Abstract
Background and Objectives: Jaundice is a common disorder in preterm and term newborns and G6PD deficiency is one of its etiologic agents. With respect to high incidence of G6PD deficiency in Iran, this study was conducted to determine the clinical correlation of this deficiency with time of jaundice onset and mean level of serum bilirubin. Management and complication rates are also evaluated in these neonates in comparison with those without this deficiency. Methods: This is a case-control study performed through G6PD assay in icteric newborns admitted to neonatal intensive care unit in 22 Bahman hospital in Mashhad within 3 years. This case group was compared with the control group who were randomly selected from among icteric neonates without G6PD deficiency. The data were collected by a questionnaire and analyzed by SPSS. Results: This study was done on 505 icteric neonates. The case group consisted of 34 newborns (6.7%) with G6PD deficiency. M/F ratio in case group was higher than control group (P= 0.01). Mean level of total serum bilirubin was significantly higher in case group (P=0.047). There were no significant differences in duration of phototherapy and hospitalization between two groups, but exchange transfusion rate in case group was twice as much. Complications such as seizure and kernicterus in both groups were the same. Conclusion: Due to high incidence of G6PD deficiency in our study (6.7%) and significant incidence in female patients, determination of enzyme level is recommended in all newborns with jaundice regardless of their gender. Because of higher level of serum bilirubin in G6PD deficient patients, phototherapy must be done as soon as possible to avoid invasive procedures such as exchange transfusion.

Abstract
Background & Objectives:Urinary tract infection (UTI) ranges from asymptomatic bacteriuria to severe renal infection with sepsis. Urinary tract infection can cause many complications as abscess, hypertension, renal failure, renal scar, reflux and so on. Since there is no general agreement on the administration of additional fluid (1.5 times maintenance) on the early treatment of UTI on this study was conducted to see the effect of additional fluid. Methods:206 children with UTI who were admitted in Amir Kabir Hospital of Arak from March 2005 to March 2006, were divided into two groups of 103. One group received the usual amount of maintenance fluid and the other 1.5 times of maintenance. The course of resolution of dysuria, frequency, malodorous urine, abdominal pain and fever were compared in the two groups. Patients' urine culture was performed on the second day, 7-10 days, and 90 days after admission. Patients were matched according to their age and sex. Those who presented with azotemia were excluded from the study. Results: Receiving additional fluid had a significant effect on the malodorous urine dysuria. However, it had an inverse effect on the treatment of fever and urinary frequency and it made them even last longer. In other areas like abdominal pain and urine culture in 3 stages there was no difference. Conclusion: It seems that intake of excessive amounts of water has no significant effect on the results of the UTI treatment.

Abstract
Background and objectives: Common cold is the most common disease in children. Young children have an average of 6 to 7 colds each year. Some complications of common cold include: secondary bacterial infections, school absence and excessive cost for treatment. The management of the common cold consists primarily of symptomatic treatment. An effective and safe drug is needed to reduce the duration of common cold ۥ s symptoms in children and reduce subsequent complications. In some studies zinc sulfate has showed an antiviral effect and immune enhancing activity. Therefore this study designed to evaluate the efficacy of zinc sulfate in duration of cold ۥ s symptoms in children. Methods: This was a clinical trial study. The subjects were children between 1 - 7 years old with common cold that attended to pediatrics clinic of Arak Amir Kabir Hospital. The study included 112 patients randomized in two groups. One group received zinc sulfate within 10 days and other group didn’t receive this drug. Using a researcher made questionnaire duration of symptoms: rhinorhea and nasal obstruction, cough, sneezing, fever and duration of illness, in both groups were evaluated. Results: Consumption of zinc sulfate significantly reduced the mean duration of rhinorhea and nasal obstruction in children with common cold (p < 0.05). Also the mean duration of cough in zinc group was lower than the control group (p < 0.05). Compared to the group that didn’t receive zinc sulfate, the zinc group had shorter mean duration of sneezing (p < 0.05). Also the mean duration of fever in zinc group was shorter than the other group (p < 0.05). The mean duration of illness in patients who had received the zinc sulfate was significantly less than the other group (p <0.05). No side effect was observed in subjects received zinc sulfate. Conclusion: According to the results zinc sulfate can be administered with other supportive treatments of common cold, for reduction of symptoms duration and complications in children.

