Abstract

 Background and Objectives: Pre-eclampsia is one of the common causes of maternal mortality. A main problem with induction of general anesthesia in these patients is exagergted increase of blood presscure during laryngoscopy and intubation that is associateed with a significantly increased intracranial pressure with the risk of cerebral hemorrhage. There are some strategies to minimize extreme increase in blood pressure during laryngoscopy and intubation. The aim of this study was to evaluate the effect of Fentanyl-Droperidol combination (Talamonal) on reducing cardiovascular responses due to laryngoscopy and intubation in preeclamptic parturient.

 Methods: Thirty preeclamptic parturients who were admitted for emergency caesarean section under general anesthesia were studied. All patients had diastolic pressures sustained at ≥ 100 mmHg, and had received antihypertensive therapy. A standard general anesthesia was induced in all patients. Fentanyl-Droperidol combination was used only in the case group five minutes before induction of anesthesia.

 Results: Arterial pressure and heart rate increases, improved in approximately 86.6 % of the case group. The mean increase in systolic pressure, and heart rate following laryngoscopy and intubation was significantly different in two groups (P=0.0001). There was no significant difference in the mean Apgar score of the neonates at 1 and 5 minutes after delivery in two groups.

 Conclusion: Using Fentanyl-Droperidol Combination during induction of anesthesia produced a clinically significant decrease in sympathetic response to laryngoscopy and intubation in most mothers, without any adverse effect on the neonates.

Abstract

  Background & Objectives: It is estimated that as many as 75% of women experience at least one episode of vulvovaginal candidiasis during the lifetime and about 5% will have recurrent infections. There are clear association between vaginal candidiasis and uncontrolled diabetes mellitus. The purpose of this study was to determine whether non-diabetic women with recurrent vaginal candidiasis have an impaired glucose metabolism.

  Methods: This case-control study was carried out with 32 patients suffering from recurrent vulvovaginal candidiasis as case group and 30 control patients who had negative history of vaginal candidiasis during the previous year and had referred to the clinics for reasons other than vaginal candidiasis. The case group had referrd to the gynecology clinics three times during the previous year and were recognized to have vaginal candidiasis. They were being treated by antifungal drugs and had at least one positive candida culture. Both groups underwent standardized oral glucose tolerance test with 75-gram dose of glucose. The data were analysed using Chi-square and t-test.

  Results: Women with recurrent vaginal candidiasis had a mean body mass index greater than the control subjects (23.4 versus 21.5, p=0.001). They had no more incidences of overt or preclinical diabetes mellitus than the control subjects, but a greater proportion of them had at least one glucose concentration above the 95th percentile (p=0.015). Glucose concentrations were significantly higher in recurrent vaginal candidiasis cases than in control subjecs at 30, 60 and 75 minutes after the intake of 75-gram dose of glucose (p<0.05). This amount of glucose led to a 15.8% increase in serum glucose level in case group compared to the control group.

  Conclusion: Glucose tolerance test was mildy impaired in women with recurrent vaginal candidiasis. The plasma glucose concentration increased in 2 hours after 75 g glucose intake. Although the obtained results were not in the range of occult diabetes, it can designate the important of glucose tolerance test in women with recurrent vaginal candidiasis.

Abstract

  Background & Objectives: Myocardial infarction is one of the most common causes of mortality throughout the world. Utilization of thrombolytic drugs at the first hours upon MI leads to decrease in the rate of mortality. Streptokinase (SK) is commonly used because of its cost and availability. The golden time for maximum effect of SK appeared to be first hours after MI. The drug efficiency diminishes after 3 hours and it will be ineffective after 12 hours.

  This study was aimed to investigate interval between onset of the clinical symptoms and streptokinase receiving in patients with acute myocardial infarctionadmitted for drug reception.

  Methods: This cross-sectional study was conducted in 150 patients with STEMI (ST elevation Myocardial Infarction ) who were attended to emergency room of Imam Khomeini hospital and fulfilled a questionnaire containing the data about age, sex, educational level, onset of symptoms, referring time to emergency room, SK utilization, past medical history of cardiac diseases, type and location of AMI, contraindications and side effects of SK.

