Abstract
Background & Objectives: Benign prostatic hyperplasia is one of the most common diseases in males and Transurethral Resection of Prostate (TUR-P) is a standard method for its surgery. Distilled water is used to clean the field of operation from blood and debris. The overabsorption of this washing fluid can lead to hemodynamic disorders and neurological complications called TUR-P syndrome. Because of hemolysis and ATN, most of the urologists, nowadays, prefer to use non-hemolytic solutions such as cytol and glycine 1.5%. Distilled water is widely used in Iran. Replacing this water with a nonhemolytic and hypoosmolar solution with reasonable price such as manitol 3% seems reasonable. This study was carried out to campare manitol 3% with distilled water during TUR-P. Methods: In a double-blind randomized clinical trial, we studied 78 patients with BPH who were admitted to Shahid Moddares hospital in Tehran for TUR-P. These patients were allocated to disstiled water group and manitol 3% group. Na, K, Cr, BUN, CBC, U/A, U/C, 24 hr urine volume and creatinine were checked preoperatively to determine GFR, Na, K, BUN and CBC were checked right after, 4 hours after and on the morning after the operation. Clinical signs of TUR-P syndrome were recorded in the patients as bradycardia, hypertension and neurologic sign. Results : Mean age was 68.6 in distilled water group and 66.4 in the manitol group. Mean weight of resected tissues, mean volume of the solution used and mean resection time were 19.8 gr, 19 litre, 50.8 minutes respectively in distilled water group and 20.2 gr, 20.3 litre and 51 minutes in the manitol 3% group. These differences were not statistically significant. The difference between decrease in serum Na and serum osmolality was not significant in two groups, however, hemolysis rate in two groups was statistically significant (p<0.01). The incidence rate of TUR-P syndrome was 34% in the distilled water group and 18% in manitol group. This difference was not statistically significant. Postoperative creatinine increase was 0.625 mg/dl in distilled water group and 0.04 mg/dl in manitol group. This was not statistically significant. Conclusion: Regarding the role of distilled water in hemolysis and ATN and due to the unavailability of non – hemolytic solutions such as glycine and cytol in Iran, the use of manitol 3% soluion which is a cost-effective, non hemolytic and hypoosmolar solution is recommended.

Abstract
Background & Objectives: Urinary tract infections (UTI) are among the most common bacterial infections in humans. One of the most frequent form of UTI is asymptomatic bacteriuria. The aim of this study is to determine the prevalence of asymptomatic bacteriuria in female students of Babol Islamic Azad University. Methods: This research was carried out on 207 female students from Babol Islamic Azad University in 2002. The age ranges of the students were between 18-29 years. Mid-stream urine (MSU) was collected from students. Urinalysis and bacteriological identification culture of MSU were done twice on all specimens. The prevalence of asymptomatic bacteriuria was determined by dividing the total number of observed bacteriuria by the total number of samples. Results: Bacteriological tests of MSU revealed that 19 (9.2%) cases were positive for asymptomatic bacteriuria. The microbe most frequently causing asymptomatic bacteriuria was staphylococcus saprophyticus in 7 (36.8%) cases Escherichia coli in 6 (31.6%) cases and Klebsiella in 3 (15.8%) cases. Conclusion: Asymptomatic bacteriuria was highly prevalent in our study. Therefore, young women must consider general health rules about genitourinary system to avoid asymptomatic bacteriuria.

Abstract
Background & Objective: Tuberculosis is more prevalent in developing countries and death from tuberculosis meningitis is strongly associated with delays in diagnosis and treatment. Polymerase chain reaction (PCR) has been incorporated as a diagnostic tool for the diagnosis of tuberculosis. The rapid results and greater sensitivity compared to traditional microbiological methods makes PCR a suitable technique in tuberculosis, especially in tuberculosis meningitis, when diagnosis is difficult or when rapid diagnosis is needed. However, the possibility of false positive and false negative results must be considered. The aim of this study was to compare the conventional bacteriology (culture Ziehl- Neelsen staining) with polymerase chain reaction (PCR) technique for rapid diagnosis of tuberculosis meningitis. Methods: This study included 25 clinically diagnosed patients that were suspected to have tuberculosis meningitis and 10 other bacterial or viral meningitis patients were investigated. DNA was extracted from CSF and the NESTED PCR using specific primers were done. Results: In 25 samples, Mycobacterium tuberculosis DNA was detected in 9 (36%) by PCR, 2(8%) and 1(4%) with culture and direct smear was obtained, respectively. whereas no DNA bands were detected in patient with the other 10 meningitis. The entire procedure was repeated and the same result was obtained. Conclusion: The findings of this study indicated that PCR is a powerful method for rapid and sensitive diagnosis of tuberculosis meningitis. In a way that it decreases obtaining the results from several weeks in bacteriological methods to one to two days, especially in smear negative patients. This is very important in tuberculosis meningitis because it is a medical urgency and needs rapid diagnosis and early treatment.

