Abstract
Background & Objectives: Recurrent respiratory papillomatosis (RRP) is the most common benign laryngeal tumor and the second common cause for hoarseness in children. Despite its omplete benign histology, it has a great tendency to spread through the respiratory track and frequent recurrences may occur following surgery, possibly due to the virus survival in apparently healthy adjacent tissues. According to the most recent evident experiences, the treatment of choice for RRP disease is repetitive resections with CO2 laser. The present study evaluates the efficacy of adjuvant medical therapies for eradication of this disease in affected children. Methods : This was an experimental study with sequential control (self-control). The subjects were children who referred to ENT-H&N surgery dept. of Hazrat-e- Rasoul hospital in Tehran between Mar 1998 and Mar 2001 with chief complaint of hoarseness and respiratory obstruction. Among these children patients with confirmed diagnosis of RRP were selected as study samples and the effect of acyclovir and alpha-interferon combination in reducing the severity and recurrence of RRP was evaluated. Results: Totally 18 patients entered this study (10 (55.5%) females and 8 (44.5%) males). The mean age of the patients was 6.5 and 94% of them were the first child of young mothers (under 25 years of age). The signs of respiratory obstruction in most of the cases with a mild to moderate severity was the main cause of referral in 45% of the subjects. Laryngeal lesions were mostly superficial and limited to glottis region. Severity and spread of RRP disease was greater in children under 3, which led to tracheotomy in 3 of them. The combined administration of alpha-interferon and acyclovir, made a significant reduction in disease severity and spread in 55.6% of the patients. Conclusion : The findings of this study indicated that combination of acyclovir and alpha-interferon can be effective in reducing RRP recurrences and severity. Nevertheless, because of small sample size(due to rarity of RRP), Further multi-institutional studies are required to obtain a greater sample size and more valid results.

Abstract
Background & Objectives: Nowadays, appearance of ESBL producing bacteria is medical problem in the treatment of infections. Uropathogenic Escherichia coli like many other bacteria can produce these types of enzymes. T he assessment of the ESBL production by clinical isolates is not done routinely in laboratories. The aim of this study was to determine the prevalence of ESBL producing E.coli and its antibiotic resistance pattern in Kermanshah. Methods: This cross - sectional study was done on 200 Uropathogenic E. coli strains isolated from people in Kermanshah. Sensitivity of isolates to different antibiotics was determined by disk diffusion test and ESBL production was assessed by DDST method. Results: The E. coli strains showed high susceptibility to imipenem (100%), amikacin (97%), nitrofurantoin (95.5%), gentamicin (85%), cefepime (75%), ceftazidime (74%), ofloxacin (73.5%), ciprofloxacin, ceftriaxone and aztreonam (71%) and cefotaxime (70%) respectively. The highest resistance was seen to ampicillin (77%), carbenicillin (76%), pipracillin (74%) and SXT (62.5% ). Resistance rate to third generation cephalosporins was 63-75%. Fifty seven isolates (27%) were ESBL producers and 47 isolates (87%) produced all four types of ESBL enzymes. Conclusion: There are some similarities and differences in the antibiotic resistance pattern and ESBL production among the isolates in different areas of Iran and other countries. Identification of ESBL producing bacteria and determining its antimicrobial resistance pattern are recommended to effective treatment of infections.

Abstract
Background & objectives: Diazinon (DZN) as an organophosphate pesticide widely used in agriculture is associated with reducing the antioxidant capacity of the cell. Use of thiol compounds such as N-acetyl cysteine (NAC) as an antioxidant decreases oxidative stress in the cells. The aim of this study was to investigate the effect of NAC as an antioxidant against DZN- induced oxidative stress in rat brain and heart.
Methods: In present experimental study, male Wistar rats were randomly divided into four groups including: control group (corn oil as DZN solvent), DZN group (100 mg/kg), NAC group (160 mg/kg), and NAC+DZN, all of which were given intraperitoneally. 24 hours after injection, animals were anesthetized by ether, and the brain and heart tissues were quickly removed. After tissues hemogenation, superoxide dismutase (SOD), catalase (CAT), glutathione S-transferase (GST) and lactate dehydrogenase (LDH) activities, as well as glutathione (GSH) and malondialdehyde (MDA) levels were determined by biochemical methods.
Results: DZN increased SOD and GST activities and MDA level and decreased LDH activity and GSH content in brain and heart. Also, DZN increased CAT activity in the heart and increased it in the brain. Administration of NAC inhibited the change in these parameters.
Conclusion: DZN through free radical production leads to enhanced membrane lipid peroxidation, depleted GSH content and oxidative stress induction in the brain and the heart. Administration of NAC as antioxidant decreases the DZN-induced oxidative stress by scavenging free radicals and GSH synthesis, but its protection is not complete.