  Results: In 111 men (74%) and 39 women (26%), the mean for interval between onset of symptoms and drug injection was 8 hour and 27 minutes ± 8 hour and 18 minutes .Only 41% of patients received streptokinase in less than 3 hours after chest pain. The main reasons for delaying in drug consumption were delayed decision to come to hospital in 61% of persons, long distance to hospital for 22% of individuals, delayed admission tor CCU for 13% of cases and delay in physician diagnosis for 0.043% of patients. To have a male sex and residency in city where the hospital was establilished were associated with less-delayed treatment start. However, the educational level, age and history of MI were not correlated with onset of treatment.

  Conclusions: Our results clearly show that the majority of patients receipt streptokinase more than 3 hours after chest pain. With considering the factors associated with delay to receive SK, teaching the patients with ischemic heart disease and giving information about golden time for thrombolytic therapy and its effect in prognosis as well as extending cardiac disease treatment centers in suburbs are recommended.

Abstract

  Background: Multiple sclerosis (MS) is an inflammatory disease of the central nervous system with multifocal areas of demyelination. Genetic and environmental factors could be involved in etiology of disease. The aim of this study was to evaluate important risk factors for MS progression in patients admitted in Alavi hospital, Ardabil.

  Methods : The present research is an analytical, case-control study in groups with or without MS with same age and sex (80 subjects in each group). A self-made questionary including demographic characteristics and risk factors used as a data compilation instrument which validity reliability were confirmed by content and α-Coronbach tests respectively. Data were analyzed by SPSS software using descriptive and inferential statistics.

  Results: Factors such bachelorship (p=0.037), accidental head and back traumas (p=0.003), smoking (p=0.035), smoke exposures (p= 0.0001), meat regimen (p=0.0001), contact with birds (p=0.032), inadequate sunlight exposure (p=0.034), occupational pollution (p=0.002), menarche age (p=0.016), family history (p=0.029), being third born in family (p=0.034), were found as significant risk factors for multiple sclerosis.

  Conclusion : The present study demonstrates that factors such as smoke, trauma, environmental pollution, touching birds and family history of MS are important risk factors for onset of multiple sclerosis.

Abstract

  Background & Objectives: The development of technology has naturally given rise to an increase in environmental low-frequency electromagnetic fields and consequently has attracted scholars' attention. Most of the studies have focused on transmission lines and power system distribution with 50 Hz. This research is an attempt to show the effect of 50 Hz magnetic fields on bioelectric parameters and indicates the possible influence of this change in F1 cells of Helix aspersa .

  Methods: The present research used Helix aspersa neuron F1 to identify the location of magnetic fields as well as the rate of effects of environmental magnetic fields on nervous system. Control group was used to study the effect of elapsed time, electrode entering and the cell membrane rupture. Intuition group and environmental group were considered in order to study the potential impact of interfering environmental factors and identify the effectiveness rate of magnetic fields, respectively. For the purpose of producing uniform magnetic field Helmholtz coil was used. Electrophysiological recording was realized under the requirements of current clamp. And, in order to show the impacts from magnetic fields on ion channels Hodgkin-Huxley cell model was applied. All data were analyzed taking the advantage of SPSS 16 software and two-way ANOVA statistical test. P < 0.05 was considered as significance level. And MATLAB software environment and PSO were used in order for applying the algorithm and estimating the parameters.

  Result: No statistically significant difference was found between control and sham groups in different time intervals. Once the 45.87 microtesla was applied significant differences were observed 12 minutes after the application. The highest amount of change happened 14 minutes after the application of more fields. With the application of the field, the amplitude of the sodium action potential shows decreasing trend . No significant changes were observed in different time intervals, whereas significant differences were seen in frequency of action potential during different time intervals. The amplitude of AHP shows no significant changes .

  Conclusion: The results indicated that low-frequency magnetic fields with 50 Hz frequency will directly lead to change in bioelectric activities of neurons through a change in amount and rate of opening and closing of ionic channels and the conductivity of sodium and potassium channels reduces together with increase in conductance of potassium dependent calcium channel (AHP).