Abstract
Background & Objectives: E. coli is an opportunistic and pathogenic bacterium in human. It is a fecal contamination indicator of the water. The aim of this study is to investigate the efficiency of ultra violet radiation in disinfection of E. coli in aquatic environments in a batch system. Methods: At first, reactor was designed and made. The effects of pH, exposure time and initial inoculum were studied. Kinetic parameters were obtained by application of zero, first and second - order equations . Results: The result showed that removal efficiency decreased with increase of initial inoculumand the kinetics of disinfection was described by first-order model. The result also showed that removal efficiency augmented with increasing exposure time and pH . Conclusion: The results of this study shows that ultra violet radiation can be used as an effective disinfection method for E. coli in aquatic environments.

Abstract
Background & Objectives: Urinary stone disease is one of the common urinary tract complications that can lead to renal failure. It occurs usually in middle-aged and older men The aim of this study was to determine the association between types of urinary stones and risk factors for urinary stone formation in patients referred to Ardabil therapeutic centers. Methods : In this descriptive cross-sectional study, 150 patients with urinary stone were followed from March 2008 to July 2009 in therapeutic centers of Ardabil. Data were collected by a questionnaire include demographic factors such as age, gender, body mass index (BMI), Job and Habitation place. Data were analyzed by chi-square test and ANOVA in SPSS 16 software. The level of significance was p<0.05. Results: 116 of patients (77.3%) had calcium oxalate stone, 15 of patients (10%) had phosphate calcium stone, 10 of patients (6.7%) had acid uric and 9 of patients (6%) had phosphate ammonium stone. The mean age of patients was 41.90 + 14.41 years (48% of patients were 30-50 years old) and the mean body mass index (BMI) was 25.81+3.66. 91 of patients (60.66%) were male and 59 (39.4%) of them were female. There was no significant difference between types of urinary stones and factors including BMI the job of patient, (p=0.252), habitation place (p=0.173), history of previous illness (p=0.207) and history of drug use (p=0.247). Conclusion: The most common type of urinary stones was calcium oxalate (77.3%) in both men and women however ammonium phosphate (6%) is much less common.

Abstract
Background & Objectives : Urolithiasis is the third common urinary disease. While, relationship between the urinary stones and diet is known to some extent, but there are controversies about it. The aim of this study was to determine the association between food habit, nutrients intake and kinds of urinary stone disease. Methods: This cross-sectional and descriptive analytical study was performed on 150 patients (91 male and 59 females) with urinary stone disease in Ardabil during 2008-2009. Data of urinary stones analysis, type of stone in the graph, type of stone, preventive emprise and type of drinking water were recorded. Food data were recorded using 24-hour dietary recalls three days in week and food frequency questionnaires. Data were analyzed using SPSS, Food Processor III software, and ANOVA Statistical program . Results: Findings showed that the most common and lowest form of urinary stone disease in men and women were calcium oxalate and ammonium phosphate, respectively. There was significant inverse association between beta-carotene intake and the percentage of uric acid stone disease (p<0.05). We observed significant relationship between vitamin A and folacin intake with percentage of uric acid stone disease (p<0.05). There was significant relationship between consumption of cheese and pickles with calcium phosphate stone disease (p<0.05). There was no significant association between other foods and nutrients intake with types of urinary stone disease . Conclusion: This study showed the consumption of some nutrients including folacin, vitamin A and some foods such as cheese was associated with the incidence of urinary stone disease. Therefore, taking accurate diet could possibly prevent the incidence of urinary stone diseases.

