Abstract
Background & Objectives: Goiter prevalence in school age children is an indicator of the severity of iodine deficiency disorders (IDDS) in the society and a goiter prevalence ≥5 % in school age children indicates a public health problem. In area of mild to moderate IDDS, measurement of thyroid volume by ultrasonography through observation is preferrable to population for grading goiter. Considering the importace of this issue, because of being mountainous and lack of this method's application for determining the incidence of goiter this study was desinged. Methods: In this descriptive – analytical study, thyroid volumes of 230 boys between 12 to 15 years old were measured by portable ultrasonograph in Tabriz. Also urinary iodine concentrations were determined by method A (Sandell-kolthoff reaction). Results: Mean of subjects’ thyroid volume was 8.12 ±2.21 ml and with latest references of Iran and WHO/NHD, goiter prevalence was taken based age 51.7 % and based surface body 81.1 %. Urinary iodine median of tested samples was 15.2µg/dl and iodine deficiency prevalence was 29.1 % . There was no significant correlation between urinary iodine and thyroid volume. Conclusion: Large thyroid volume of middle school boys in Tabriz is probably due to iodine intake deficiency in the first years of their life. Also, the role of goitrogenic factors and effect of climate condition on thyroid volume and goiter prevalence of middle school boys in Tabriz shouldn't be ignored. Further studies are recommended for determination of a local reference for thyroid volume Also it is necessary to be sure from consumption of iodine salt and its standardization.

Abstract
Background & Objectives: Uremia polyneuropathies especially peripheral neuropathy in chronic hemodialysis patients are very common and its orevalance different and reported between 18-80%. This study was designed to investigate prevalence of neuropathy and its relation ship with duration and adequacy of dialysis in Booali Hospital. Methods: This study was an analytic-descriotive study that has been done on 30 patients under hemodialysis and the examinations of which was done by neurologists. To analyze the KT/V for dialysis adequacy, a software from HDCN site was used EMG and NCV to the lower and upper parts were performed to all patients by a neurologist. The required tests were extancted from patients' serial tests. In this study patients with hepatitis C, D, diabetes and Amiloid were excuded from the study. Collected data were analyzed using descriptive statistical method in SPSS program. Results: EMG showed that 40% of patients had carpal tunnel syndrome (65.7% Mild, 13.3% Moderate, and 13.3% Sever in which then was no significant relationship between efficacy and time length. In 56.7% of patients KT/V had higher than 1.2 (Normal), 16.7% had KT/V between 1-1.2 (Acceptable) and 16.7% have KT/V less than 1 (Non Acceptable). Conclusion: Inspite of the new and improved method of hemodialysis, in comparson with the past the prevalence of neuropathy was higher in hemodialysis patients and there was no change in the prevalence of neuropathy. There is no significant relationship between dialysis efficacy and neuropathy. It seems that caulative factors neuropathy like urea were not prevent and they are made within the body.

Abstract
Background & Objective: Limited information is available on adolescent’s substance abuse in Iran. The aim of this study is to estimate prevalence of substance abuse among the sample of sophomore male students in Tabriz city, and to evaluate the associated factors. Methods: Of all sophomore male students in Tabriz, 1785(13.7%) were randomly sampled, and a self-administered questionnaire with 48 questions was distributed to students in February and March 2005. The questions aimed to obtain information on substance abuse, smoking, self-confidence, tendency toward smoking and risk-taking behaviors as well as demographic information. The influence of different factors on substance abuse was evaluated with a logistic regression model. Results: The mean age of the subjects was 16.3±0.87 (min. 15, max. 19) years. Among 1785 students 226(12.7%) had the experience of taking alcohol and 36(2.0%) had used substances. The results indicate that, older age (OR= 1.55), having general risk taking behavior (OR= 1.70), higher smoking stage (OR= 3.70), having masochism (OR= 1.22), high socioeconomic class (OR= 1.62), and use of illicit substances (OR= 5.72) were factors associated with prevalence of the use of alcohol. Also higher smoking stage (OR= 3.26), high socioeconomic class (OR= 1.52), and the use of alcohol (OR= 7.81) were factors associated with prevalence of the use of substances. Conclusion: This study showed the low prevalence of substance abuse, and determined some of its risk factors among students. This study may be supportive of interventions that target multiple behaviors. The findings of this study is important for public health officials however, more studies are necessary to generalize the findings.