Abstract
Background & Objectives: Previous studies demonstrated that selected probiotic bacteria elicit beneficial effects in animals. Probiotic bacteria inhibit pathogens growth in the gut, improve lipid metabolism and activate immune system of animals. In the present study Enterococcus spp were isolated from Iranian traditional cheese and their effects on intestine pathogens ( Shigella dysenteriae, Escherichia coli and Salmonella Typhimurium) growth, serum lipids level and activation of immune systems in mice were studied. Methods: Iranian cheese samples were collected from Ardabil province. Enterococci spp were isolated using selective culture mediums and identified using API kites. Inhibitory effects of isolated Enterococci on growth of Pseudomonas aeruginosa and intestine pathogens (Shigella dysenteriae, Escherichia coli and Salmonella typhimurium) were tested using agar well method . In order to study probiotic activities of isolated bacteria in live animals, NMRI mice were divided into different groups and Enterococci was administrated orally (1 ML/mouse) with doses equal to 2 (6×108 cfu/ml) 3 (9×108 cfu/ml) and 4 (12×108 cfu/ml) MacFarland standard for 2 weeks. After two weeks continues treatment, blood samples were collected from retroorbital sinus and serum levels of cholesterol, triglycerides, LDL and HDL measured using enzymatic method. Interleukins (IL-2, IL-6 and IL-10) levels were measured using ELISA kites. Results: Results of this study demonstrated that treatment with faecium species decreases serum cholesterol and increases serum IL-10 level, while it has not showed significant effects on serum levels of glucose, triglycerides, IL-2 and IL-6 (p<0.05). Administration of faecalis species have no significant effects on lipid levels of serum ( p <0.05). Moreover, results revealed that treatment with faecalis species increased IL-6 and IL-10 ( p <0.05). None of the species affected pathogens growth significantly ( p <0.05). Conclusion: The results obtained from current study demonstrate that continues treatment with both species can affect immune functions of animal by altering the cytokines profile and treatment with faecium species decreases serum level of cholesterol.

Abstract
Background &Objectives: Antiemetic drugs can be used in complicated cases of vomiting secondary to gastroenteritis. There is no previous study dealing with the use of pyridoxine in treatment of vomiting in pediatrics. This Studyintendsto review the effects of intravenous pyridoxine on control of gastroenteritis related vomiting. Methods: This study is a single blind randomized clinical trial with placebo. A total number of 147 children diagnosed with gastroenteritis induced vomiting and admitted in Amirkabir hospital from August to December 2011 were studied and divided to two case andcontrol groups. In the control group intravenous fluid with distilled water (placebo) was used and the pyridoxine was given along with intravenous fluid therapy in case group. Since admission for three days, numbers of vomiting and other symptoms were collected in data form. Results were analyzed using SPSS-16 by means of descriptive analysis for basic information. Results: Most children were male, aged between 6 months and 2 years and having urban life. There was no significant difference between two groups in the mean of vomiting frequency during the period of hospitalization ( p>0.05 ). T here was also no significant difference between two groups in duration of hospitalization ( p=0.19). Conclusion: Based on this study pyridoxine (Vitamin B6) was not effective in treatment of vomiting due to gastroenteritis in children, and did not reduce the duration of hospitalization.

Abstract
Background & objectives: The purpose of this study was to show the dose response relationship of anti-inflammatory effect of methylsulfonylmethane (MSM) on carrageenan induced rat paw edema as an acute model of inflammation.
Methods: A total of 54 male, Sprague-Dawley rats, weighing 180-190 g, were used. One hundred, 200, 400, 800 and 1200 mg/kg of MSM were administered intraperitoneally to the rats 30 minutes before induction of paw edema with injection of 0.1% carrageenan. Diclofenac was used as a control drug. Rats were divided into three groups: MSM, diclofenac and normal saline, and their paw tissue were collected for the study of inflammatory and oxidative markers (MDA, GSH, TNF-α and IL-1 β). The relationship between the different concentrations of MSM and decrease in rat paw edema was calculated using a simple Emax model.
Results: the ED50 value for effect of MSM on carrageenan induced rat paw edema was 193±9.7 mg/kg. A significant reduction in paw edema following administration of MSM at 200, 400, 800 and 1200 mg/kg was observed, but statistical analysis did not reveal any significant reduction in paw edema after administration of 100 mg/kg. MSM did not show statistically significant difference from control group in tissue level of GSH, but it was able to decrease MDA level significantly. MSM was able to significantly alleviate IL-1 β and TNF-alpha tissue levels.
Conclusion: The recommended anti-inflammatory dosing range of MSM is 200-800 mg/kg for pharmacological studies in rats and the average appropriate dose is 400 mg/kg. Also, it seems that anti-inflammatory effect of MSM is more profound than its anti-oxidant effects.