Abstract
Background & Objectives: The purpose of this study was to investigate the effectiveness of rTMS (repetitive transcranial magnetic stimulation) in reduction the signs and symptoms of depression, such as cognitive, bodily and negativity-worthlessness in recurrent major depression. Methods : It was used a quasi-experimental, pretest-posttest design with control group , a sample consisting of 32 patients who had recurrent depression on the basis of DSM-IV,SCID and BDI-II scale & Hamilton scales, were randomly assigned to two groups. The experimental group underwent 20 sessions of rTMS as the independent factor and both groups (control &experimental) had 12 sessions of psychotherapy and drugs treatment. Upon the intervention, both groups were tested with triplex tests, to determine the effect of the independent factor on the dependent factor of rTMS. Data were analyzed using Chi square, T- test, covariance and repeated measures analysis of variance. Results : With comprise between pre &post-test, we understand the effect of rTMSin cognitive, bodily and negativity-worthlessness scalesthe difference of two groups was significant, but the decrease of cognitive scale was higher than other two scales. All the tests showed the reduction of signs and symptoms of recurrent depression in participants . Conclusion: The rTMS is effective in treatment of triple signs andsymptoms (cognitive, bodily and negativity-worthlessness) in patients with recurrent major depression.

Abstract
Background & Objective: Advances in public health and control of infectious diseases have led to increased number of elderly people in the world including Iran. Thus serious concerns exist in terms of age-related diseases . Consequently, Iran’s ministry of health has chosen “ageing and health” as the theme for 2012 along with WHO. Psychological disordersare among the most prevalent diseases in elderly people. Recent studies suggest a two-way relationship between some psychological disorders and insomnia. Also there is evidence implying magnesium role in improvement of aforementioned disorders. The objective of this study was to determine the effects of dietary magnesium supplementation on mental health in insomniac elderly subjects. Methods: A double-blind randomized clinical trial was conducted on 46 insomniac elderly subjects randomly allocated into the magnesium or the placebo group, receiving daily for 8 weeks either 500 mg magnesium or a placebo, respectively. SCL-90-R and ISI questionnaires were conducted at baseline and at the end of the intervention period. Serum magnesium and cortisol levels were also determined in the patients. In addition, information was obtained on anthropometric confounding factors and daily intake of magnesium, calcium, potassium and caffeine using 24-hr dietary recall questionnaire for 3 days. The N4 and SPSS19 were used for data analysis. Results: No significant differences were observed in assessed variables between the two groups at baseline. As compared to the placebo group, in the experimental group, dietary magnesium supplementation brought about statistically significant decreases in GSI, PST, PSDI, ISI scores as well as somatization , anxiety, psychoticism , and depression symptoms and serum cortisol concentration. While the obsessive-compulsive , interpersonal sensitivity, hostility , phobic anxiety, paranoid ideation scores and serum magnesium concentration were not different between the experimental and the placebo groups. Conclusion: In this study dietary magnesium supplementation resulted in improvements in insomnia severity index and some indices of mental health.

Abstract
Background & Objectives: Non-synonymous single nucleotide polymorphism (nsSNPs) which results in disruption of protein function are used as markers in linkage and association of human proteins that might be involved in diseases and cancers . Methods: To study the functional effect of nsSNP in cyclooxygenase-2 (COX2 ) amino acids, the nucleotide sequences encoding COX-2 gene in cancers were extracted from the NCBI (gi|223941909) data bank (283 cases) and analyzed by SIFT, I-Mutant 2.0, SNP and GO, PANTHER and FASTSNP servers. These servers involve programs that predict the effects of amino acid substitution on protein function, stability and missense . Results: COX-2 is an essential enzyme for the production of pro-inflammatory prostaglandins which are relevant to cancer development and progression. The substitutions in some positions such as R228H and S428A of COX-2 in most of cancers linked to reformed protein function through disruption in enzyme active site. Conclusion: Amino acid substitutions as a consequence of COX-2 nsSNPs have important role in human disease. Substitutions which are located in catalytic domain are important for the enzymatic function of COX-2 and associated with higher expression of COX-2.

Abstract
Background & objectives: Type 2 diabetes mellitus results in vascular dysfunction and reduction of nitric oxide (NO) level. In this study the effect of electromagnetic field (EMF) on NO level, an effective factor in increasing blood flow, and on ankle brachial index (ABI) as a n index of lower extremity blood flow in type 2 diabetic patients, have been investigated. Methods: This study was a randomized controlled clinical trial carried out in Imam Khomeini hospital of Ardabil and Mehr physiotherapy center in 2013. Thirty diabetic patients after initial pilot study participated in this experiment based on inclusion and exclusion criteria. Then, they were randomly assigned into two equal groups, electromagnetic therapy and control. The baseline levels of ABI and NO of both groups were measured. Patients in electromagnetic group were exposed to 10 Hz EMF radiation (with intensity of 8 mT) together with scanning lower limbs (three sessions per week, and 40 minutes per session for 8 weeks). The control group did not receive any treatment. After eight weeks, the amount of ABI and NO level were recorded. For statistical analysis paired t-test and independent t-test were used in SPSS 19 software. Results: Statistical analysis showed significant differences between pre (1.009±0.02) and post (1.052±0.02) measurement of ABI of diabetic patient after applying magnetic field ( P = 0.013). However, there was no significant difference between p and after measurement of ABI and NO level in controlgroup and also NO level in magnet groups ( P> 0.05). Difference between pre and post therapy of ABI in electromagnetic group (0.05±0.01) showed significant increase compared with control group (0.004±0.003) ( P = 0.001). Conclusion: Increase in ABI after 8 weeks electromagnetic therapy could be considered as an accretion index in blood flow in lower extremity.