Abstract
Background & Objective: Depression is a debilitating disorder with high prevalence especially in child-bearing women. This study was done to determine relationship between postpartum depression and its various risk factors. Methods: This was a descriptive-analytic study on 600 women referring to health centers in Tabriz. Data were gathered using a questionnaire including 5 parts: demographic characteristics, mother-newborn characteristics, Rosenberg self-esteem scale, Holms stress scale, Edinburgh Postnatal Depression Scale. These data were analyzed by descriptive and inferential statistical methods in SPSS 14/win software. Results: In this study 34.7% of mothers were depressed (scores>12). Significant risk factors for postpartum depression were: Marital dissatisfaction, relationship with mother, mother-in law and husband's family, job dissatisfaction, house condition, having an unpleasant pregnancy experience, unplanned pregnancy, baby care stressors, infant feeding method, nursing problems, mother's self-esteem and stress level. Conclusion: Concerning high prevalence of postpartum depression in this study and other similar studies and effects of some factors in this disorder, it seems to be necessary to improve the knowledge of mothers and health care staff about this risk factors for reducing effects of this disorder on physical and psychological health of newborns, mothers and community.

Abstract
Background & Objectives:Urinary tract infection (UTI) ranges from asymptomatic bacteriuria to severe renal infection with sepsis. Urinary tract infection can cause many complications as abscess, hypertension, renal failure, renal scar, reflux and so on. Since there is no general agreement on the administration of additional fluid (1.5 times maintenance) on the early treatment of UTI on this study was conducted to see the effect of additional fluid. Methods:206 children with UTI who were admitted in Amir Kabir Hospital of Arak from March 2005 to March 2006, were divided into two groups of 103. One group received the usual amount of maintenance fluid and the other 1.5 times of maintenance. The course of resolution of dysuria, frequency, malodorous urine, abdominal pain and fever were compared in the two groups. Patients' urine culture was performed on the second day, 7-10 days, and 90 days after admission. Patients were matched according to their age and sex. Those who presented with azotemia were excluded from the study. Results: Receiving additional fluid had a significant effect on the malodorous urine dysuria. However, it had an inverse effect on the treatment of fever and urinary frequency and it made them even last longer. In other areas like abdominal pain and urine culture in 3 stages there was no difference. Conclusion: It seems that intake of excessive amounts of water has no significant effect on the results of the UTI treatment.

Abstract
Background & Objectives: Pregnancy is one of the most sensitive periods of lifetime and married life and sexuality may bring about numerous physical and psychological changes. Lack of attention to sexual matters during pregnancy may cause sexual dysfunction in each of couples. The aim of this study was identifying male sexual behavior and its relevant factors during the partner’s pregnancy. Methods: This was a descriptive-analytical study in which 350 men, whose wives were pregnant, were selected in two stratified stages. The instrument for data collection was Arizona Sexual Experiences Scale- male questionnaire. All data were analyzed by SPSS11.5. Descriptive information was used for describing the frequencies and also the x2 test was used for determining relation ship among variables. Results: The findings revealed that 21.3% of men in the first trimester, 19.3 in the second and 28.3 in the third trimester had sexual dysfunction. In each trimester, inability to reach orgasm was the most common sexual dysfunction in men. Frequency of vaginal coitus decreased in 64.9% of men and in 46.6 % of them fear from risk for fetus health during vaginal coitus was the essential reason for this reduction. There were significant differences between sexual dysfunction and fear from risk for fetus health and guilt feeling of sexual activity. Therefore decreased vaginal coitus caused sexual dysfunction. Conclusion: As men’s knowledge and attitudes about sexuality affect their sexual behavior during pregnancy it is crucial to provide proper consultation regarding sexual relations in prenatal care services.

