72%) of them were male and 66 (31.28%) were female. Seventy patient (33.17%) were more than 70 years old. About 33.22% of patients had a history of cigarette smoking and 9.95% were opioid abused. The 34.12% of patients had hypertension (HTN) history, 40.28% diabetes mellitus (DM), 17.06% history of hyperlipidemia and 63.98% had a history of the previous MI. Chest pain was the most common complication among the patients (93.36%). According to our study the prevalence rate of post CABGS complications were: bleeding after surgery 13.27%, postoperative myocardial infection 8.05%, neurological disorders 12.32%, renal complications 2.36%, respiratory symptoms 11.37% and 34.59% for cardiovascular complications. Also the total mortality was 5 (2.36%) persons. The data analysis showed that there was a significant relationship between sex with survival status, local infection and neurological disorders, and also there was a significant relationship between age with post CABGS survival, bleeding after surgery and renal complications. There was also a significant relationship between DM and post CABGS survival, local infections and respiratory complications and between HTN and local infections. Conclusion: According to the results of this study, the most common post CABGS complications were cardiovascular complications, especially arrhythmias.

Abstract
Introduction: Complete Amputation of wrist is physical and psychological trauma that can affect person's life. During recent years, various amputated organs were replanted back into their own body leading to improvement in quality of life, function and body image. Case Report : The case was a34 years old man with complete amputation at left hand at wrist level, while working with chainsaw. Patient was admitted to the emergency unit at Fatemi hospital of Ardabil city, and underwent tosurgery for replantation. Conclusion: Complete amputation of wrist and its successful replantation are among rarely occured and reportable cases . The5 -month follow-up indicated a successful operation . During this period, no abnormalities were found in the blood circulation . The finger grasping and nerve development were acceptable and no symptom of infection was observed

Abstract
Background & objectives: Type II diabetes is a metabolic disorder accompanied with insulin resistance of the whole body cells and is considered be the fifth cause of death in the world. Adaptation to altitude can lead to tolerance to many diseases. Therefore, the aim of this study was to determine the effect of simulated intermittent altitude on the metabolic and hematologic parameters and liver function in streptozotocin induced diabetic rats.
Methods: In the current experimental study, twenty four male Wistar rats weighing 220±20 gr were randomly divided into three groups; normal control group (NC, n=8), diabetic control group (D, n=8) received fat diet for 2 weeks then were injected with streptozotocin (37 mg/kg) and diabetic+hypoxia group (D+H, n=8) including diabetic rat exposed to chronic intermittent hypoxia (PiO2≈106 mm Hg, simulated altitude≈3400 m, 14% oxygen for 8 weeks). Diabetic, hematologic and lipid parameters as well as ALT and AST activities were measured in peripheral blood.
Results: Our findings showed that intermittent hypoxia significantly decreased serum total cholesterol, LDL ,VLDL and triglyceride in D+H group compared to D group (p<0.05). Serum levels of fasting blood glucose and homeostatic model assessment-insulin resistance )HOMA-IR( index and ALT were decreased in D+H group vs. D group )p<0.05). Also, hemoglubin and hematocrite level increased in D+H group in comparison to D group )p<0.05). No significant difference was detected in red blood cell count in D+H vs. D group.
Conclusion: Based on resultant data, it seems that intermittent exposure to hypoxia (simulated to chronic and intermittent lodgement in altitude) can be used to control of type 2 diabetes by increasing hemoglobin, decreasing insulin resistance and improving liver function as well as lipid parameters.

Abstract
Background & objectives: Hip muscles insufficiency plays a significant role in deterioration of patellofemoral pain syndrome (PFPS), which can be manifested as myofascial trigger point (MTrPs) in hip muscles. Then, our purpose was to determine the prevalence of MTrPs in the gluteus medius (GMe) and quadratus lumborum (QL) muscles and to investigate the effect of a therapeutic intervention on pain intensity and hip abductor muscles isometric strength in patients with PFPS.
Methods: Forty volunteer subjects (20 patients and 20 healthy) participated in the study. Latent MTrPs in GMe and QL were evaluated and a handheld dynamometer was used to measure peak isometric strength test (PIST) for hip abductors. Patients with PFPS having MTrPs in GMe were randomly divided into either a treatment group (Mean age±SD: 23.2±4.3 years) or control (Mean age±SD: 24.4±4.6 years). The therapeutic intervention included trigger point pressure release (TrPPR) and Kinesio Taping® (KT). Pain intensity and PIST for hip abductors were assessed at baseline and after intervention in both groups.
Results: There is more significant patients with PFPS having latent MTrPs in GMe and QL than the healthy counterparts (p<0.001). Using TrPPR and KT significantly decreases pain in such patients (p<0.001); however, no significant effect was observed on hip abductors peak isometric strength (p>0.05).
Conclusion: Concomitant using of TrPPR therapy and KT method can decrease pain intensity in individuals with PFPS. Further studies are required to understand the underlying mechanisms.