Abstract
Background & objectives: Lung involvement is crucial in patients with Covid-19. The CT scan plays a key role in diagnosing of this disease. This study aimed to survey CT scan involvement patterns in patients with suspected Covid-19 symptoms.
Methods: The present study was cross-sectional,analytical in which the statistical population was patients with typical clinical symptoms of Covid-19 who were referred to the hospital imaging center from March 2019 to May 2019. A total of 301 patients were randomly selected as a sample. Data analysis was performed using SPSS version 26 at a significance level of 0.05.
Results: The average age of the participants in the study was 54.6±17.6. 151 patients (50.2%) were male. 255 patients (84.7%) were treated on an outpatient basis, 37 patients (12.3%) were treated in the Non ICU ward and nine patients (3%) were treated in the intensive care unit(ICU). One hundred sixty eight patients (55.8%) with grand glass conflict, 25 patients (8.3%) with consolidation conflict, 49 patients (16.3%) with linear turbidity were identified while 44 patients (14.6%) had crazy paving, 7 patients (2.3%) had small nodules and 45 patients (15%) had round opacities on contrast-free CT scan. The pattern of conflict in the form of linear turbidity was directly and significantly related to the severity of the disease.
Conclusion: Imaging findings in patients with Covid-19 have a wide range. Despite these findings, attention should be paid to less common and rare signs and symptoms that can help make the right decision and more definitive diagnosis of the disease.

Abstract
Background & objectives: Menopause and especially acute menopause due to surgery is associated with many complications in women. The aim of this study was to determine the effects of genistein and regular swimming exercise (alone/or in combination) on pain through a possible mechanism of inflammation and oxidative stress in ovariectomized rats.
Methods: In this study, rats were divided into six groups, including: control, sham, ovariectomy (OVX), ovariectomized with eight weeks of swimming exercise training (OVX.E), ovariectomized with eight weeks of genistein administration (OVX.G), and ovariectomized with eight weeks of combined treatment (OVX.G.E). The effects of genistein and/or exercise were evaluated by examining the pain intensity with tail-flick and formalin tests. The serum levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), total antioxidant capacity (TAC), total oxidant status (TOS), and malondialdehyde (MDA) were also evaluated by ELISA and spectrophotometer.
Results: In the OVX group compared to the control group, tail-flick and formalin tests showed an increase in pain response. Also, a significant increase in the serum levels of IL-1β, TNF-α, MDA, TOS and a decrease in TAC was observed in the OVX group, however, in the OVX.E, OVX.G and especially OVX.E.G groups, pro-inflammatory cytokines and oxidative stress as well as pain responses showed a significant decrease compared to the OVX group.
Conclusion: A combination of genistein and regular swimming exercise was synergistically more effective in reducing acute and chronic pain than using them alone in the postmenopausal period.

Abstract
A severe acute respiratory syndrome caused by coronavirus 2 infection (COVID-19)was identified in December 2019 and caused a massive pandemic that claimed millions of lives worldwide. The Chimpanzee adenovirus vector vaccine is one of the few vaccines against coronavirus disease in 2020. This case report introduces, a patient who developed thrombocytopenia, fever, and lethargy nine days after vaccination with the chimpanzee adenovirus carrier vaccine. The patient was admitted to the hospital and thrombotic thrombocytopenic purpura (TTP) symptoms appeared during hospitalization.

Abstract
Background: One of the major challenges in surgery is pain control after surgery. The present study was conducted with the aim of investigating the role of magnesium sulfate in improving the quality of anesthesia in patients who are candidates for appendectomy surgery.
Methods: In this clinical trial study, 42 people in the control group and 42 people in the intervention group were randomly included. For the intervention group, 50 mg/kg of magnesium sulfate was injected and for the control group, the same amount of distilled water was injected as a placebo. After surgery, the patients were examined for pain, nausea and vomiting, anxiety, blood pressure and heart rate, and the satisfaction of the surgeon in terms of muscle relaxation. Collected data were analyzed in SPSS-26 by statistical methods and p<0.05 was considered as significant level.
Results: The average pain of the patients in the first, third, sixth and twelfth hours of the study in the intervention group with 1.4, 2.66, 3.3 and 3.19 were significantly lower than the control group. In terms of relaxation, a significant difference was observed between the two groups in the first, third, sixth, twelfth and eighteenth hours of the study. In the examination of nausea and vomiting of patients, no significant difference was observed between the two groups. During surgery, a significant difference was observed in patients receiving magnesium sulfate compared to the control group in terms of surgeon satisfaction.
Conclusion: Magnesium sulfate was effective in controlling patients' pain and anxiety and surgeon's satisfaction during surgery, but it was not effective in controlling patients' nausea and vomiting.