Abstract
Background & Objectives: Neonatal mortality rate is a main indicator of hygienic development and recognition of the mortality causes is the first step for reduction of the mortality rate and promotion of this indicator. Therefore, in this study, the causes of neonatal mortality have been investigated in Ardabil medical university related hospitals. Methods: This study is a retrospective, cross-sectional and descriptive-analytical one that has been done on the bases of admission's units and filling the medical record form for all neonates who have been admitted to Alavi and Imam Khomeini hospitals in Ardabil from September 2006 to September 2007. The questions of forms consisted of infant’s sex, birth weight, gestational age, cause and age of death. The data were analyzed by SPSS software with utilizing T- test and Chi-Squares. Results: During this study, there were 1881 admissions of which there were 148 deaths (7.86%). The causes of mortality were hyaline membrane disease (HMD) (52.02%), sepsis (14.86%), pneumonia (9.45%), congenital anomaly (12.83%), asphyxia (7.43%) and meconium aspiration (3.37%). Among the dead neonates, 54.72% were male and others were female. There were 550 LBW neonates and 111 deaths in this group result of our study showed that mortality rate in LBW neonates was 20.18%. There were 578 preterm neonates and 122 deaths in this group prevalence of mortality rate in preterm neonates was 21.10%. Conclusion: Prematurity and low birth weight are the most important and fixed cases of infant mortality. The most common causes of mortality were hyaline membrane disease and sepsis. Therefore, the prevention of preterm labor for reduction of prematurity and its related complication recommended which may lead to decreased rate of neonatal mortality.

Abstract
Background & Objectives: Postpartum depression is one of the most important problems during post partum. Several new medications have been introduced for treatment, but considering their side effects and also breast feeding women's desire for dietary complements rather than chemical drugs, this research was done to determine comparison of effectiveness of Omega-3 fatty acids with placebo in treatment of mild to moderate postpartum depression. Methods: This double-blind randomized placebo controlled trial study was done on 120 women with postpartum depression, who had the required criteria. First by using Edinburgh Postnatal Depression Scale (EPDS) in women who gave birth 2 weeks to 3 months before, postpartum depression approved for determining the severity of depression, Beck Depression Inventory scale (BDI) was used. Women with mild to moderate depression who had a score 46 on the (BDI) and did not have any tendency to use anti-depressant drugs under the supervision of the psychiatrist, enrolled in the study and randomly assigned to receive either 1gr of Omega-3 capsules or placebo for 8 weeks. Severity of depression was measured before treatment and weekly during treatment in both groups. The data analyzed through T-Test, repeated measurements of one way ANOVA and chi square test in SPSS 14/Win. Results: There were no significant differences between two groups with respect to demographic characteristics. Results show that Mean Depression Scores before treatment in Omega-3 group (35.4 ± 9.2) decreased after treatment (17.7 ± 7.0), which was significant (p<0.0005). Mean Depression Scores before treatment in placebo group (34.2 ± 3.4) decreased after treatment (33.6 ± 9.3) which wasn't significant (p=0.57). There was a significant difference between reductions of Depression Scores in two groups(P<0.001). Conclusion: According to research results, use of Omega-3 1gr/day for 8 weeks improves postpartum depression.

Abstract
Background and Objective: Staphylococcus aureus is known as an important pathogen causing a variety of bacterial infections. Treatment of this bacterium with antibiotics has led to antibiotic-resistancey, especially against methicillin (MRSA) and more recently rare resistance against vancomycin. The aims of this study were to determine nasal carriage rates of S. aureus in Meshgin Shahar Valiasr hospital’s personnel and to determine antibiotic-resistance patterns in the mentioned isolates. Methods: Staphylococcus aureus isolates were collected from the nose of 200 hospital personnel in Meshgin Shahar Valiasr hospital in a 2 month period in 2006. Antibiotic sensitivity of the collected strains were tested against antibiotics used in routine treatment of S. aureus infections. Oxacillin agar was also used to screen for MRSA according to NCCLS recommendation. Results: Our results showed there were 45% and 16% nasal carrier rate for S. aureus and MRSA (methicillin resistant staphylococcus aureus) strains, respectively in hospital personnel. Thirty two isolates were able to grow in oxacillin agar media, indicating 35% MRSA strains. Antibiotic resistant pattern of strains in disks method were recorded as follows: 35% to oxacillin, 97.8% to penicillin, 34% to erythromycin, 2.1% to chloramphenicol, 39.36% to tetracycline, 11.7% to gentamicin, 30.85% to trimetohoprim sulfamethoxazol and 19% to clindamycin. All of the isolates were sensitive to vancomycin and ciprofloxacin. Conclusion: In this study, nasal carriage rate of Staphylococcus aureus among hospital Personnel was more than community expected rate (%40) and lower than hospital expeeted rate (%50-80). All of the test strains were sensitive to Vancomycin.