Abstract
Background & objectives: Social capital is defined as norms and networks which provide conditions for participation in social activities in order to profit mutually. This study was designed to evaluate the social capital status of students of Ardabil University of Medical Sciences and its relationship with religious attitudes.
Methods: This was a cross-sectional study of 500 students from Ardabil University of Medical Sciences during second half of the academic year 2014. Data were collected using demographic, social capital and religious attitudes questionnaires and analyzed by SPSS 22 using T-Test, Pearson correlation and ONOVA tests.
Results: Total means score for social capital was 80.0±16. There was significant correlation between all dimensions of social capital (except for dimension of family and friends connections) and religious attitudes status (p<0.05).
Conclusion: Due to the influence of social capital and its relationship with the religious attitude, it can be a guide for reduction of concerns about the educated classes and increasing their social capital

Abstract
Background & Objective: Nowadays, female infertility and abortion is considered one of the most important issues in the medical world. Due to high consumption of chamomile as a medicinal herb, this study aimed to investigate the effects of chamomile consumption on abortion, estrogen, progesterone, FSH, LH hormones and ovarian follicles in adult female rats.
Methods: In this experimental study, 80 adult female rats were divided to 2 categories in 5 groups of 8 pregnant and non-pregnant rats, including control groups, sham group and groups receiving intraperitoneal doses of 30, 60 and 120 mg/kg chamomile hydro-alcoholic extract. At the end of the day 16 of pregnancy, aborted fetuses in pregnant groups were counted, and in day 21, the number of follicles and corpora-lutea in non-pregnant groups was obtained by separating ovaries, and sexual hormone levels were measured after phlebotomizing the samples. The results were analyzed by SPSS software (Ver.18) using ANOVA and Tukey tests. Significant difference of data was set at p≤0.05.
Results: The results of this study showed that chamomile caused a significant increase in the number of aborted fetuses and follicle atresia and a significant decrease (p≤0.05) in serum level of estrogen, progesterone, FSH and LH hormones as well as the number of pre-antral follicle, antral follicles, graph and corpora-lutea.
Conclusion: The results showed chamomile extract decreased LH and FSH, thereby decreasing ovarian follicles, sexual hormones and aborted fetuses.

Abstract
Background & objectives: The purpose of this study was to investigate the effect of endurance swimming exercise training on structural remodeling (volume and parenchymal cell number) and apoptotic index of adrenal gland in pregnant rats exposed to cadmium poisoning.
Methods: A total of 32 pregnant rats weighing 200 ± 20 g were randomly divided into four groups of control, cadmium, swimming, and cadmium-swimming. Cadmium dissolved in drinking water was administered to treatment groups, available ad libitum during pregnancy. Swimming exercises 5 days/week and 60 min/day were performed from the first day of gestation until the end of the period. Two days after delivery, the mothers were sacrificed and their adrenal glands were removed. After stabilizing the samples, Hematoxylin-Eosin staining and TUNEL assay were performed, and the number of necrotic and apoptotic cells in 10 microscopic fields was counted randomly. The size of various regions of the adrenal gland and total number of parenchymal cells were estimated using stereological methods. Data were analyzed by two-way ANOVA under SPSS software (version 21).
Results: Cadmium poisoning caused extensive bleeding and tissue destruction in the adrenal gland of the pregnant mothers, but endurance training reduced the amount of bleeding. Cadmium poisoning during pregnancy decreased the total volume of the gland, the volume of the cortical part and its different layers as well as the number, size and function of parenchymal cells in all three cortical zones, especially the fasciculata zone. Performing swimming exercise training in this condition worsened the structural state of the gland and led to a further reduction in the number of parenchymal cells within all three parts of the adrenal gland.
Conclusion: Exercise training in determined intensity increased the structural and morphological complications of cadmium toxicity in the adrenal gland of pregnant rats. So, pregnant mothers are advised to use low-intensity exercises and trainings.