Abstract

  Background & Objectives : Nowadays, coronary artery disease is the most common cause of death in developed countries and in the whole world. According to the WHO reports, it will be the main cause of death in 2020. Myocardial infarction is the most common diagnosis among hospitalized patients in industrialized countries. This disease causes more mortality and morbidity than others. Coronary artery bypass grafting surgery (CABGS) is one of the common treatments for ischemic heart disease but it may have some complications. In this study we wanted to evaluate the incidence of CABGS complications in Imam Khomeini Hospital of Ardabil in the first month after CABG.

  Methods: This study was a cross sectional analytic descriptive type and performed on all of the patients underwent CABGS in Imam Khomeini Hospital during 2011-2012. All of the data from patients (like demographic, past medical history, physical exam findings, paraclinical findings and CABGS complications) were inserted in special forms after gathering, and analyzed by SPSS v.16.

  Results: In this study, 211 patients were studied. About 145 (68.72%) of them were male and 66 (31.28%) were female. Seventy patient (33.17%) were more than 70 years old. About 33.22% of patients had a history of cigarette smoking and 9.95% were opioid abused. The 34.12% of patients had hypertension (HTN) history, 40.28% diabetes mellitus (DM), 17.06% history of hyperlipidemia and 63.98% had a history of the previous MI. Chest pain was the most common complication among the patients (93.36%). According to our study the prevalence rate of post CABGS complications were: bleeding after surgery 13.27%, postoperative myocardial infection 8.05%, neurological disorders 12.32%, renal complications 2.36%, respiratory symptoms 11.37% and 34.59% for cardiovascular complications. Also the total mortality was 5 (2.36%) persons. The data analysis showed that there was a significant relationship between sex with survival status, local infection and neurological disorders, and also there was a significant relationship between age with post CABGS survival, bleeding after surgery and renal complications. There was also a significant relationship between DM and post CABGS survival, local infections and respiratory complications and between HTN and local infections.

  Conclusion: According to the results of this study, the most common post CABGS complications were cardiovascular complications, especially arrhythmias.

Abstract

  Background & Objectives : Visceral leishmaniasis is an important endemic disease in Iran and Meshkin shahr County is the most important foci of visceral leishmaniasis in Iran. The aim of this study is the survey of recent changes in epidemiological patterns of visceral leishmaniasis in Meshkin Shar area during 12 years.

  Methods : This is a descriptive retrospective study. Records of 193 cases with confirmed kala-azar disease were reviewed in the period of 2001-2012. Data collected from files and were analyzed using descriptive statistics and SPSS v.15.

  Results : From 193 patients with confirmed kala-azar disease, 85% were resident in rural and 15% in urban areas. Among of these patients, 43% were female and 57% male. About 66% of patients were under 2 years old, 23% in age group between 2-5 years and 11% above five years. Among four parts of Meshkin Shahr County, in the first five years of the study, the most cases of the disease were from the central part, but in second six years the most cases were from the west part of this county and in latest year (2012) it has been coverted .

  Conclusions : Incidence of kala-azar was decreased during last 12 years. Because of high infection rate in dogs and considering important role of these reservoirs in conserving and distribution of disease, stopped health activities in any parts of area can increase the incidence of disease.

Abstract

  Background & objectives: Nitrate is one of the major contaminants in food and water. Excess intake of this substance can increase the risk of stomach cancer and also cause other health problems. The objectives of this study were estimation of dietary nitrate consumption in the Varzaghan with high and Parsabad with low stomach cancer incidence in country, and also the comparison dietary nitrate consumption with World Health Organization standards.

  Methods: In this comparative study performed during autumn and spring of 2011 about 216 food samples (including all food groups) and drinking water collected and their nitrate concentration was determined by colorimetric method. Then daily dietary consumption of nitrate calculated based on daily diet of each person (according to national study on food consumption pattern in Iran) and nitrate concentration in each group of food and drinking water.

  Results: Daily consumption of nitrate in Varzaghan and Parsabad was 8.53 ± 0.35 and 8.17 ± 0.54 mg per kg of body weight (of adults), respectively, which is much greater than the amount recommended by FAO/WHO (0-3.7 mg per kg of body weight). Significant difference was not observed in the dietary consumption of nitrate in two cities at studied period (P> 0.05).