Abstract
Background and objectives: Common cold is the most common disease in children. Young children have an average of 6 to 7 colds each year. Some complications of common cold include: secondary bacterial infections, school absence and excessive cost for treatment. The management of the common cold consists primarily of symptomatic treatment. An effective and safe drug is needed to reduce the duration of common cold ۥ s symptoms in children and reduce subsequent complications. In some studies zinc sulfate has showed an antiviral effect and immune enhancing activity. Therefore this study designed to evaluate the efficacy of zinc sulfate in duration of cold ۥ s symptoms in children. Methods: This was a clinical trial study. The subjects were children between 1 - 7 years old with common cold that attended to pediatrics clinic of Arak Amir Kabir Hospital. The study included 112 patients randomized in two groups. One group received zinc sulfate within 10 days and other group didn’t receive this drug. Using a researcher made questionnaire duration of symptoms: rhinorhea and nasal obstruction, cough, sneezing, fever and duration of illness, in both groups were evaluated. Results: Consumption of zinc sulfate significantly reduced the mean duration of rhinorhea and nasal obstruction in children with common cold (p < 0.05). Also the mean duration of cough in zinc group was lower than the control group (p < 0.05). Compared to the group that didn’t receive zinc sulfate, the zinc group had shorter mean duration of sneezing (p < 0.05). Also the mean duration of fever in zinc group was shorter than the other group (p < 0.05). The mean duration of illness in patients who had received the zinc sulfate was significantly less than the other group (p <0.05). No side effect was observed in subjects received zinc sulfate. Conclusion: According to the results zinc sulfate can be administered with other supportive treatments of common cold, for reduction of symptoms duration and complications in children.

Abstract
Background & objectives: Pre-eclampsia, as a idiopathic disease, is one of the most important causes of maternal and fetal morbidity and mortality throughout the world. Deficient or excessive level of trace elements can be considered as an unfavorable factor in pregnancy.The aim of this study was to evaluate copper status in pre-eclamptic women . Methods: 95 pre-eclamptic and 92 healthy pregnant women were enrolled in a case - control study .The sera and 24-hour urine copper of two groups were measured by atomic absorption spectrometry. The data were recorded and analyzed statistically by using t-test, X2, and pearson's correlation . Result: Copper concentration of serum and 24-hour urine in pre-eclamptic patients were significantly higher than normotensive ones (p<0.000). Intensity of pre-eclampsia did not correlate with copper concentration in the serum and 24-hour urine . Conclusions: The results suggest that the high level of copper in maternal serum and urine is in correlation with pre-eclampsia but pre-eclampsia intensity have not significant effects on copper levels. However, further studies would be helpful for this test to be used as a prognostic method for pre-eclampsia.

Abstract
Background & objectives: Screening and early diagnosis of delayed development in children particularly among high risk groups are obligatory. Children with a history of one or more risk factors in pre-natal, peri-natal and post-natal periods are prone to delayed development in comparison with normal population. This study was launched to assess the developmental status in children with a history of NICU admission. Methods : In this study 114 children, aged between 4-60 months with a history of NICU admission were recruited. ASQ questionnaires including information in 5 fields: communication, fine and gross motor status, personal-social and problems solving, were filled for each one of the cases by their parents. The data were analyzed using descriptive statistics and Chi square test. Results: Among 114 cases, there were 71.1% male. Totally 70.2% of cases were premature. The most prevalent reason for admission was immaturity. The average weight of infants was 2233 ± 680 gram. Figures of abnormal findings in 5 fields were: communication 20.2%, gross motor 17.5%, fine motor 19.3%, problem solving 8.8% and personal-social 16.7%. In this study 67.5% of children showed normal development while 6.1% were abnormal in all 5 fields and 26.3% were abnormal in at least one filed. Statistically, there weren’t significant differences between various developmental fields and genus, length of hospital stay and birth weight. Conclusion : Despite normal developmental status in majority of children with a history of NICU admission, a considerable number of them had delayed development in at least one field. These findings suggest a need for patient screening and intense follow up programs.