Abstract
Background & objectives: Diet plays an important role in control of seizure in epileptic patients. Therefore in this research, the effect of acute and chronic sesame oil consumption on the seizure induced by strychnine in adult rats was investigated.
Methods: In this experimental study, forty -two rats were divided into six groups: control (saline recipient, 1 ml/kg (, acute recipients of sodium valproate as positive control group (100 or 200 mg/kg, ip.), acute recipients of sesame oil (0.75 or 1.5 ml/kg, ip.) and chronic recipient of sesame oil (1.5 ml/kg/day, orally, 21 days). To induce seizure, strychnine was injected intraperitoneally 30 minutes after receiving saline, valproate or oil. Then seizure onset time and death time were recorded within 30 minutes.
Results: Acute injection of sesame oil increased seizure onset time and death time compared to control group but it was no significantly different. The chronic consumption of sesame oil significantly increased seizure onset time (p=0.029) in compared to control group, but there was no effect on the death time. Also, there were no significant differences in seizure onset time and death time between acute and chronic groups.
Conclusion: It seems that chronic consumption of sesame oil delayed the onset of seizure and reduced the kindled seizure acquisition.

Abstract
Background & objectives: Insulin-like growth factor -1 (IGF-1) has a variety of roles, but the abundance of scientific evidence indicates that it is a metabolic biomarker associated with physical fitness and health. The present study investigates the effect of eight weeks of polarized exercise training on serum GH / IGF-1- indices in active young men.
Methods: In this double-blind experimental study, 20 young males were allocated randomly into polarized training group (N=10) and a control group (N=10). The polarized training group performed 80-70% of the main workout volume (30 minutes) with light to moderate with 50-60% reserve heart rate (RHR) intensity and the remaining 20-30% at 85-95% RHR intensity; in a way that they ran two periods consisting 3 repetitions of 15-30 seconds, with 30-60 seconds of active rest after each repetition and 3 minutes of active rest after each period. Blood samples were taken from all subjects in three stages, including: pre-test stages, 24 hours before the start of the post-test, and after 12 hours overnight fasting. Post-test samples were collected, one sample immediately after the first session and the another 48 hours after the end of the last exercise session.
Results: The results of the present study showed that bipolar training significantly increased growth hormone and free IGF-I levels after one training session, and after eight-week bipolar training program. However, total IGF-1 levels decreased significantly after one exercise session and after eight-week bipolar exercise program. Also, no significant change was observed in IGFBP-3 and IGFBP-5 levels after one training session and eight-week training program. Acid-labile subunit levels did not change significantly after one training session, but decreased significantly after eight weeks of bipolar training.
Conclusion: Based on the results of the present study, it seems that the use of bipolar exercises, training may be a good way to improve the hormonal function and assess the level of health and physical fitness of active young men.

Abstract
Background & objectives: One of the main effective factors in combating gastric cancer is having good health literacy in the community. The aim of this study was to determine the health literacy of gastric cancer in clients and patients admitted to Imam Khomeini Medical Center in Ardabil.
Methods: The present study was a cross-sectional study. The sample of this study consisted of clients and patients admitted to Imam Khomeini educational -medical center in Ardabil in the second half of 2019. The sample size was 320 people which were randomly sampled. Researcher-made tools were used after ensuring validity and reliability. The questionnaire had two parts: demographic characteristics and items of health literacy dimensions. Patients' informed consent was obtained before completing the questionnaire. Data were collected by nurses and entered into SPSS-26 software. By using descriptive and analytical statistical parameters such as frequency and frequency percentage, mean and standard deviation, t-test and ANOVA analyzed.
Results: The mean and standard deviation of the age of participants was 42.98±14.35. The overall level of health literacy about stomach cancer was calculated as 31.13. The average gastric cancer health literacy in five dimensions in the clients and patients of Imam Khomeini educational-medical center in Ardabil was calculated as follows: access to health information resources was 38.02±10.6, reading health information was 13.47±4.41, understanding health information was 30.37±6.9, evaluation of health information was 16.73±4.58 and decision-making and application of health information was 58.49±10.77. Apart from reading in other dimensions of health literacy, no significant difference was found between clients and inpatients.
Conclusion: The results of this study showed that the mean score of health literacy of clients and patients admitted to Imam Khomeini medical center in Ardabil from gastric cancer is low and not sufficient. Considering the role of health literacy in energy storage, cost and time management of the individual's disease, it is necessary to plan and perform more efficient interventions to promote health literacy in the community, especially clients and patients admitted to Ardabil educational medical centers on gastric cancer.