  Conclusion: Despite of no significant difference in dietary consumption of nitrate in two cities, the daily consumption of nitrate in both of studied cities was significantly greater than the recommended level. Therefore monitoring sources of contamination and reduction of environmental pollution for decreasing food nitrate concentration are recommended.

Abstract

  Background & Objectives : Social phobia is one of the common anxiety disorders characterized by fear of social situations, shame and embarrassment in communicating with unfamiliar people. The aim of this study was to examine the effectiveness of acceptance and commitment therapy in reducing anxiety and depression in students with social phobia .

  Methods: An experimental pretest-posttest with control group design was used to conduct the study. The population consists of all female students in Ardabil city, 2013-2014 academic years. Using a multiple-stage cluster sampling method, a representative sample consists of 400 students was selected for the study. Then, among the students those with social phobia (based on the cut-off point), 34 participants were randomly selected and divided into two groups of 17 people (one experimental group and one control group) . The participants in experimental group received acceptance and commitment therapy for 10 sessions of 90 minutes. The control group did not receive any intervention. Anxiety, depression and social phobia questionnaires were administered to the two groups before and after the intervention. Descriptive statistics and multivariate ANOVA (MANOVA) were used for data analysis.

  Results: The results of data analysis showed that treatment based on acceptance and commitment therapy reduces anxiety, depression and social phobia of students in the experimental group compared with the control group (p<0.001) .

  Conclusion: The present study showed that acceptance and commitment therapy for students with social phobia can be used as a psychological intervention along with other intervention mechanisms.

Abstract

Background & objectives: Iron overload is involved in the pathophysiology of many diseases including diabetes. In fact, the excess iron by creating free radicals makes damage to pancreas and leads to insulin resistance and diabetes. Andrographolide extract has hypoglycemic and antioxidant properties. This study has surveyed the effects of andrographolide on blood glucose and lipid profile in rats with secondary iron overload.

Methods: In this experimental study, 36 male Wistar rats were randomly divided into 6 groups: the healthy control group, secondary iron overload group, secondary iron overload groups treated with a dose of 3.5 and 7 mg/kg of andrographolide extract, and andrographolide groups treated with a dose of 3.5 and 7 mg/kg of extract. Iron and extract were injected for 6 and 12 days, respectively. Blood samples were taken for measurement of blood glucose and lipid profiles. Data were analyzed using ANOVA test.

Results: The pathological results of samples from liver of animals receiving iron showed that the iron was deposited in the liver tissues. Iron injection significantly increased blood glucose levels compared to healthy control group (p<0.05). In the iron overload group, andrographolide extract with a dose of 3.5 mg/kg or 7 mg/kg significantly decreased blood glucose levels (p<0.05). Iron injections did not increase the serum triglyceride and cholesterollevels. Injections of andrographolide extract with a dose of 3.5 mg/kg and 7 mg/kg, significantly decreased the cholesterol levels compared to iron receiving group (p<0.05).

Conclusion: Results of this study showed that the andrographolide with different doses may be effective in the treatment of diabetes by reducing serum glucose and cholesterol levels.

Abstract

Background & objectives: The role of spirituality with its different levels in different countries and societies should be considered for investigating the elderly's health. Therefore, it is necessary to evaluate spirituality with a scale specifically designed to measure spirituality. This study investigated the psychometric properties of the Persian version of the Attitudes Related to Spirituality Scale (ARES) in the elderly.
Methods: In this psychometrics study, the study population included all the elderly of Ardabil city, among whom 459 individuals were randomly selected to fill ARES. The data was analyzed by SPSS and Amos Software.
Results: The reliability of ARES was 0.94 by Cronbach's alpha. Its construct validity, calculated by confirmatory factor analysis, showed that the model for measuring attitudes related to spirituality in the elderly had a suitable fit and its one-factor structure was confirmed (RMSEA=0.001, GFI=0.99, CFI=1).
Conclusion: The Persian version of the Attitudes Related to Spirituality has a one-factor structure, and could be used in psychological studies in Iranian samples due to its acceptable necessary validity and reliability.