Abstract
Background & Objectives: Bipolar disorder in children and adolescents is a childhood critical disorder with negative course and outcome consequences. The aim of this study was determination of six-month outcome predictor factors (recurrence rate, the rate of hospitalization, severity of illness and recovery rates) in manic and mixed bipolar disorders of children and adolescents admitted in Tehran Rozbeh hospital. Methods: In this prospective Cohort study, 80 patients with bipolar disorder (10-18 year's old) admitted in Tehran Roozbeh Hospital from January 2009 to July 2010 were selected. The available sampling method was used for selection. Participants at admission, discharge, and follow-up at 3 and 6 months, were evaluated by using researcher made questionnaires, K-SADS (to confirm the diagnosis), CDI ( Children Depression Inventory) or BDI ( Beck Depression Inventory) , YMRS ( Young Mania Rating Scale) , CGI- S ( Clinical Global Impression Scale) , CGAS ( Children's Global Assessment Scale), CGI-S ( Clinical Global Impression Scale) and PAS ( Premorbid Adjustment Scale) . The data were analyzed by Pearson correlation coefficient and Multivariate regressions . Results: The results showed that disease outcome was not associated with age. Gender (male) had correlation with mania severity in 6-month follow-up. Disease duration predicted recurrence rate and severity of disease. Manic type disorder was related with mania severity, and mixed mania predicted mania severity negatively in 6-month follow-up. Therapeutic compliance was correlated with mania severity (negative correlation) and improvement rate (positive correlation). Presence of psychosis was correlated with recurrence rate positively in 6-month follow-up. Co-morbidity with ADHD ( Attention Deficient /Hyperactivity Disorder) predicted clinical global improvement (CGI-G) negatively and mania severity positively in 6-month follow-up. Pre-morbid coping showed negative relationship with mania severity and positive relation with global improvement rate in 6-month follow-up. Conclusion: Our results showed that gender of patients, duration and manic type of disease, presence of psychosis at admission have a direct relation with inappropriate outcome of manic and mixed bipolar disorders of children and adolescents. These findings emphasize necessity of special curing during treatment.

Abstract
Background & objectives : Amikacin is one of the key second-line drugs for treatment of tuberculosis. Mutations at the codons 1400, 1401and1483 of the 16srRNA gene are associated with resistance to amikacin. The purpose of this study was to detect these mutations using PCR-RFLP method in multi-drug resistant (MDR) strains of Mycobacterium tuberculosis showing resistance to amikacin. Methods : Susceptibility of strains (n=100) against first and second–line anti-tuberculosis drugs was performed by proportional method. Based on antimicrobial resistance pattern 97 strains were analyzed by PCR-RFLP method. rrs1096 and rrs1539 primers were used to amplify a 460bp region of the rrs gene. Then, the PCR products were digested using Tai 1 and Dde1 restriction enzymes. The results were analyzed by the SPSS software using Chi-square test. Results : Based on results from proportional method, 63 strains (64.9%) were MDR (Multiple Drug Resistant), 26 (26.8%) and 8 (8.2%) strains were susceptible and non-MDR, respectively. Also, 13.4% and 6.1% of the strains were XDR (Extensively Drug Resistant) and TDR ( Totally drug resistant ) respectively. Using PCR-RFLP method, 7 (7.2%) strains were resistant and 90 (92.7%) strains were susceptible to amikacin respectively. Moreover, we found that the mutation at the codon 1400 was the most frequent mutations responsible for resistance to amikacin. Conclusion : The PCR-RFLP method can be used as a supplemental method to detect resistance to amikacin however to increase our knowledge, mutatuions in several number of codons in rrs gene need to be studied.