Abstract
Background & objectives: According to the new theory of cancer stem cells, interruption in the self-renewal pathway of tissue stem cells can cause cancerous tumors. Current work has evaluated the role of self-renewal Oct-4 gene in thyroid tumors.
Methods: In this case-control study, the expression of Oct-4 gene has comparatively assessed between cancerous specimens, marginal tissues of tumors and non-tumoral nodules of thyroid using RT-PCR technique.
Results: Statistical analysis of data by one-way ANOVA showed that Oct-4 gene is significantly expressed in thyroid papillary carcinomas in comparison with tumor margin and non-tumoral nodules (p<0.05).
Conclusion: In conclusion, the dominant expression of Oct-4 gene in thyroid tumoral cells not only demonstrates the cancer stem cell theory but also shows its role in thyroid cancer appearance that can be used in differentiating thyroid papillary carcinomas from non-tumoral nodules as well as demarcation of tumors.

Abstract
Background & objectives: The use of supplements such as L-carnitine, optimum nutritional support and early feeding after surgery in cancer patients can be important in the prevention of cancer complications. The aim of this study was to determine the effects of early L-carnitine-rich feeding on complications of esophageal cancer surgery and duration of hospitalization.
Methods: In a clinical trial, 50 patients with esophageal cancer under surgery in two equal groups were randomly selected. Nutritional information was obtained using food record questionnaire. Basic blood samples and 24-hour urine samples were taken in order to measure the hematological parameters (like blood cell count, hemoglobin, hematocrit and blood urea) and nitrogen balance, respectively from two groups at the beginning and end of the study. Then, the intervention group received 3 g L-carnitine daily in 3 doses on the first day after the complementary operation, with a routine hospital diet at each meal, and the control group received a routine hospital diet for ten days. Data were analyzed using independent t-test, paired t-test and chi-square test.
Results: The results showed that changes in the mean BUN and nitrogen balance were significantly different after intervention between the study groups (p<0.05). Patients with early L-carnitine-rich feeding had less hospitalization time in hospital wards and were discharged earlier (p<0.05).
Conclusion: The results of this study showed that early L-carnitine-rich feeding after surgery has probably a positive effect on the duration of hospital stay in patients with esophageal cancer.

Abstract
Background & objectives: Type 2 diabetes mellitus is characterized by metabolic disorders and elevated blood glucose. It is one of the most common diseases in developed countries. Thyme is one of the herbs which have been used in some drug content due to its high antioxidant properties. Thyme can increase the activity of mitochondrial oxidative enzymes and affect metabolic and hematological indices due to its phenolic compounds.
So, the aim of this study was to determine the effect of Thymus migricus extract supplementation on the metabolic, hematological indices and oxidative stress in type 2 diabetic rats.
Methods: For this purpose, 24 Wistar rats (220-240g) were divided randomly into three groups; group1: healthy control group (NC, n=8), group2: diabetic control group (D, n=8) that took fat diet for 2 weeks then were injected with streptozotocin (37 mg/kg), and group3: diabetic+Thyme group (Th+D,n=8). Thyme hydro-alcoholic extract dissolved in distilled water to the desired concentration (400 mg/kg) according to their daily water consumption (30 ml). Then glycemic, blood and lipid indices were measured in the peripheral blood of the rats.
Results: The results revealed that thyme supplementation significantly decreased the fasting blood glucose level (356.35±40 mg/dl to 261.61±35 mg/dl) and HOMA-IR index (3.52±0.30 mg/dl to 2.2±0.33 mg/dl). Also significantly decreased the lipid profile level of the Th+D group compared to those of the D group (p<0.05), but the hematological parameters of the Th+D group compared to those of D group were not significantly changed. FRAP in Th+D group was not significantly different in comparison with the other two groups, but MDA was significantly increased in diabetic groups (p<0.05).
Conclusion: Based on the results, thyme supplementation by decreasing insulin resistance and fasting blood glucose can improve lipid parameters, but does not affect blood parameters and antioxidant indices.