Abstract
Background & objectives: Hydaticosis is a zoonotic helminthic disease of human and other intermediated hosts in which larval stages of the tapeworm Echinococcus granulosu transfect human. The liver and lung are the host tissues for the hydatid cyst . It is unknown which mechanisms are involved in infertility of the cyst and suppression of the fertile cyst. This study was aimed to evaluate the expression of the apoptosis inducing-ligands such as TRAIL and Fas-L in germinal layer of the cyst and human normal tissue surrounding the cyst that is one of the unknown host innate immunity mechanisms against the hydatid cyst. Methods: In this study, four isolated hydatid cysts were used which had been diagnosed in patients by radiography and parasitological examination in Mashhad Ghaem hospital. Furthermore, the germinal layer of the cyst and accompanied normal peripheral tissues were separated by scalpel in sterile conditions. After homogenization, expression of TRAIL and Fas-L genes were studied by semi-quantitive RT-PCR method. Results: The TRAIL and Fas-L showed significant higher level expression in germinal layer of infertile cyst than the fertile cyst and host normal tissues. Conclusion: The host tissue-induced apoptosis of germinal layer of the fertile cysts is probably one of the infertility mechanism in patients with hydaticosis

Abstract
Background & Objectives: Ethambutol is one of the four main drugs in treatment of tuberculosis. The most common mutation associated with this drug resistance usually occurs in codon 306 of embB. The aim of this study was to detect ethambutol resistance using Allele-Specific PCR and Spoligotyping in various subtypes of Mycobacterium tuberculosis. Methods : 140 sputum specimens were collected from suspected TB patients. They were digested and decontaminated using Pettrof method before culturing them on LJ medium. Drug susceptibility testing was performed on 106 culture positive specimens using proportional method. DNA was extracted from the isolated organisms and subsequently subjected to Allele-Specific PCR to detect any mutationin embB306. Spoligotyping was then used to determine the subtypes. Results: Out of 106 cultures positive samples, 36 samples (33.9%) showed resistance to ethambutol using proportional method. Allele-Specific PCR assay identified 93 as sensitive and 13 (27.6%) as resistant strains. The results of PCR were in agreement with result of proportional method. The PCR method revealed that 61.5% of mutation occurred in the first and 38.5% in third nucleotides. Spoligotyping differentiated Mycobacterium tuberculosis strains into Beijing (10 9.4%), Bovis (2 1.8%), CAS (24 22.6%), EAI (1 0.9%), Haarlem (27 25.4%), LAM (5 4.7%), Manu (5 4.7%), T (27 25.4%) and U( 2 1,8%) families. The high frequency of mutation in embB gene was belonged to Haarlem, CAS and T subfamilies. Conclusion: Based on results current study, mutations in the genes other than embB might have occurred in the resistant strains that gave negative result in Allele-Specific PCR assay. Therefore other mechanisms of resistance to this antibiotic should be investigated.

Abstract
Background & Objectives: Food consumption for its hedonic properties can result in caloric intake exceeding requirements and is considered as a major culprit in the rapidly increasing rate of obesity. We compared the dietary patterns and the hedonic questionnaire score in obese and non-obese women. We also investigated the relation between the score of this questionnaire and variety of food items. Methods: This case-control study was conducted on 140 women aged 17-44 years that had referred to obesity club at district 4 of Tehran in 2011. Food intake and hedonic score were determined using validated semi-quantitative 168-item food frequency questionnaire and 21-item hedonic questionnaire, respectively. Logistic regression was used to determine the association between obesity and the dietary patterns identified by factor analysis and hedonic hunger score. Results: Two dietary patterns were identified as healthy and unhealthy patterns. The adjusted odds ratio of obesity didn’t show any significant trend in none of the dietary patterns. The mean hedonic questionnaire score was higher in case than control group, significantly (p<0.05). The adjusted odds ratio of obesity, for the second and the third tertiles of this questionnaire versus the first tertile of this, was 2.8 and 10.8, respectively (p<0.05). Conclusion: This study shows that there is no difference between the dietary patterns of obese and nonobese women while there is a positive association between hedonic hunger and obesity. It seems that hedonic hunger may lead the obese women to consume unhealthy food choices.

Abstract
Background & Objectives: Elevated apolipoprotein B-100 and decreased Plasma HDL due to hypertriglyceridemia are common abnormalities in insulin-resistant subjects that increase the risk of cardiovascular disease. The aim of this study was to investigate the effects of 8 weeks progressive aerobic exercise (AE) on blood glucose, lipidand lipoprotein profile, and aerobic power in adults with type 2 diabetes. Methods: Twenty nine women with type 2 diabetes (mean age 43.4 ± 1.7 years and blood glucose range of 150 to 230 mg/dl ) were divided into a control (n=14) and exercise (n=15) groups. Exercise group participated in an aerobic training program 3 times per week for 8 weeks. The intensity of aerobic exercise was 60–75% of HR max. The control subjects were asked not to alter their physical activity patterns. All subjects underwent anthropometric, lipid profile and aerobic power evaluation, at the baseline and 48 hours after last session training. Following a 12 h overnight fasting, a venous blood sample was drawn for the analysis of total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), low-density lipoprotein (LDL), and (apoB100). Paired t test and analysis of covariance (ANCOVA) were used to analyze the data. Results: Significant improvement was observed in FBS, aerobic power, percent body fat, (Apo) B-100 and triglycerides (p< 0.05) for the exercise compared with the control group after training. No significant difference was observed in TC, LDL and HDL between two groups. Conclusion: According to our findings moderate intensity physical exercise can be effective in preventing the atherosclerosis via significant improvement in blood sugar, cardiovascular endurance, Apo B-100 and triglyceride in patients with type 2 diabetes.

Abstract
Background and Aim: Birth in the 21st century is characterized by interventions. That interventions increase the risk for mother and baby without improving outcomes. The aim of this study was to compare the effect of physiological birth and routine normal delivery on some of maternal and fetus outcomes. Methods: This quasi-experimental study was performed on 160 pregnant women. The participants were allocated in two groups of physiological birth (n=80) and routine normal delivery (n=80). Inclusion criteria were as following: Apgar score between one and five dilatation between three and four cm maternal age between 18-35 years gestational age between 37-40 weeks cephalic presentation and neonatal birth weight of 2500-4000 gr. Physiological birth care included labor begins on its own, freedom of movement throughout labor, continuous labor support, spontaneous pushing, no separation of mother and baby. Routine birth cares include routine interventions in labor or birth. Rate of cesarean, Apgar score and neonatal intensive care unit were evaluated in both groups. Data were analyzed by SPSS 16. The t-test, chi-square and Mann Whitney were the statistical tests of choice. Results: Rate of cesarean was significantly different between two groups (p=0.005). First minute Neonatal Apgar score was significantly different (p=0.011), but fifth minute Apgar was similar in both groups (p=0.470). The transmission to neonatal intensive care unit had also significant difference among two groups (p=0.029). Conclusion: The present study showed that the physiological birth is safe. It seems that the physiological birth decreases the rate of cesarean and transmission to neonatal intensive care unit, also improves the first minute neonatal Apgar scores.

Abstract
Background & objectives: Trichophyton rubrum is one of the most common pathogeniccause of dermatophytosis. One of the drugs which have been prescribed widely for fungal infections is terbinafine which belongs to allylamines group of antifungal agents. Recently molecular typing methods have been developed for answering the epidemiological questions and disease recurrence problems. Current study has been conducted on 22 isolates of Trichophyton rubrum obtained from patients randomly. Our aim was the investigation of correlation between genetic pattern and sensitivity to Terbinafine in clinical isolates of Trichophyton rubrum. Methods: Firstly the genus and species of isolated fungi from patients have been confirmed by macroscopic and microscopic methods, then, the resistance and sensitivity of isolates against drug have been determined using culture medium containing defined amount of drug. In next step fungal DNA has been extracted by RAPD-PCR (random amplified polymorphic DNA) with random sequences of 3 primers. Results: Each primer produced different amplified pattern, and using each 3 primers differences have been observed in genetic pattern of resistant and sensitive samples using each 3 primers, but there was no bond with 100% specificity. Conclusion: The 12 sensitive isolates which didn’t grow in 0.1 mg concentration of drug, also had limited growth at the low concentration of drug. Ten resistant isolates which grew in 0.1mg/ml of drug, in lower concentration of drug were resisted. RAPD analysis for molecular typing of Trichophyton rubrum seems to be completely suitable.