Abstract
Background & objectives: Methicillin-resistant Staphylococcus aureus is one of the most common causes of nosocomial infections. The polysaccharide adhesion mechanism encoded by the ica operon generates a direct role in biofilm formation and infection of the bacteria. The aim of this study was to evaluate the frequency of icaA gene in Staphylococcus aureus isolates isolated from clinical specimens of patients admitted to some clinical centers of Rasht.
Methods: This descriptive, cross-sectional study was performed on 100 Staphylococcus aureus isolates from some clinical centers of Rasht in 2019 and confirmatory tests were performed to identify the bacteria. icaA gene identification and its frequency were investigated using molecular methods . The antibiotic resistance pattern against 10 antibiotics and biofilm-forming ability of the isolates were determined using the disk diffusion method and Congo red method respectively.
Results: In the present study, among the 100 studied isolates, the highest drug resistance was related to penicillin, and the lowest antibiotic resistance was belonged to ciprofloxacin. 81 isolates (81%) were resistant to methicillin and 37 isolates (37%) had multiple resistance. Of 37 isolates with multiple resistances, 32 isolates (86.48%) had icaA gene and 24 isolates (64.9%) ،had the ability to produce strong biofilms.
Conclusion: According to the findings of this study, the prevalence of Staphylococcus aureus isolates carrying icaA gene with strong biofilm forming ability and resistance to methicillin, were high. This necessitates the need for serious management of antibiotic administration.

Abstract
Background & objectives: Endoscopy is an invasive and painful procedure. The use of sedatives, especially in children, is expanding. The aim of this study was to compare the sedative effects of ketofol and the combination of midazolam-ketamine in upper endoscopy in children aged 1 to 14 years who referred to Taleghani Hospital in Gorgan.
Methods: In this study, 55 children entered the study 1 to 14 years after obtaining parental consent. They were then divided into two groups, ketamine-propofol (KP) and ketamine-midazolam (KM), by simple randomization. At all stages of the work, patients underwent cardiac and respiratory monitoring. All hemodynamic parameters and ramsy sedation score (RSS) were recorded for all participants in the 5 time periods before, during and after endoscopy. ANOVA test and bonferroni tracking test were used in spss software to analyze and compare data.
Results: 54.5% were male patients. The ANOVA test showed that the scores of the two groups at the time of the study had a statistically significant difference (p=0.02). In the KM group, the mean blood pressure and heart rate of patients increased at all times compared to before the intervention (p=0.005). Blood pressure in the KP group had a steady trend over time (p<0.001).
Conclusion: There was a statistically significant difference between the scores of the two groups and the group (KM) had higher scores, but the use of two drug regimens Propofol-Ketamine and Ketamine-Midazolam caused a good sedation in pediatric endoscopy. The propofol-ketamine compound resulted in greater hemodynamic stability in cardiovascular parameters during and after endoscopy

Abstract
Neuroarthropathy occurs following the loss of pain sensation in a joint. Martin Charcot was the first researcher who explained this strange, non-infectious destruction of bone and joints. This disease mostly affects the lower extremity of patients, especially their feet and ankle. In this paper, we present a rare case of simultaneous neuroarthropathy in the wrist and ankle of a 39-year-old man with no history of diabetes. Simultaneous neuroarthropathy of two joints and its occurrence in uncommon parts of the body like the wrist is quite rare and might lead to misdiagnosis and incorrect treatment.

Abstract
Background & Objective: Bleeding is a common complication during heart surgery, and management of heparin and protamine consumption during operation affects hemeostasis. The dosage of protamine sulfate is variable based on the amount of heparin used and studies have reported different results. The aim of this study was to evaluate the effect of protamine sulfate administration after cardiac surgery on the drainage of the patient's blood secretions.
Methods: In this retrospective cross- sectional study, required data from 120 medical records patients, from 2011 to 2020 undergoing heart surgery, for example CABG, was extracted and examined. Patients were grouped (60 people in each group) and compared based on protamine sulfate intake and not receiving it, in the intensive care unit and body mass index, age, sex, ACT values and blood drainage rate.
Results: 76 (63.37%) patients were male and 44 (36.66%) patients were female. Most patients were in the age range of 50-70 years and most patients were overweight. Blood drainage in the protamine group was significantly lower than the control group. There was no statistically significant difference between the two groups in terms of ACT values. Blood drainage in the protamine group was significantly lower in normal BMI and overweight individuals than in the control group. The amount of blood drained in the protamine group in women and in people over 70 years was significantly lower than the control group.
Conclusion: despite previous studies, our study showed a reduction in the amount of postoperative bleeding following re-prescribing of protamine sulfate in the intensive care unit, which eliminated the effect of heparin and the coagulation disorder caused by heparin. The limitations of the present study included patients under 12 years of age, patients weighing more than 120 kg, and patients requiring hypothermia treatment less than 28 Cº.
